Early Exits
With 25 years in end-of-life care and firsthand experience with death, dying, and MAiD, David Maginley explores the unaddressed issues at the heart of our effort to control the final human experience. What drives most requests for Medical Assistance in Dying (MAiD)? Surprisingly, it's not physical pain but existential distress-the fear of suffering, the loss of dignity, and the struggle to find meaning and purpose as life draws to a close. Yet these very struggles are functional in a process few recognize: the deconstruction of the personal self (the ego), which prepares us for our final and most significant state of being-transcendence. In Early Exits, David Maginley offers a radical reinterpretation of MAiD as an attempt to preserve the ego in the face of this transformation. This is a reflection of our culture's spiritual poverty and reveals MAiD as a medical procedure being used to treat what is, fundamentally, a spiritual condition.Early Exits goes beyond discussions of autonomy, ethics, and dignity to explore the fundamental human experience of dying. Each chapter begins with a real-world case example and ends with practical tools to help readers live-and die-more fully informed. Drawing on decades of research into end-of-life phenomena and over 25 years of experience providing spiritual care to cancer and palliative patients, Maginley illuminates the inner dimensions of dying and how to address the distress that drives MAiD requests. With compelling research, personal stories, and profound spiritual insights, Early Exits reframes dying as a transformative process that leads us beyond grief and despair to a state of grace and growth.Central topics explored: Why existential suffering-not physical pain-is at the heart of requests for assisted death, and how to best address it.How the deconstruction of identity accelerates spiritual growth and calibrates us for transcendence. The importance of addressing both physical and psycho-spiritual needs in palliative care. How MAiD compromises the mystical, expansive states of consciousness experienced by most dying people, regardless of culture or belief.Maginley's insights are shaped not only by decades of experience supporting those facing mortality but also by his own journey as a four-time cancer survivor. Whether you are navigating a terminal diagnosis, supporting a loved one, or providing care as a professional in hospice or palliative settings, this book offers hope and clarity. Early Exits is an essential resource for understanding-and transforming-the most profound and ultimate event in life. It offers a way forward to a better society, a better tomorrow, and a better end to our days.

Acute renal failure in intensive care
Acute renal failure in intensive care is an independent factor in morbidity and mortality. The prevalence of AKI in patients with hemodynamic instability in the ICU setting can be as high as 50%. The definition is based on the 2012 KDIGO criteria, which combine a clinical parameter represented by the rate of diuresis, and a biological parameter that evaluates the variation in creatinine levels. Prevention starts with identifying subjects at risk, and optimizing systemic and locoregional hemodynamic conditions. The use of products with nephrotoxic potential must be carefully balanced in terms of benefit and risk. The therapeutic approach is essentially symptomatic, with the main aim of preventing worsening of renal damage while awaiting recovery of renal dysfunction.

Local recurrence of nephroblastoma
Recurrent nephroblastoma tumors have a poor prognosis. They pose the problem of their etiology. In a retrospective study of 14 cases of local recurrence of nephroblastoma, we investigated factors predictive of recurrence and assessed the prognosis. The mean age of patients at the time of initial tumor diagnosis was 4.5 years, with a sex ratio of 0.4. The mean time to local recurrence was 25.5 months. It was shorter in cases of: age greater than 3 years, tumor size greater than 110 mm, immediate association with metastases, difficult tumor excision, absence of lymph node sampling and resistance to chemotherapy. Also, in stages III and IV, blastematous type, anaplasia and tumor weight greater than 550 g. The prognosis for recurrence was good in 71.5% of patients (30% of whom had distant metastases) and poor in 28.5%. We recommend particular attention and further investigation of therapeutic alternatives for local recurrence of nephroblastoma, in order to improve prognosis.
Triple-negative breast cancer in young women in western Algeria
Triple-negative breast cancer represents a particular group of breast carcinomas defined immunohistochemically by the absence of hormone receptor expression and the absence of HER2 receptor overexpression. This group of tumors, which have the most unfavorable prognosis, does not currently benefit from any targeted treatment, and the only validated systemic therapy is chemotherapy. This is a retrospective study of the last six years conducted at the Etablissement H繫spitalier Universitaire d'Oran "1er Novembre 1954" (EHUO) between 2017 and 2022 involving 59 young patients with triple-negative invasive breast carcinomas.Pathologically, infiltrating ductal carcinoma was the most predominant type, accounting for 86.4% of cases, with a majority T2 tumor size of 66.1%.Despite advances in treatment and the advent of targeted therapies, breast cancer remains the leading cause of death in women. Current clinical and histological classifications do not allow us to fully establish the prognostic and predictive parameters of response to treatment.
