Lithotripsy - Novel Technologies, Innovations and Contemporary Applications provides a comprehensive overview of urolithiasis management. Lithotripsy, the act of stone fragmentation, achieved either extracorporeally (ESWL) or intracorporeally (ISWL), has experienced tremendous progress in the last 40 years. ESWL, using an electrohydraulic energy source, which was introduced in the early 1980s, revolutionized the management of stones for which there were very limited options besides open surgery. Two chapters in this book discuss the history of ESWL and its contemporary applications and indications. The remaining four chapters discuss intracorporeal energy sources for stone fragmentation, including pneumatic, ultrasonic, and laser sources. Whereas pneumatic and ultrasound lithotripsy are the current workhorses of endourologists, particularly in the management of large and medium-sized kidney stones, lasers, on the other hand, are predominantly used for small renal and most ureteral stones. Due to innovations and rapid developments, laser technology is quickly replacing other energy sources for most kidney stones as well.

A dramatic increase in knowledge regarding the molecular biology of gliomas has been established over the past few years, and this has led to the development of novel therapeutic strategies for these patients. This book describes some improvements in the surgical management of gliomas, including segmentation of brain MRI images using 4D MRI volumes to help with the diagnosis and monitoring of patients. Another novel topic reviewed involves the applications of photosensitizers and their efficacy in the generation of anti-tumor responses in photodynamic therapy. A review of the application of nanoparticles and their ability to deliver drugs to the tumor site with a reduction in systemic toxicity is another developing therapy discussed. Diverticulosis refers to an outpouching of colonic mucosa and submucosa outside of the intestinal lumen. It commonly occurs around the penetrating blood vessels. It can occur in any segment of the bowels; however, the sigmoid colon is the most affected segment. Diverticulosis (the presence of diverticula in the colon) remains asymptomatic in most individuals, but approximately 25% of people with diverticulosis will experience symptomatic diverticulosis, which is referred to as diverticular disease. In the past two decades, diverticular disease has become one of the most commonly diagnosed bowel disorders. It is seen across the globe and its incidence is high, especially in Western countries. It is estimated that in many developed countries around 50% of the population older than 50 years is affected by diverticular disease. It was once believed to be associated with increasing age, however, in recent studies, its prevalence in younger populations has increased. In about 10%-25% of patients with diverticulosis, the disease progresses and leads to diverticulitis. Many theories have been suggested for the different etiologies of diverticular disease, and the most common cause is believed to be a low-fiber diet. It is believed that diets low in fiber are associated with increased intracolonic pressure, which leads to diverticula formation. Diverticular disease can have variable clinical presentations. Most often it presents as abdominal pain or change in bowel habits and rectal bleeding. Its clinical picture can at times mimic inflammatory bowel disease or bowel malignancy. Its common sequelae include diverticulitis, perforation, and peri-colic abscess/phlegmon. The complications of diverticular disease include abscess, fistula, and bowel strictures. The Neuropathogenesis of Mood Disorders
Neurology does not include study of the limbic system as this is regarded as more relevant to psychiatry. Paradoxically, psychiatrists receive no education on the anatomy, physiology or neuropathology of the limbic system either during their specialist training programs internationally. Their focus is purely on the diagnosis and treatment of mental illness. Thus, they are unique among the medical specialities in the 21st century. The morbidity and mortality of the many millions of patients with mood disorders has remained unchanged for multiple decades, as opposed to the progressive advances made in all other medical specialities. This is a travesty and a tragedy for a medical specialist discipline. Accordingly, using advances in immunology, radiology and my own personal insights, and after considerable research, I have discovered the complex neuropathogenesis of mood disorders and how to substantially improve the treatment regimen in order to substantially improve patient outcomes. A detailed, comprehensive description of the anatomy, physiology, the neuropathology of the limbic system and the underlying etiology of mood disorders are described in this book. It is likely that a team approach using a psychiatrist and an immunologist will be required for patients with bipolar 2 and treatment resistant depression as the use of biological agents are complex and require considerable experience.

I view correctional facilities as the best place to start programs addressing diversity, equity, and inclusion. Correctional facilities house mentally retarded and brain damaged individuals with impaired frontal lobe executive functions. The ability to grasp and process abstract and complex concepts necessary for independent functioning in the community is grossly impaired. This in large part provides the basis for recidivism, return to prison for repeated crimes. This population needs neuropsychiatric testing to inform the probation officers and parole boards a rational basis for restoration programs that will successfully return this population to communities without return to correctional facilities. Very often this population has multiple crimes prior to the first incarceration despite extensive engagement with mental health services and the criminal justice system.

There are many brain damaged and mentally retarded individuals in correctional facilities who are not able to grasp and process abstract and complex concepts necessary for living independently in the communityl These individuals do not have the intellectual capacity for restoration to safe and independent living in the community. They are in in need of guardians and conservators and supervision to prevent future crimes and incarcerations. This book details the variaous groups in prison that includes the criminals who exploit the brain damaged and mentally retarded who find themselves in prison. These issues are the subject of a book, Of Mice and Men is a novella written by John Steinbeck.[1][2] Published in 1937, it narrates the experiences of George Milton and Lennie Small, two displaced migrant ranch workers, who move from place to place in California in search of new job opportunities during the Great Depression in the United States.
