The Order of St Lazarus saw its origins as an establishment located outside the walls of Jerusalem serving the needs of victims of leprosy. While its role in society changed as a result of political machinations, it still retained a hospitaller role in the subsequent centuries. Since the advent of the first drugs to treat herpesvirus infections, the antimicrobial community has been looking for better and more specific drugs and vaccines to both prevent and suppress these infections. The greatest active pharmaceutical ingredient that has been discovered since then is acyclovir and its derivatives, and the greatest vaccine successes against human herpesviruses have been against the varicella zoster virus. Antiherpetic drug and vaccine discovery continues to this day, an example of which being the recent clinical licensing of drugs such as letermovir and animamevir and vaccines such as Shingrix. This Special Issue takes a look at both the current antiherpetic interventions that are available today, such as the medications and vaccines that are used both prophylactically and therapeutically, and the direction that the field is headed toward in the near future. Fast Quantitative Magnetic Resonance Imaging
Among medical imaging modalities, magnetic resonance imaging (MRI) stands out for its excellent soft-tissue contrast, anatomical detail, and high sensitivity for disease detection. However, as proven by the continuous and vast effort to develop new MRI techniques, limitations and open challenges remain. The primary source of contrast in MRI images are the various relaxation parameters associated with the nuclear magnetic resonance (NMR) phenomena upon which MRI is based. Although it is possible to quantify these relaxation parameters (qMRI) they are rarely used in the clinic, and radiological interpretation of images is primarily based upon images that are relaxation time weighted. The clinical adoption of qMRI is mainly limited by the long acquisition times required to quantify each relaxation parameter as well as questions around their accuracy and reliability. More specifically, the main limitations of qMRI methods have been the difficulty in dealing with the high inter-parameter correlations and a high sensitivity to MRI system imperfections. Recently, new methods for rapid qMRI have been proposed. The multi-parametric models at the heart of these techniques have the main advantage of accounting for the correlations between the parameters of interest as well as system imperfections. This holistic view on the MR signal makes it possible to regress many individual parameters at once, potentially with a higher accuracy. Novel, accurate techniques promise a fast estimation of relevant MRI quantities, including but not limited to longitudinal (T1) and transverse (T2) relaxation times. Among these emerging methods, MR Fingerprinting (MRF), synthetic MR (syMRI or MAGIC), and T1‒T2 Shuffling are making their way into the clinical world at a very fast pace. However, the main underlying assumptions and algorithms used are sometimes different from those found in the conventional MRI literature, and can be elusive at times. In this book, we take the opportunity to study and describe the main assumptions, theoretical background, and methods that are the basis of these emerging techniques. Quantitative transient state imaging provides an incredible, transformative opportunity for MRI. There is huge potential to further extend the physics, in conjunction with the underlying physiology, toward a better theoretical description of the underlying models, their application, and evaluation to improve the assessment of disease and treatment efficacy.
Patient-Centered Design of Cognitive Assistive Technology for Traumatic Brain Injury Telerehabilitation
Computer software has been productive in helping individuals with cognitive disabilities. Personalizing the user interface is an important strategy in designing software for these users, because of the barriers created by conventional user interfaces for the cognitively disabled. Cognitive assistive technology (CAT) has typically been used to provide help with everyday activities, outside of cognitive rehabilitation therapy. This book describes a quarter century of computing R&D at the Institute for Cognitive Prosthetics, focusing on the needs of individuals with cognitive disabilities from brain injury. Models and methods from Human Computer Interaction (HCI) have been particularly valuable, initially in illuminating those needs. Subsequently HCI methods have expanded CAT to be powerful rehabilitation therapy tools, restoring some damaged cognitive abilities which have resisted conventional therapy. Patient-Centered Design (PCD) emerged as a design methodology which incorporates both clinical and technical factors. PCD also takes advantage of the patient's ability to redesign and refine the user interface, and to achieve a very good fit between user and system. Cognitive Prosthetics Telerehabilitation is a powerful therapy modality. Essential characteristics are delivering service to patients in their own home, having the patient's priority activities be the focus of therapy, using cognitive prosthetic software which applies Patient Centered Design, and videoconferencing with a workspace shared between therapist and patient. Cognitive Prosthetics Telerehabilitation has a rich set of advantages for the many stakeholders involved with brain injury rehabilitation.
Strategic Health Technology Incorporation
Technology is essential to the delivery of health care but it is still only a tool that needs to be deployed wisely to ensure beneficial outcomes at reasonable costs. Among various categories of health technology, medical equipment has the unique distinction of requiring both high initial investments and costly maintenance during its entire useful life. This characteristic does not, however, imply that medical equipment is more costly than other categories, provided that it is managed properly. The foundation of a sound technology management process is the planning and acquisition of equipment, collectively called technology incorporation. This lecture presents a rational, strategic process for technology incorporation based on experience, some successful and many unsuccessful, accumulated in industrialized and developing countries over the last three decades. The planning step is focused on establishing a Technology Incorporation Plan (TIP) using data collected from an audit of existing technology, evaluating needs, impacts, costs, and benefits, and consolidating the information collected for decision making. The acquisition step implements TIP by selecting equipment based on technical, regulatory, financial, and supplier considerations, and procuring it using one of the multiple forms of purchasing or agreements with suppliers. This incorporation process is generic enough to be used, with suitable adaptations, for a wide variety of health organizations with different sizes and acuity levels, ranging from health clinics to community hospitals to major teaching hospitals and even to entire health systems. Such a broadly applicable process is possible because it is based on a conceptual framework composed of in-depth analysis of the basic principles that govern each stage of technology lifecycle. Using this incorporation process, successful TIPs have been created and implemented, thereby contributing to the improvement of healthcare services and limiting the associated expenses.