Comprehensive Treatment of Diabetic Foot Syndrome
In the book the author considers methods of complex treatment of purulent necrotic complications of the lower extremities against the background of diabetes mellitus. The results of complex treatment with the inclusion of autotransfusion ultraviolet irradiated blood, ultrasonic cavitation of wounds and local use of a new enzyme preparation of plant origin are presented. The comparative analysis of the proposed methods of complex treatment of diabetic foot syndrome was studied. The therapeutic and diagnostic algorithm of diabetic foot syndrome is developed. The book is designed for specialists working in this branch of medicine.
Autoimmune hemolytic anemias in children
This book examines the characteristics of autoimmune hemolytic anemias in children, based on a retrospective study of pediatric patients with a positive direct Coombs test. Epidemiological, clinical, biological and therapeutic data were analyzed using SPSS software. A total of 17 patients were included (10 girls and 7 boys), with an average age of three years. The most frequent etiologies identified were visceral leishmaniasis (5 cases) and β-thalassemia major (4 cases). In addition to transfusions, corticosteroid therapy was administered to 10 of the 17 patients, with cure observed in 4 cases (23.5%). Although viral infections are generally considered to be the main causes of these anemias, our study revealed a predominance of visceral leishmaniasis, an etiology rarely mentioned in the literature. Autoimmune hemolytic anemias remain rare pathologies, often associated with a poor prognosis. A thorough etiological investigation and complete immunohematological workup are essential to optimize therapeutic management.
Extradural hematomas of the posterior cerebral fossa
Extradural haematomas of the posterior cerebral fossa are rare and tend to result from direct trauma to the occipital region. The aim of our study was to describe the management of this lesion. Our retrospective study included 25 cases hospitalised in the neurosurgery department of the Tunis Military Hospital over a period of 16 years. We collected and analysed the epidemiological, clinical, therapeutic and evolutionary data. The mean age of the patients was 26.84 years. Public road accidents were the most frequent aetiology (48%), and haematoma thickness was greater than 10 mm in 32% of cases. Treatment was conservative in 56% of cases and surgical in 32%. The outcome was favourable in 92% of cases.Extradural haematomas of the posterior cerebral fossa are a life-threatening diagnostic and therapeutic emergency. Surgical evacuation is required for forms that cause repercussions, with a favourable outcome in more than 80% of cases.

Potential New Biomarkers Of Diabetic Nephropathy 2
Diabetic nephropathy is one of the most serious microvascular complications of diabetes, and has become the leading cause of CKD. Its pathophysiological mechanism is complex and intricate. The stereotypical history of diabetic nephropathy passes through various stages that are accompanied by histological lesions that remain silent for a long time. Unfortunately, while this sequence is well described in T1DM, it is much more difficult to identify in T2DM, and it is therefore essential to identify impaired renal function at an early stage by measuring creatinine (GFR estimation) and microalbuminuria on an annual basis.Given that these investigative tools have had their limitations in use, and are sometimes insensitive to the detection of early renal failure, it would make sense to assay biomarkers of renal function that can optimize early detection strategies for the prevention and treatment of diabetic nephropathy. These biomarkers will be classified according to the renal structure affected by the pathological process.cys C and ? 2 M.

Impact of Blood Diseases on the Course of
In recent years there has been a significant increase in the number of diseases of the nose and paranasal sinuses (NNPS). According to the observations of S.Z. Piskunov and G.Z. Piskunov (2005), the number of sinusitis cases per 1000 people increased from 4.6 to 12.2 between 2000 and 2004 [54]. During this time interval, the number of patients with MNiONP increased annually by 1.5-2% and reached 52.7%. From 2005 to 2015, patients hospitalized for paranasal sinus diseases accounted for about 2/3 of the total number of patients in specialized hospitals. In Uzbekistan, the incidence of MNIOP is 65-70% and tends to increase. It is connected with significant pollution of the atmosphere with various ecotoxicants, significant decrease of the immune system of the macroorganism, high frequency of mixt-infections leading to the development of purulent-inflammatory process in the paranasal sinuses and development of antibiotic resistance. The monograph covers etiopathogenetic, genetic, clinical and diagnostic aspects of adenoiditis in frequently ill children and approaches to complex treatment. The monograph is intended for otolaryngologists, doctoral students, masters and clinical residents.
