Immediate and Delayed Drug Therapy Effects on Low Dose Sarin Exposed Mice Myocardial Performance
Recent studies have shown that a single asymptomatic dose exposure to the nerve agent sarin can lead to long term cardiac dysfunction. This study looked at immediate and delayed treatment therapies post exposure on cardiac function. The mice were studied using electrocardiography (QTc, STc, and Tc intervals) and histological techniques (hematoxylin and eosin staining and brain natriuretic peptide (BNP) staining). Male C57BL/6J mice were injected with sarin (0.4LD₅₀) and studied up to 11 weeks. Mice which were given a single dose of the nerve agent treatment atropine/2PAMCL, showed significantly higher cell size and BNP levels than the control (p = 0.001) and increased QTc and Tc intervals, all of which are indicators of cardiac insult . Mice which received drug treatments after the onset of cardiac remodeling showed increased (thus worsening) normalized heart size for isoproterenol (β agonist) treated mice (p = 0.0018) versus saline treated mice along with inverted T-waves pre cardiac stressing. Propranolol (β blocker) treated mice showed only T-wave depression and no difference in normalized heart size versus the saline group. Pharmaceutical Surge Requirements for the Prime Vendor Program-Europe in Support of Military Operations Other Than War
The U.S. military in Europe often provides medical support to military operations other than war due to unstable situations in that region of the world. The increasing support to peace operations and humanitarian assistance have led the military medical units in Europe to reexamine the pharmaceuticals traditionally maintained to support surges. This research was a qualitative, exploratory study using multiple-case study strategy to identify the specific pharmaceuticals utilized in the initial stages of military operations other than war primarily peacekeeping, peace enforcement and humanitarian assistance. The operations chosen as the units of analysis were Operation Provide Promise (Croatia), Operation Support Hope (Rwanda), and Operation Joint Endeavor (Bosnia-Herzegovina). The results of the research show that some unique surge requirements exist for military operations other than war. Taking advantage of existing clauses in its Prime Vendor contracts, the military medical community in Europe can position itself to adequately support surges for MOOTW. An Update on Mechanisms and Treatments for Thyroid Cancer in Current Oncology
The incidence rate of thyroid cancer (TC) has risen sharply in recent decades on a global scale. It is the most common malignant tumor of the endocrine system, accounting for 4.1% of all new cancer cases worldwide, with age-standardized incidence rates (ASIRs) of 9.1 per 100,000 in 2022, establishing itself as a significant threat to population health. Although TC generally exhibits a better prognosis compared to other cancers, poorly differentiated thyroid cancer (including poorly differentiated TC, PDTC and anaplastic TC, ATC, two subtypes of TC) displays the shortest average survival time. The precise cause of TC's high incidence remains unclear, particularly the molecular mechanisms underlying its development. Consequently, the primary approaches for treating TC involve surgery, often supplemented by 131I therapy, radiotherapy, endocrine therapy, and biomarker-targeted therapy. Unfortunately, these treatments, except for targeted biological therapies, often result in irreversible harm to patients, such as thyroidectomy or radiation injuries. While targeted therapy can be expensive, and current TC biomarkers remain limited, their effects are still being explored. Additionally, aside from surgical resection, alternative treatment methods remain subject to ongoing controversy.

Reactive oxygen species (ROS) are essential in regulating various physiological functions. However, the overproduction of ROS can be related to many diseases. Antioxidant administration represents an effective strategy to treat oxidative stress. In recent decades, nanotechnology and nanoparticles have received great interest and seen applications in many fields, from pharmaceutics to cosmetics and food. For instance, lipid nanoparticles, due to their "green" characteristics related to their biodegradation and non-toxicity, have received significant attention as a carrier for antioxidant molecules. Over the past few decades, significant achievements have been made in the development of antioxidant nanotherapies, exhibiting multiple advantages.This Special Issue collects recent advances in the design, production, characterization, use, and effect of antioxidant nanoparticles, including polymeric or lipid-based nanoparticles, ROS-scavenging inorganic nanoparticles, organic nanoparticles with intrinsic antioxidant activity, and drug-loaded antioxidant nanoparticles, to be administered by different routes. Idiopathic Parkinson's disease is generally described as a neurodegenerative pathology characterized by a progressive loss of dopaminergic neurons in the substantia nigra, responsible for impaired motor functions. Despite significant advances in understanding its biological mechanisms, a significant proportion of cases remain without a clearly identified organic explanation, which raising the question of other etiological avenues.In this context, the clinical work of Dr. Janice Hadlock makes an original contribution by suggesting the existence of a functional dysfunction of the dopaminergic system, not related to irreversible neuronal degeneration, but potentially reversible. This hypothesis, resulting from the observation of numerous cases in his clinical practice, calls into question the irreversible evolution of idiopathic Parkinson's disease and paves the way for a redefinition of clinical subtypes.This gives many patients hope of belonging to a reversible subtype.However, these observations have not yet been corroborated by a large-scale study, which limits their scientific significance. An in-depth multicenter survey would not only make it possible to estimate the prevalence of this functional form, but also to validate or refute the diagnostic criteria and therapeutic strategies it recommends. In the absence of such data, the distinction between organic Parkinson's and functional Parkinson's remains difficult to establish, but is plausible as also suggested by some scientific publications. The neurophysiological study of functional parkinsonian syndrome gains in depth when considering the autonomic mechanisms of the nervous system, particularly those related to fear and archaic reflexes. Development of Fenoprofen Calcium Tablets for Colon Targeting
