The National Centre for Infectious Diseases (NCID) is the result of many years in the planning, and it finally officially opened its doors in September 2019, just months before the entire world was tested by the COVID-19 pandemic. This book is the work of many people who represent an even larger pool of people from NCID, Singapore and the rest of the world in trying to understand and contain the SARS- CoV-2 virus. There are chapters on science, the public health response both locally and globally, as well as personal reflections from NCID and Tan Tock Seng Hospital staff and staff from other public healthcare institutions who were deployed to NCID which bring home the human impact of the pandemic. We are very grateful to all the authors for taking the time to put together their thoughtful chapters as well as the senior academics and public health leaders who have provided us with generous comments on the manuscript. We hope that the readers of the book will gain a better insight into the response to the virus from so many different perspectives. Although the pandemic has evolved far beyond the pages of this book globally, the lessons learned from the early days are still relevant. We hope that the chapters will be helpful as we review our experience of this pandemic and face the next emerging infectious disease in the years to come.This book provides a comprehensive look at many different aspects of response in Singapore to the pandemic in the crucial first several months, including clinical, laboratory, epidemiology, research, community engagement and the unprecedented challenge of outbreak involving migrant workers in dormitory settings. Broadly Engaged Team Science in Clinical and Translational Research
Despite the large U.S. investment in health science, and the vast and growing body of peer-reviewed research findings it has produced, a compelling body of evidence suggests that research too often has been slow, inefficient, and fallen short of desired impacts on health. A key question is how research might be changed to be more innovative, less wasteful, and more responsive to unmet health needs. One emerging response within clinical and translational science is to advance an approach that attempts to close the gap between research scientists and key stakeholders; the individuals and groups responsible for or affected by health-related decisions. Broadly engaged team science promises to support this aim by transforming the gold standard, multi-disciplinary team science, to include key stakeholders in activities across the research spectrum. These new roles and responsibilities range from generating research questions to implementing research projects, to aiding in the translation of discoveries from the laboratory to the community. A transition to broadly engaged team science reflects the idea that inclusivity and a diversity of perspectives are necessary to achieving progress in addressing complex health issues while representing a new benchmark for ethical research practice. This is one of the first collections of papers describing how clinical and translational science researchers are defining and implementing new research practices, and the successes and challenges involved. This book represents a first and critical step towards organizing knowledge of broadly engaged team science and advancing the development of evidence-based practices. Sanitation is a system that comprises not only a latrine but also the works for the treatment and disposal of human waste. Sanitation systems do not function by themselves but have significance only through social management. The process of decision-making also largely depends on socio-cultural conditions, and the importance of sanitation needs to be socially acknowledged. The health benefits of sanitation improvement--among the significant contributions of sanitation--also need to be considered in the socio-cultural milieu. Further, the social-culture itself is affected, and potentially even created, by sanitation. In this context, more progress on the improvement of sanitation requires a more holistic approach across disciplines.In this book, we present the concept of the Sanitation Triangle, which considers the interconnections of health, materials, and socio-culture in sanitation, as a holistic approach, and the case studies based on the Sanitation Triangle by diverse disciplines such as Cultural Anthropology, Development Studies, Health Sciences, Engineering, and Science Communication. Challenges and Opportunities in Health Professions Education
This book addresses health professions educational challenges specific to non-Western cultures, implementing a shifting paradigm for educating future health professionals towards patient-centered care. While health professions education has received increasing attention in the last three decades, promoting student-centered learning principles pioneered by leaders in the medical community has, for the most part, remain rooted in the Western context. Building from Hofstede's analysis of the phenomena of cultural dimensions, which underpin the way people build and maintain their relationships with others and influence social, economic, and political well-being across nations, this book demarcates the different cultural dimensions between East and West, applied to medical education. The respective 'hierarchical' and 'collectivist' cultural dimensions are unpacked in several studies stemming from non-western countries, with the capacity to positively influence healthcare educationand services. The book provides new insights for researchers and health professional educators to understand how cultural context influences the input, processes, and output of health professionals' education. Examples include how cultural context influences the ways in which students respond to teachers, how teachers giving feedback to students, and the challenges of peer feedback and group work. The authors also examine causes for student hesitation in proposing ideas, the pervasive cultural norm of maintaining harmony, the challenges of teamwork in clinical settings, the need to be sensitive to community health needs, the complexity of clinical decision making, and the challenge of how collectivist cultural values play into group dynamics. This book aims to advocate a more culturally-sensitive approach to educating health professionals, and will be relevant to both students and practitioners in numerous areas of public health and medical education.
Resilient Health Systems
Since February 2020 the COVID-19 pandemic has strained health systems worldwide. Despite several measures adopted, including the deployment of emergency funds to sustain health service transformations and investment in vaccine development, the crisis has taken over a year to be controlled. The consequences are still affecting the capacity and capabilities of health systems. In light of the ongoing pandemic, this book explores the factors determining the ability of health systems to cope with and recover from a crisis, and therefore their level of resilience. The term resilience has gained momentum in health systems research, with several scholars contributing to the definition of the concept. Providing a comprehensive health system resilience agenda, policy-makers, public health specialists, health managers, scholars and practitioners will find in this work both a grounded framework for the assessment of the level of resilience of healthcare systems and organizations, and specific actionable changes that should be pursued to consolidate and improve that level of resilience. Ultimately, this work identifies desirable paths of action through the investigation of real-world cases to improve health systems and organizations capacity and capability to face any future dramatic crisis.
Promoting the Health of the Community
Community health workers (CHWs) are an increasingly important member of the healthcare and public health professions who help build primary care capacity. Yet, in spite of the exponential growth of CHW interventions, CHW training programs, and CHW certification and credentialing by state agencies, a gap persists in the literature regarding current CHW roles and skills, scope of practice, CHW job settings, and national standards. Sex Work, Health, and Human Rights
This open access book provides a comprehensive overview of the health inequities and human rights issues faced by sex workers globally across diverse contexts, and outlines evidence-based strategies and best practices. Sex workers face severe health and social inequities, largely as the result of structural factors including punitive and criminalized legal environments, stigma, and social and economic exclusion and marginalization. Although previous work has largely emphasized an elevated burden and gaps in HIV and sexually transmitted infection (STI) services in sex work, less attention has been paid to the broader health and human rights concerns faced by sex workers. Successful Ageing
The ambition of ageing science to discover the golden fleece of longevity and health is closely connected to the concept of 'successful ageing'. Still, for a large portion of the population, frailty and cognitive impairment is the reality of ageing, and it is by no means certain if health promotion, prevention and other interventions will reduce the probability of its occurrence. This book argues that a narrow understanding of 'successful ageing' as good health, full functioning, and active participation in society excludes a large portion of ageing individuals from the quest for a good life in old age. The challenge is that the term 'successful ageing' comes with ambitions but also with ambivalence. On the one hand, it counteracts the deficit view of ageing and facilitates visionary thinking on what might be possible in the future. On the other hand, its implicitly ageist and derogative features have negative consequences to older people and society at large. So, what is successful ageing? We provide answers at three levels: First, we synthesise the various models used to define successful ageing into a heuristic scheme able to unravel the normative complexity and differences inherent in existing models. Second, we anchor successful ageing models in a tripartite way at micro (individual), meso, and macro analytic levels of human development. Third, we argue that the usefulness of successful ageing in guiding policy will only profit from the concept, if it follows a pluralistic and holistic view without hastily deciding for one model only.
