Health Policy
This important new textbook provides comparative and critical analysis of health care policy from high-income countries in Europe to low-income developing countries in the Global South. It integrates conceptual themes drawn from the fields of sociology, policy analysis, and political science to offer a unique combination of theory, historical background, and wider social commentary. The book is divided into three sections: Section I establishes the conceptual basis for the analysis of the health policymaking process, including implementation. Section II provides an introduction to the key elements of conducting a comparative health system's analysis, including chapters that provide examples of performance analysis in both high- and low-income developing countries. Section III examines key challenges now facing health policy-makers that include long-term social care provision, widening the scope of public health to address social inequalities in health outcome, the integration of genomic medicine within a health care system, and the establishment of an effective national pharmaceutical policy. Each chapter includes case studies, historical-institutional contexts, and summaries of key health policies. Detailed and clearly written, it is the ideal text for health and social science students in this expanding area of analysis.
Psychobiology of Language
What is language? Can we distinguish language from general cognition? Is language an isolable, biologically coherent system? Does the linguistic description of language as an autonomous system, formed from a combination of more-or-less autonomous subsystems, correspond to psychological and neurophysiological fact? This collection of original contributions, drawn from a Neurosciences Research Program Work Session, provides a comprehensive and thoroughly up-to-date discussion of the research, knowledge, and debates in the neurobiology of language. This book inaugurates a new series in Neuropsychology and Neurolinguistics, edited by David Caplan.
The Organization of the Cerebral Cortex
These published proceedings of a Neurosciences Research Program Colloquium do not deal exhaustively with particular cortical issues--rather, they convey the highlights of the topic, beginning with a series of presentations on the ontogenetic and morphogenetic development of the cerebral cortex followed by a systematic view of the remarkable explosion during the last decade of our knowledge of the cellular organization and connectively of the cortex. All of the topics in the book are put into perspective in an opening keynote by W. Maxwell Cowan. He there observes that theoretical constructs (or the lack of them) are the weakest aspect of neurobiology at the moment. Thus the book's final section (with contributions by three Nobel laureates--Francis Crick, Gerald Edelman, and Leon Cooper--among others) is a meaningful new effort toward redressing the balance.

Nursing Care Protocol.
The nurse consists of a fully compensatory system in which she manages care, makes judgments and decisions about patient care. This can be seen in the way this professional acts and in the extensions of his or her functions. In this descriptive, prospective and transversal research with the objective of evaluating the behavior of the threat of preterm delivery and designing a nursing care protocol in the Perinatal Maternal Care service at the "Tamara Bunke Bider" North Maternal Hospital in Santiago de Cuba, the study was carried out in 210 patients, who were admitted to the service in this period, with this diagnosis, evaluating the patient's responses after the procedure. The observations in the notes and the independent, dependent and interdependent actions, allowed validating the efficacy of the interventions and the satisfaction of patients and service providers. The procedure will achieve the implementation of a nursing care protocol and the design of instruments for its evaluation in order to achieve the termination of gestation, thus obtaining a live, healthy and uncomplicated newborn.
The emergency medical service (SAMU)
The mission of emergency medical services is to respond to unscheduled emergencies of varying severity, using resources that are limited in comparison with the diagnostic and therapeutic arsenal available within hospitals. The role of the medical dispatcher, sometimes faced with life-threatening emergencies, is crucial in the initial presumption of diagnosis, enabling the most appropriate discharge from the mobile emergency and resuscitation services (SMUR). In this book, we analyze the relationship between the diagnostic suspicions of the medical dispatcher and the intervention, through a study of clinical performance indicators.
Versatility of Microvascular Free Flaps in Reconstruction of Mandible
Microvascular free-flap reconstruction is a surgical procedure that involves the transfer of tissue, along with its blood vessels, from one part of the body to another. This technique is commonly used in reconstructive surgery to repair defects or restore function in areas affected by trauma, cancer, or congenital abnormalities. They can offer the finest biological and biomechanical features, a higher success rate of bone union, at the same instance, the cosmetic and functional score is superior compared to that of non-vascularized flaps, as well as the ability to osseointegrate with dental implants. They are regarded as the gold standard for mandibular reconstruction, especially for those with large or complex defects.
”Advanced Breast Cancer Detection Using Machine Learning and Segmentat
Breast cancer is the second leading cause of death among women, often undetected until it reaches advanced stages. Early identification is crucial, as accurate classification of benign and malignant tumors can prevent unnecessary treatments. This study explores the application of machine learning techniques for breast cancer diagnosis using the Wisconsin Breast Cancer Dataset from the UCI Repository.Initial experiments with the Na簿ve Bayes classifier yielded 88% accuracy for benign and 86% for malignant tumors. However, it faced limitations, such as low accuracy and issues with zero frequency probabilities. Switching to Artificial Neural Networks (ANN) improved results to 90% for benign and 92% for malignant classifications, but still did not yield optimal outcomes. The research ultimately employed Support Vector Machine (SVM) techniques, achieving the highest accuracy at 97% for benign and 95% for malignant tumors. This method effectively distinguishes between tumor types using a linear model based on hyperplanes. All algorithms were implemented using the R tool, which is user-friendly and free, facilitating data handling for breast cancer classification.