Error in HIV/AIDS Prevention and Control
This book is a successful Doctoral Thesis presented to the University of Nigeria for the award of the Degree of Doctor of Philosophy (PhD) in Public Administration. The book assessed the effectiveness of American-model Voluntary Counseling and Testing (VCT) policy in the prevention and control of HIV/AIDS in Nigeria, using Anambra State in South-East Nigeria as a case study. The study highlights the limitations of policy transfer from the developed North to the developing South. The book draws attention to the insidious errors in strategic policy management that have caused HIV/AIDS to remain an "unending epidemic" for more than 25 years. Accordingly, the book proffers solutions to accelerate the end of HIV/AIDS to meet the Sustainable Development Goal. The book throws a life-line to millions of persons and families affected by HIV/AIDS and to Developing Countries 'drowning' under the HIV/AIDS burden. Reading this book offers a special privilege of a "binocular vision" from the empirical analysis of an ardent scholar and the recondite knowledge and experience of a hands-on field practitioner in epidemic response.
Cognitive, Semantic and Evolutionary Aspects of Aesthetic and Moral Emotions
The book offers a systematic study of selected aesthetic and moral emotions, such as beauty, admiration, disgust, anger, guilt, and the feeling of (in)justice. The authors contemplate the possibilities of their conceptual grasp and describe their psychosomatic phenomena from a first-person perspective. They focus on a hermeneutic-phenomenological-existential analysis of the fundamental elements, structure, and significance of these types of emotions in both individual and collective human experiences. Additionally, they provide interdisciplinary insights into the subject, enabling the comprehension of these emotional states from various perspectives, including linguistic, psychological, philosophical, cognitive scientific, legal, and theological. They document that we are not rational systems that have emotions, but rather emotional systems that occasionally behave rationally. The text simultaneously outlines a new theory of emotions and explores the possibilities for their linguistic apprehension through the geometry of thought.
Detection of Anti-aspergillus Antibodies
Aspergillus spp. is ubiquitous fungi, commonly occurring in soil, water and decaying vegetation. The members of the genus Aspergillus form an important group of pathogens of the bronchopulmonary system. The spectrum of diseases caused by them encompasses aspergilloma, chronic necrotizing pulmonary aspergillosis, invasive pulmonary aspergillosis; allergic bronchopulmonary aspergillosis (ABPA); bronchial asthma and allergic alveolitis. The present study was carried out to find out the prevalence of aspergilli and antibodies to Aspergillus spp. in bronchial asthmatics using cultural methods and serological techniques like ELISA.
The Significance of Prophylactic Pastes in the Prevention of Caries
This monograph about caries disease, caries continues to increase globally with increasing urbanization and changing living conditions as the prevalence of major oral diseases. This is primarily due to inadequate fluoride (in the water supply and oral hygiene products such as toothpaste), the availability and affordability of foods high in sugar, and societal associated with poor use of oral hygiene services. The introduction of foods and drinks high in sugar, as well as tobacco and alcohol, has led to an increase in the consumption of products that contribute to oral diseases and other diseases. Caries is a multifactorial disease, and therefore the same approaches to choosing a treatment method do not give highly effective results. Therefore, improving the prevention of caries remains one of the urgent problems.

Toxic Effect of Erythrosine on Zebra Fish Embryos
Unpredicted human safety events in clinical trials for new drugs are costly in terms of human health and money. The drug discovery industry attempts to minimize those events with diligent preclinical safety testing. Current standard practices are good at preventing toxic compounds from being tested in the clinic; however, false negative preclinical toxicity results are still a reality. Continual improvement must be pursued in the preclinical realm. Higher-quality therapies can be brought forward with more information about potential toxicities and associated mechanisms. The zebrafish model is a bridge between in vitro assays and mammalian in vivo studies. This model is powerful in its breadth of application and tractability for research. In the past two decades, our understanding of disease biology and drug toxicity has grown significantly owing to thousands of studies on this tiny vertebrate. This Review summarizes challenges and strengths of the model, discusses the 3Rs value that it can deliver, highlights translatable and untranslatable biology, and brings together reports from recent studies with zebrafish focusing on new drug discovery toxicology.
