Piezo Surgery-an Overview

The bone-cutting technique of the piezoelectric device works due to the use of micro-vibrations at a specific ultrasonic frequency modulated by sonic waves. The sonic and ultrasonic frequency (25-30 kHz) is produced by a mechanical shock wave that vibrates in a linear manner. Better esthetics of all Ceramic restorations compared with metal-ceramic systems and complexity of metal framework fabrication as well as concerns of biocompatibility of metallic alloys are prime factors that contributed to growth of all ceramic restorations. However there are concerns related to their low strength and toughness and associated catastrophic failure modes that lead to their failures. Over the years primary focus of dental ceramics developers and engineers has been to introduce new ceramic materials with adequate strength characteristics without compromising the esthetics. The various materials that have been introduced have different properties that specify their use in specific clinical situations.Conventional porcelains like feldspathic, aluminous core porcelian have very low flexural strength (below 150 Mpa) and therefore are indicated for onlays, three quarter crowns, veneers and complete coverage crowns in anterior segment only. Glass ceramics like Lithium disilicate reinforced ceramics were then introduced. They have moderate flexural strength (300 - 400Mpa) and can only be used in posterior segment for single crowns.

The development of bioactive surgical additives to regulate the inflammation and increase the speed of healing process is one of the great challenges in clinical research. In this sense, platelet rich fibrin (PRF) appears as a natural and satisfactory alternative with favourable results and low risks. The following is to summarize the relevant literature regarding the technique of using PRF, focusing on its preparation, advantages, and disadvantages of using it in clinical applications. PRF alone or in combination with other biomaterials seems to have several advantages and indications both for medicine and dentistry, due it is a minimally invasive technique with low risks and satisfactory clinical results.

Over the last decade, it has been observed that there is an increasing interest in the ceramic materials in dentistry. Esthetically these materials are preferred alternatives to the traditional materials in order to meet the patients' demands for improved esthetics. Dental ceramics are usually composed of nonmetallic, inorganic structures primarily containing compounds of oxygen with one or more metallic or semi-metallic elements. Ceramics are used for making crowns, bridges, artificial denture teeth, and implants. The use of conservative ceramic inlay preparations, veneering porcelains is increasing, along with all-ceramic complete crown preparations. Major developments in materials for all-ceramic restorations are addressed. These advances include improved processing techniques and greater mechanical properties. An overview of the processing techniques available for all-ceramic materials is given, including sintering, casting, machining, slip-casting, and heat-pressing. The most recent ceramic materials are reviewed with respect to their principal crystalline phases, including leucite, alumina, forsterite, zirconia, mica, hydroxyapatite, lithium disilicate, sanidine, and spinel.

Embellishments of strange happenings were unnecessary because human foibles ran rampant. However, most names and places have been altered so as not to embarrass the guilty, berserk, bizarre, and downright scurvy. The tales are what they are without color enhancement or pretrial publicity. The stories are retold in a series of vignettes which best captured my mood at the time. Who knew that dentistry could be so "funny?" Who knew, indeed? But is it ultimately a satisfying and rewarding profession as well? I will let YOU be the judge and answer that last question. In the meantime, sit back, relax and at least snicker at my "pain." Hopefully you will find the tales within the following pages to be humorous and you will laugh along with me. Maybe at me, as well.