Zoonoses and public health
Zoonoses are infectious diseases transmissible from animals to humans, and represent a major challenge for global public health. With human population growth, increasing urbanization and climate change, interactions between humans and animals are multiplying, favoring the emergence and spread of these diseases. Historically, zoonoses have played a significant role in the evolution of human societies, influencing events such as devastating epidemics and changes in agricultural practices. That's why we need to avoid zoonoses in our living environments.
Healthcare-associated infections
Introduction: Healthcare-associated infections (HAIs) are a major public health concern worldwide, and our aim was to provide an update on HAIs and to develop an example of a protocol for an incidence survey and an approach to extra costs in hospitals. Methods: A bibliographical review of the literature was carried out in order to provide a theoretical review and to draw up a protocol. Results: The protocol was a cohort study with prospective data collection. To estimate incidence, we used 3 epidemiological indicators: cumulative incidence, incidence density and the medical device exposure ratio. To assess the cost of HCAIs, we were interested in the direct medical cost, which was presented in terms of expenditure linked to the length of hospital stay (comparing infected cases with uninfected controls), as well as expenditure linked to the consumption of antibiotics necessitated by these infections. Conclusion: A method for regularly monitoring the incidence and cost of these infections needs to be introduced in every hospital establishment for effective control.
Tuberculin skin test
Introduction: Ganglionic forms of tuberculosis often escape diagnosis by conventional tests. Our aim was to evaluate the performance of the tuberculin skin test in the diagnosis of active tuberculosis in Tunisia, using ROC curve methodology. Methodology: This was a multicenter case-control study that was conducted in 11 tuberculosis clinics(DAT) in Tunisia in 2014. Results: In total, we included 1053 patients divided into 339 cases and 714 controls. The mean diameter of the tuberculin TST induration was significantly higher in cases than in controls(13.7mm vs.6.2mm;p=10-6).The area under the curve(AUC )was 0.789[95% CI:0.758-0.819;p=0.01], corresponding to a moderate discriminating power for this test. The most discriminative threshold value for the diameter of the induration of the intradermal tuberculin reaction, associated with the best sensitivity (73.7%) and specificity (76.6%), was 11 mm, with a Youden index of 0.503.Conclusion: We can conclude that the tuberculin skin test can be used to diagnose tuberculosis with good sensitivity and specificity. However, its interpretation and reading remain difficult and highly subjective.
Vaccination schedule in children under 5 years of age
This book discusses the benefits of complying with the national vaccination schedule in children under five years of age; where it is one of the priorities for the health sector in Mexico; taking into account the high morbidity and mortality rate due to immunopreventable diseases; where vaccination is one of the most effective actions for prevention.Vaccination is one of the most effective public health actions to prevent diseases and an obligation of the state. However, in Mexico there is no certainty about the coverage and timeliness with which vaccines are given to children during their first year of life (every year 2.3 million children are born in Mexico), these figures mean that up to 65.7% of children may not have their complete schedule or have received it late and, therefore, at risk of getting sick. In contrast, the Federal Health Secretariat in Mexico; admits that only 9.3% of children under one year of age are at risk for vaccine-preventable diseases.

Ischemic stroke in newborns
Ischemic stroke in newborns is the result of an interruption in blood flow to one of the main cerebral arteries due to embolism or thrombosis. Infantile strokes are 10 to 12 times rarer than in adults, with an estimated incidence of 3.3 per 100,000 births. Perinatal stroke is the most frequent form, and is the leading cause of cerebral palsy and the second most common cause of neonatal convulsions after anaxo-ischemic encephalopathy. Its clinical presentation is not very specific. The diagnosis is most often made on the basis of a brain imaging scan ordered in response to neurological warning signs, particularly neonatal convulsions. To this day, the pathophysiological mechanism of neonatal stroke remains debated. Multiple maternal and fetal risk factors have been identified. Following a neonatal stroke, the child's development may be complicated by various motor and cognitive sequelae. Early motor rehabilitation can improve the newborn's prognosis.