Objective: The objective of the present investigation was to development of enteric coated tablet of fenoprofen calcium. Methods: The 32factorial design was used to preparation of compression‐coated with a mixture of time dependent hydrophilic swellable polymer hydropropylcellulose (HPC), time released polymer Ethyl cellulose (EC) and pH responsive soluble polymer Instacoat EN Super II. The developed formulations were characterized for angle of repose, density, carr's index of powder mixture and thickness, diameter, hardness, friability, weight variation, uniformity content, in vitro disintegration, dissolution study. Results: The results showed that disintegration time and dissolution study of inner core tablet found to be in the range of 7.15 to 13.34 min and 95.92簣0.011% at 30 min. The press coat tablets at optimum concentrations of hydroxy propyl cellulose (200 mg) and ethyl cellulose N-22 (200 mg) showed the % drug release 83.53簣0.014% at 12 hr. % cumulative drug release of enteric coated tablet of optimized batch FT9 showed 76.08簣0.045 at 12h, it indicated that drug release at sustained manner.

Optimization of Therapeutic Strategies for Organophosphate Poisoning
The National Preparedness Vision requires the U.S. be prepared to prevent, protect against, respond to, and recover from all hazards associated with a chemical attack. Results of this study demonstrate that we cannot protect service members and first responders as required following a nerve agent attack. The research presented herein aimed to construct a physiologically based pharmacokinetic model to determine optimal therapeutic strategies for organophosphate (nerve agent) poisoning. The constructed model integrated organophosphates and two antidotes, atropine and oximes. Model results reasonably mirrored literature data and anecdotal observations of organophosphate poisoning. Results suggest a symptoms-based dosing strategy of atropine and a time-based dosing strategy of oximes. For patients severely poisoned with organophosphorus nerve agents, model results support documented claims of oxime's inefficacy and tendency to heighten the severity of poisoning. The results strongly indicate that military personnel attacked with nerve agents are at a significant health risk if they employ their prescribed treatment as current doctrine dictates. USAF Civil Reserve Air Fleet Aeromedical Evacuation Airlift Capability
The Department of Defense, USAF, and Air Mobility Command (AMC) rely on the Civil Reserve Air Fleet (CRAF) to provide supplemental airlift in times of national emergency. AMC oversees the CRAF program which contains three segments: international, national, and aeromedical evacuation. The CRAF Aeromedical Evacuation (AE) program involves commercial B-767 aircraft, Aeromedical Evacuation Shipsets (AESS), a Casualty Transfer System, and a large number of support personnel and equipment. Wartime planning in the early 1980s revealed a significant shortage of military airlift to accomplish the aeromedical evacuation mission. Consequently the CRAF AE program was initiated. Planning for Major Regional Conflict scenarios has currently set a requirement for 25 and 44 B-767 aircraft for CRAF Stages II and III, respectively. The provision of military medical care across the Department of Defense stands on the cusp of immense change, especially the Pharmacy benefit. Changes in civilian and military policies and procedures, escalation in pharmacy benefit utilization, and increasing medication costs are transforming pharmacy benefit structure and the face of the personnel that provide the pharmacy benefit. Medical breakthroughs are expensive and the DoD, along with the entire US health care system, must astutely manage health care expenditures. This research paper will explore the issues affecting the provision of the DoD pharmacy benefit and attempt to determine the most cost-effective venue for providing this benefit. The primary focus will be the differences between the Military Treatment Facility (MTF) and Tricare Retail Network Pharmacy (TRRx). The Tricare Mail Order Pharmacy (TMOP) is near cost neutral to the MTF, (because the dispensing fee is partially offset by the $3/$9 co-pays) therefore it is excluded from comparisons in this paper. The TMOP's only utility is refilling maintenance medications. It cannot be used to fill acute medication prescriptions like antibiotics. Data from literature sources, the DoD Pharmacoeconomic Center (PEC), and the Pharmacy Data Transaction Service (PDTS) will be used in a retrospective methodology to estimate the most cost-effective venue for providing the DoD pharmacy benefit. Optimization of Therapeutic Strategies for Organophosphate Poisoning