Anabolic Steroids
Anabolic steroids are increasingly utilized by athletes and bodybuilders for muscle growth and enhanced performance, yet their usage poses significant health risks. High doses and the practice of stacking steroids can disrupt hormonal balance, leading to conditions such as hypogonadotropic hypogonadism, testicular atrophy, and impotence. Liver dysfunction, including severe complications like hepatoma and peliosis hepatitis, is common among users, with some experiencing neuropsychiatric effects such as mood swings and aggression. The history of anabolic steroids spans from the isolation of testosterone in the 1930s to the introduction of various synthetic derivatives, with medical applications in treating hormonal deficiencies and specific health conditions. However, the potential for abuse in athletic contexts has raised ethical concerns, underscoring the need for medical supervision. As research progresses, a balanced understanding of anabolic steroids' therapeutic benefits and health risks is crucial for athletes seeking performance enhancement. This book seeks to address a crucial question: "Can anabolic steroids be used safely in bodybuilding and other sports?"
Prognosis of heart failure in sub-Saharan Africa
Heart failure is a serious disease with high mortality. The objective of this study was to identify prognostic factors in heart failure. Method: this was a case-control study carried out over a period of 24 months (from January 2017 to December 2018). Cases are patients hospitalized for heart failure with reduced ejection fraction and died during hospitalization and controls are patients hospitalized for heart failure with reduced ejection fraction and who were discharged alive.Personal history of heart failure was more represented in cases (72%) than in controls (59%) (p
A Book of Medical Discourses
Medical Discourses by Dr. Rebecca Lee Crumpler is a groundbreaking work by the first African American woman to earn a medical degree in the United States. First published in 1883, the book provides insight into Crumpler's experiences as a physician, particularly her work serving communities during and after the Civil War. Combining medical knowledge with practical advice, she addresses topics such as maternal and child health, nutrition, and disease prevention, often emphasizing the importance of compassion in caregiving.
Making Multiple Babies
Human beings have been producing more twins, triplets, and quadruplets than ever before, due to the expansion of medically assisted conception. This book analyzes the anticipatory regimes of making multiple babies. With archival documents, participant observation, in-depth interviews, and registry data, this book traces the global and local governance of the assisted reproductive technologies (ARTs) used to tackle multiple pregnancy since the 1970s, highlighting the early promotion of single embryo transfer in Belgium and Japan and the making of the world's most lenient guidelines in Taiwan.
The Siting Trash Syndrome in Systems and Genetics
In your hands, dear reader, is a book that clearly and simply explains many points that are still unknown in medical literature. You'll learn who antigens are and what they do in favour of our organism, DNA is just DN, the Central Energetic Sensory System, driven by Purkinje cells. And what an Alpha Delta child is, a Genius child, and how it happens. The existence of very rare beings with superpowers and beings who are born with innate immunity for life without any modification, and beings who are infectious, due to their cellular formation, even without having diseases, as well as the formation of twins through Purkinje cells, microcephaly, which in no way results from the Zika Virus. Left out of this study is the integumentary system, which will be presented in a concomitantly more complete compendium. In Neurology, you will learn about the real causes of Parkinson's disease, the role of the substantia nigra (Alpha Beta Delta) and the basal ganglia. This book is aimed at anyone interested in the field of health, specifically physiology. Happy reading!
Presbyopia Cure Research Progress
Presbyopia, commonly known as "old man's eye disease," is a condition that leads to difficulty seeing near objects due to changes in the eye's lens. Historical explanations date back to Aristotle, with significant contributions from figures like Ren矇 Descartes and Ernst Br羹cke, who helped identify the ciliary muscle's role in accommodation. Benjamin Franklin devised the first bifocals in the 1780s as a solution for presbyopia. In the 19th century, advancements included the development of eye charts by Heinrich Kuechler and Eduard Jaeger for assessing near vision. Hermann von Helmholtz later explained that presbyopia results from the loss of lens elasticity due to aging.Presbyopia, an age-related decline in near vision, has driven the search for effective and convenient pharmaceutical therapies. Topical parasympathomimetic agents, particularly pilocarpine, have emerged as promising treatments for this condition. The aim of this book is to provide an understanding and an overview of presbyopia research progress.