Pharmacotherapies for Benign Meningioma
Meningiomas are generally benign tumors that commonly affect females, leading to symptoms primarily due to pressure on surrounding brain structures and increased intracranial pressure. The historical context of meningioma treatment includes notable figures like Felix Platter and Harvey Cushing, who contributed to our understanding and terminology of these tumors. This book emphasizes the limitations of traditional therapies and advocates for a pharmacotherapeutic approach using hydroxy urea, which has shown modest efficacy when used alone. Combing hydroxy urea with tamoxifen is proposed. This combination aims to enhance treatment effectiveness while reducing side effects, providing a promising option for patients with unresectable or recurrent meningiomas. The book underscores the need for further research to optimize treatment strategies in managing benign meningiomas.

Medical responsibility and the influence of medical delegates
The aim of this work is to study medical responsibility in the face of the influence of medical representatives in medical prescribing in hospitals.Method: This was a cross-sectional, descriptive and prospective study carried out in the public hospitals of Ouagadougou from January 08 to January 31, 2024. Physicians were asked to complete a self-administered questionnaire. Results: Our study involved 213 physicians in the city of Ouagadougou, with an average prescriber age of 34.7 years and a male predominance of 58.7%. Specialized physicians were the most represented: 51.2%. Doctors with less than 5 years' professional experience accounted for 50%. 95.8% of prescribers said they had received gifts, mostly drug samples 91.8% medical equipment 87.1% catering 39.7% conference funding 39.2% .... The Internet was the most widely used source of information (44.1%) for prescribers to verify the information provided by the medical representative. Antibiotics were the most represented drug class at 62.3%, with high specialty prescribing at 66%.
59 cases of occupational asthma
Occupational asthma (OA) has become the most common work-related respiratory disease. We carried out a retrospective descriptive study to investigate the prevalence of OA in southern Tunisia from 2002 to 2009, to support its diagnostic strategy, to describe the epidemiological features of affected subjects, and to identify the preventive and medico-legal etiological factors of the disease. Fifty-nine cases of PA were collected in our study, with a mean age of 42+/- 9 years. Most of the patients were manual workers. In the context of our study, occupational disease declarations were essentially made on the basis of 3 tables: table N簞42 relating to isocyanates; table N簞56 relating to cereals and flour; table N簞58 corresponding to other agents responsible for allergic respiratory affections. Of the 59 cases reported, 38 were recognized as occupational diseases. A permanent partial disability rate was assigned to each recognized case (average permanent partial disability rate was 24.07%).
Superfoods
With the growing interest in healthy foods, the concept of "superfoods" is becoming increasingly popular. Superfoods are generally foods originating in exotic countries. They are attributed "superpowers" because of their interesting nutritional profiles and beneficial properties for health and well-being. At present, there is no official definition or classification of these foods. Nevertheless, they can be found on the market everywhere, from pharmacies to supermarkets. In this book, we've tried to provide detailed monographs on some of the foods that are widely used today as "superfoods", such as spirulina, maca, moringa, chia seeds and red yeast rice, and to verify their various interests and uses. However, it is important to note that these foods are not a substitute for medication. In view of the limitations of certain studies and the sometimes dubious safety profiles, further studies are needed in the future to establish a standardized protocol for use and define an official classification for these foods.
Error in HIV/AIDS Prevention and Control
This book is a successful Doctoral Thesis presented to the University of Nigeria for the award of the Degree of Doctor of Philosophy (PhD) in Public Administration. The book assessed the effectiveness of American-model Voluntary Counseling and Testing (VCT) policy in the control of HIV/AIDS in Nigeria, using Anambra State in South-East Nigeria as a case study. The study highlights the limitations of policy transfer from the developed North to the developing South. The book draws attention to the insidious errors of strategic policy management that have caused HIV/AIDS to remain an "un-ending epidemic" for more than 25 years. Accordingly, the book proffers solutions to accelerate the end of HIV/AIDS to meet the Sustainable Development Goal. The book throws a life-line to millions of persons and families affected by HIV/AIDS and to Developing Countries 'drowning' under the HIV/AIDS burden. Reading this book offers a special privilege of a 'binocular vision' from the empirical analysis of an ardent scholar and the recondite knowledge and experience of a hands-on practitioner in epidemic response.
Resveratrol (RV)
Resveratrol (C14H12O3) is a polyphenolic compound belonging to the stilbene family of compounds, which consists of two aromatic rings linked by a double bond. The molecular structure of resveratrol can exist in two isomeric forms: trans-resveratrol and cis-resveratrol, the latter being more stable and biologically active. Its structure is crucial to its biochemical and therapeutic properties, including antioxidant, anti-inflammatory and anti-cancer activities.1. Aromatic rings: Resveratrol contains two benzene rings that are key to its bioactivity. These aromatic rings provide a framework that can interact with various enzymes, receptors and proteins. The rings decorated with hydroxyl groups, which further enhance the biological interactions of the molecule, particularly its antioxidant properties.2. Stilbene backbone: The core structure of resveratrol is based on a stilbene backbone consisting of a carbon-carbon double bond (-C=C-) linking the two aromatic rings. In this book, we have 7 chapters to discuss about Resveratrol (RV): A Journey through Its Biological and Medicinal Wonders.
The compliance to and practice of social distancing amongst adults
In this book, the author examines social distancing as a public health strategy during the COVID-19 pandemic in Kinshasa. Utilizing a mixed-methods approach that combines cross-sectional and phenomenological analyses, the study evaluates the relationship between adherence to social distancing protocols and other preventive behaviors, as framed by the Health Belief Model (HBM). Additionally, it explores the personal experiences of social distancing within the community context of Kinshasa between March and April 2022.The book is organized into several chapters that cover the background, theoretical framework, research methodology, data analysis, and conclusions. This book is innovative in its combination of quantitative adherence assessments with qualitative phenomenological insights, offering a thorough understanding of the challenges in implementing and experiencing social distancing as a health measure within an urban, community-dependent setting.
Digestive neuroendocrine tumors
Digestive neuroendocrine tumors (NETs) form a heterogeneous group of rare tumors whose characteristics vary according to site and associated hormonal syndrome. Prognosis depends essentially on histology, which makes precise classification according to tumor grade (WHO) essential. The aim of our work was to study the epidemio-clinical profile, prognostic factors and therapeutic modalities of NETs.