The National Preparedness Vision requires the U.S. be prepared to prevent, protect against, respond to, and recover from all hazards associated with a chemical attack. Results of this study demonstrate that we cannot protect service members and first responders as required following a nerve agent attack. The research presented herein aimed to construct a physiologically based pharmacokinetic model to determine optimal therapeutic strategies for organophosphate (nerve agent) poisoning. The constructed model integrated organophosphates and two antidotes, atropine and oximes. Model results reasonably mirrored literature data and anecdotal observations of organophosphate poisoning. Results suggest a symptoms-based dosing strategy of atropine and a time-based dosing strategy of oximes. For patients severely poisoned with organophosphorus nerve agents, model results support documented claims of oxime's inefficacy and tendency to heighten the severity of poisoning. The results strongly indicate that military personnel attacked with nerve agents are at a significant health risk if they employ their prescribed treatment as current doctrine dictates. The Use of Stimulant Medication to Enhance Human Performance in Extended Military Operations
The technological ability to operate around the clock places considerable demands on military personnel as they attempt to adapt to military operations beyond normal human physiological capability. The combination of increased operations tempo, decreased military personnel, and the probability of a prolonged effort in the global war on terrorism, increase the likelihood of military personnel working extended hours. These conditions result in military personnel experiencing fatigue and a decline in performance. Military leaders have authorized certain military personnel to use stimulant medications to mitigate the effects of sleep deprivation when adequate rest periods are not possible. The administration of these stimulant medications has enabled military personnel to perform at or near their rested performance levels. Medications currently in use have side effects and have limited utility for some personnel because of those side effects. This paper supports the effectiveness and suitability of a new medication, modafanil, as an alertness enhancing medication for military personnel in a variety of operational environments. The side effect profile of modafanil is superior to agents currently in use. USAF Civil Reserve Air Fleet Aeromedical Evacuation Airlift Capability
The Department of Defense, USAF, and Air Mobility Command (AMC) rely on the Civil Reserve Air Fleet (CRAF) to provide supplemental airlift in times of national emergency. AMC oversees the CRAF program which contains three segments: international, national, and aeromedical evacuation. The CRAF Aeromedical Evacuation (AE) program involves commercial B-767 aircraft, Aeromedical Evacuation Shipsets (AESS), a Casualty Transfer System, and a large number of support personnel and equipment. Wartime planning in the early 1980s revealed a significant shortage of military airlift to accomplish the aeromedical evacuation mission. Consequently the CRAF AE program was initiated. Planning for Major Regional Conflict scenarios has currently set a requirement for 25 and 44 B-767 aircraft for CRAF Stages II and III, respectively. Recent Advances in Molecular Mechanisms Underlying Kidney Diseases
Kidney diseases remain a major public health concern associated with increasing morbidity and the mortality of millions of people globally. Recently, the prevalence of kidney diseases has increased significantly. Their pathological hallmarks include the excessive accumulation and deposition of the extracellular matrix, mesangiolysis, the thickening of the glomerular basement membrane, tubulointerstitial fibrosis, and many more, which eventually lead to the loss of kidney function. As the pathogenesis of kidney diseases is multifactorial, several experimental models have recently been established to mimic clinical situations and provide a mechanistic understanding of various renal pathologies, with the aim of identifying and developing potential therapeutic targets. These models have been utilized to identify several underlying molecular mechanisms, including the following: the PI3K/Akt/mTOR, Jak/Stat, and inflammatory signaling pathways; the activation of the renal hydrogen sulfide system; the induction of oxidative and endoplasmic reticulum stress and apoptotic pathways; the activation of fibroblasts and the renin-angiotensin-aldosterone system; and mitochondrial dysfunction. Targeting these molecular mechanisms has proven to be beneficial and offers huge potential for developing novel therapies. This Special Issue focuses on recent developments in research on the pathophysiology of kidney diseases and their therapeutic targets for diagnosis and treatment.

Advances in Diagnosis and Treatment of Peritoneum Cancer
Approximately 4-10% of patients with tumors of the abdominal cavity have peritoneal metastasis (PM) at the time of diagnosis, while 20-40% will develop PM during the evolution of their tumor. Until recently, the diagnosis of PM was ominous and associated with severe and incapacitating symptoms, a poor response to systemic chemotherapy, and a poor survival of 4-6 months. The scenario has changed following the introduction of a new generation of chemotherapy and agents against biological targets, as well as with the introduction of new combined therapies, including cytoreductive surgery and new diagnostic-therapeutic schemes. However, there are still many unresolved issues relating to the role, indications, and efficiency that these new therapeutic schemes may offer. We aimed to evaluate the latest knowledge in relation to PM with respect to the incidence, clinical and molecular prognostic factors, survival, and the role of surgical-chemotherapeutic techniques in this era of biological and immunotherapy agents. Special interest had been placed on colorectal, gastric, and ovarian cancer, as well as primary peritoneal cancer.
