Trichomoniasis negligence
Vaginal discharge constitutes an important symptom in women attending outpatient clinics of RTI/STI, Medicine and Obs & Gyne. Some of these cases are due to sexually transmitted diseases like such as N. gonorrheae, C. trachomatis and T. vaginalis and can lead to adverse reproductive health and co-infection with HIV. The control of these infections can lead to substantial reduction in the transmission of HIV. Given the high cost of diagnosis, National Aids Control Organization advocates syndromic case management of patients with genitourinary complaints and treatment with drugs that target the most frequent etiological agents. However, it can lead to emergence of antibiotic resistant strains due to overtreatment. The utility of syndromic case management (SCM) is not as good in the treatment of vaginal discharge syndrome due to its limitations of distinction between cervicitis and vaginitis. The narrative of this book critically appraises the syndromic approach to trichomoniasis and other STIs, discusses the challenges that it faces, and offers suggestions to improve its performance.

Appendicitis
Appendicitis involves inflammation of the vermiform appendix, which is associated with a systemic inflammatory response in humans. Emergency surgical treatment is often necessary as it is the leading cause of surgery-related mortality. However, correct diagnosis remains a challenge due to variations in symptoms. This study assessed the impact of gender, seasonal variations, and the immunohistochemical expression of the IL 6 and 8 cytokines in patients with appendicitis at the Federal Medical Centre (FMC), Yenagoa. A total of 313 patients diagnosed with appendicitis and did appendicectomy were enrolled in the research from January 2015 to December 2020. The findings indicate that seasonal changes influence the outcome of appendicitis, with a statistically significant rise in cases reported during summer in comparison to winter. Among the studied subjects, females constituted a higher percentage than males, making up 56.9% (n=178) while males represented 43.1% (n = 135). Interleukins 6 and 8 were markedly expressed in appendix tissues regardless of age, gender, or season.

Ayurveda maintains 3 essential humours, which cause disease if they become imbalanced. These three humours known as 'Vata', 'Pita', and 'Kapha' are usually translated in English as Wind, Bile and Phlegm. Occasionally, in the surgical tradition, a fourth humour - blood - was added. Ayurveda is an Ancient Indian medicine, which has its beginning in the sixth century BC and thrives even to the present-day. There was once a celebrated doctor called Charaka who lived in the second century. He was one of the greatest physicians who have ever lived. He recorded the fact that the Gods themselves were perplexed by the continued existence of disease, which was a hindrance to humanity's progress to enlightenment. These same Gods, he says, therefore prepared the way for Ayurveda, which literally means the 'science of longevity' to be taught to the human race. Ayurveda is not just the science of curing diseases, but of preventing them too by developing immunity.
