Treating Posttraumatic Stress Disorder
This manual is for licensed mental health providers who provide psychotherapy and related services. Treating clients suffering from posttraumatic stress disorder and related disorders can be challenging, even for a seasoned practitioner. This is partially due to the fact that posttraumatic stress disorder is about something. The reasons for your client'sdistress are known. Your client has experienced trauma, and the symptoms are an expression of, and the result of, the traumatic experience. When a client presents with symptoms of posttraumatic stress, there is always a difficult story waiting to be told. And your client may or may not be ready to tell it. And you, the practitioner, will need to hear the story. This, too, can be difficult. Traumatic experiences may call into question, both for the practitioner and the client, everything they believe to be true: That people are inherently good. That good things happen to good people. That we can protect ourselves and our families from harm. That life is fair. Traumatic experiences happen in spite of us believing that they should not. And more often than not they don't just happen. Much trauma is related to what people do to each other. Children are hurt by their parents. Spouses hurt each other. In other words, there is often an interpersonal component to traumatic experiences. This can make recovery from Posttraumatic stress disorder difficult. If your client has beenhurt by someone close, it can be difficult to enter into a new relationship-and that includes the therapeutic relationship. For the treatment provider, this can be frustrating. You may know exactly what you need to do but find that your client can't let her guard down. Also keep in mind that clients who come from historically marginalized populations-such as those subjected to broad-scale, historical trauma-may have suffered relational (and actual) injuries that can make it very difficult for them to believe that any one person or institution can be trusted at all. Schools, when they were able to remain open, quickly shifted to adopt untested virtual instruction methods that varied widely in implementation. Youth were exposed to increased stress at home, including adverse childhood experiences (ACEs), and mental health conditions increased. Young children who needed support for developmental and learning delays, as well as disabilities, missed critical interventions and/or entered school later. Older adolescents were lost entirely from the educational system. Although the acute phase of the pandemic has passed, the significant developmental and psychosocial impact on this generation of young people will be felt for decades to come, especially as resources have not yet been allocated in a manner that effectively and aggressively intervenes on behalf of those most impacted. Academic achievement data has shown significant attenuation in math and reading gains. The mental health system is presently unable to meet the demands of the population, prompting prominent national health care provider associations and other experts to declare a state of emergency for youth mental health. There are significant implications of changes in academic achievement for the identification of disability using pre-pandemic methods, especially for the fields of forensic neuropsychology and special education law. Radical educational and psychosocial changes during the COVID-19 pandemic may impact neurodevelopment from birth through the transition to early adulthood, with lasting impacts on psychological and social functioning, as well as academic achievement, especially for vulnerable youth. Rumination syndrome (RS) is a condition characterized by repeated, effortless regurgitation of material (usually food). RS is unfortunately an overlooked (and thus under-treated) condition, as individuals with RS may often describe their symptoms as "reflux" or "vomiting." RS can also co-occur with other gastrointestinal and eating disorders, further complicating the clinical picture. The good news is that RS can be successfully treated. Melancholy and Memory
This study is a reflection on the ambiguity of melancholic states, which are vectors for creation, but which also hinder the evocation of life events due to ruptures in memory chains. This second strand has been the subject of great controversy in the theoretical and clinical spheres. We begin with Aristotelian thinking about why exceptional men, geniuses, poets, scientists, among others, go through melancholic states, but who, in extreme situations, resort to creative outlets. A distinction is made between the forms of suffering resulting from loss, emphasising the aspect of melancholy as a gateway to creation. We understand that there is an oscillation in man who goes through an experience of melancholic states, in which there is a space where, at one extreme, there is misery, depreciation and ruin; at the other, there is splendour, to the extent that there is creation with the potential for elaboration. We believe that man, pressed by melancholic states, can leave testimonies of his passage through the state of darkness.
