Medical practice has gradually lost its sacrosanct sovereign role in favour of the bureaucratic control of the clinically unqualified. Medical bureaucracy is no longer content with housekeeping. Its decisions now impact the clinical arena. As a result, many doctors and nurses have become dispirited, demeaned and discontented. In considerable numbers, they are leaving the UK medical profession or emigrating.Fundamental change is required if we are to cure our sick sacred cow. The many suggestions made here to cure our sick sacred cow will doubtless be resisted by a corporation intent on extending its workforce and expanding its influence.

Risk Factors for NASH in Cryptogenic Cirrhosis
Cryptogenic cirrhosis represents the final stage of some occult chronic liver diseases, with NASH being one of the main underlying diseases responsible for these cases. Some studies have looked for an association between cryptogenic cirrhosis and NASH by comparing the "metabolic profile" of the two groups, emphasizing diabetes mellitus and obesity. The aim of this study was to determine the prevalence of risk factors for NASH in patients with cryptogenic cirrhosis in Porto Alegre, southern Brazil, in order to establish a possible causal relationship. The study was carried out at the ISCMPA Gastroenterology Department between 2008 and 2010. It was concluded that, in assessing the possible etiological relationship between NASH and cryptogenic cirrhosis, the results allow an inference to be made, since a concordance was found in the glycemic profile and a difference between the mean age, suggesting a time necessary for the progression of the disease.

A novel theoretical framework that describes a possible rationale for the regularity in how we move, how we learn, and how our brain predicts events. In Biological Learning and Control, Reza Shadmehr and Sandro Mussa-Ivaldi present a theoretical framework for understanding the regularity of the brain's perceptions, its reactions to sensory stimuli, and its control of movements. They offer an account of perception as the combination of prediction and observation: the brain builds internal models that describe what should happen and then combines this prediction with reports from the sensory system to form a belief. Considering the brain's control of movements, and variations despite biomechanical similarities among old and young, healthy and unhealthy, and humans and other animals, Shadmehr and Mussa-Ivaldi review evidence suggesting that motor commands reflect an economic decision made by our brain weighing reward and effort. This evidence also suggests that the brain prefers to receive a reward sooner than later, devaluing or discounting reward with the passage of time; then as the value of the expected reward changes in the brain with the passing of time (because of development, disease, or evolution), the shape of our movements will also change. The internal models formed by the brain provide the brain with an essential survival skill: the ability to predict based on past observations. The formal concepts presented by Shadmehr and Mussa-Ivaldi offer a way to describe how representations are formed, what structure they have, and how the theoretical concepts can be tested.
Adult celiac disease and associated autoimmune diseases
Celiac disease (CD) is an autoimmune inflammatory enteropathy, caused by one of the protein fractions of gluten (food antigen), gliadin (a protein contained in wheat, rye and barley) and which occurs in genetically predisposed subjects [1,2,3]. Its pathogenesis results from the interaction between genetic, immunological and environmental factors which, through the intervention of the human leukocyte antigen (HLA) class II genes (encoding the HLA-DQ2 molecule, and those encoding the HLA-DQ8 molecule) induce an immune response in the intestinal mucosa leading to villous atrophy resulting in malabsorption, and the presence of specific antibodies in the serum of patients, which has changed the epidemiological view of celiac disease. The association with other autoimmune diseases (AID) is frequent, which imposes their systematic research. The aim of our work was to evaluate the prevalence of autoimmune diseases during celiac disease and to analyze their epidemiological, clinical, and immunological profile.

He found that just as Borges had perhaps dreamed the results of Quian Quiroga's discoveries, other thinkers--William James, Gustav Spiller, John Stuart Mill--had perhaps also dreamed a story like "Funes." Evolution from Bipolar Affective Disorder to Dementia
This study was based on a rare sample (patients diagnosed with dementia and bipolar affective disorder simultaneously) whose small number, 130 patients, poses statistical challenges. A history of bipolar disorder is associated with a significantly higher risk of dementia in older adults and has been associated with cognitive decline. Some studies investigating the risk of dementia and death in older people with bipolar disorder have concluded that bipolar disorder in senile age is associated with an increased risk of dementia (designated as greater neurocognitive impairment in the Diagnostic and Statistical Manual of Mental Disorders, Fifth Edition) and premature death. Bipolar disorder is a chronic psychiatric illness characterised by fluctuations in mood, with a relapsing and remitting course.The most common causes of dementia in individuals over the age of 65 are (1) Alzheimer's disease (2) vascular dementia and (3) vascular dementia and Alzheimer's type. Other diseases that account for around 10 per cent of the total include dementia with Lewy bodies; Pick's diseases; frontotemporal dementias.

A randomized controlled trial, conducted on 26 sub-acute hemiplegic stroke patients age of 35 to 80 and sample was collected from Zafar hospital, Lahore. Upper limb sensory deficit, dexterity, and quality of movement were checked by standardized tools which were fugyl-meyer scale, Semmes-Weinstein Monofilament, box and block test and motor activity log assessment. Social package for statistical sciences version 25 used for statistical work. Parametric Independent sample t test was used for between group studies which shows that p-values of all the variables were less than 0.05 which means that there is a significant difference in pre- and post-intervention. Within group studies done by using paired sample t test which shows there is a difference in pre- and post-interventions of all variables but mean differences of all variables in experimental group was more significant. It is concluded that both interventions group shows difference, but visual tactile stimulation group shows better results.

Clinical, Radiological and Genetic Investigation of Fahr's Disease
Fahr's disease (FD) or Familial Primary Calcification of the Basal Nuclei is a rare neurodegenerative disease, with an incidence of less than 1 in 1 million inhabitants (SALEEM et al., 2013) worldwide, autosomal dominant, characterised by symmetrical and bilateral calcifications in the basal nuclei (nb) and other areas of the cerebral cortex, thus allowing for diverse clinical presentations, mainly with extrapyramidal and psychiatric symptoms. In recent years, mutations in four genes have been identified as causing the disease (SLC20A2, PDGFRB, PDGFB and XPR1) allowing for a specific genotype-phenotype correlation as shown in Table 1 (TADIC et al., 2015). The author of this research is a neurologist working in the field of neuropediatrics. Through clinical history and examinations, she identified the likelihood of FD in the proband and found ways to develop this work in postgraduate studies.