Urinary tract infection in women
Urinary tract infection (UTI) is a medical condition in which pathogenic microorganisms, usually bacteria, are present in the urinary tract[1]. The urinary system includes the bladder, ureters, urethra and kidneys[2]. UTIs can manifest themselves in a variety of ways, depending on the site affected. Clinical manifestations can range from simple burning to septic shock. Urinary tract infection is the second most common community-acquired infection after respiratory tract infection, with a worldwide incidence of 404.61 million in 2019. Antibiotic therapy is of course the cornerstone of therapeutic management, and is vital to vital prognosis.
Prevalence of the degree of hyponatremia in patients with liver cirrhosis.
Hyponatremia is a hydroelectrolyte alteration defined as a serum sodium concentration lower than 135 mEq/L, being the most frequent electrolyte imbalance in clinical practice in patients with cirrhosis. Objective: To estimate the prevalence of hyponatremia degrees in patients with liver cirrhosis admitted to the Gastroenterology area of the Public Hospital in the period from January 2021 to June 2022. Methodology: Observational, retrospective, analytical and descriptive study; a sample of 111 patients whose information was obtained from medical records was analyzed. Results: The predominance of female sex was identified (50.5%), as well as age between 51 and 70 years (64.9%) and 72.1% had a hospital stay of 1 to 15 days. Mild hyponatremia had a prevalence of 23.7%, moderate hyponatremia a rate of 10.1% and severe hyponatremia a rate of 1.3%. Regarding comorbidities, cancer was the only one that was related to the degrees of hyponatremia. On the other hand, ADH was the only complication. We conclude that the prevalence of hyponatremia in these patients was 35.1%.

Legal Answers to Common Medical Questions Habituals
This paper was inspired by the concerns of physicians and lawyers themselves, the other party investigated by us, since we are convinced that knowledge is the best weapon for the prevention of lawsuits against the medical corps and health teams. The spirit is obviously legal, since physicians must always keep in mind that the one who will judge them will not be another colleague but a lawyer who will be the judge at that moment, while thinking that medical reasoning is not always in agreement with legal reasoning. Many answers were directly based on the guidelines of Law 17132, the new Civil Code and other legislation together with the Code of Ethics of the Argentine Medical Association in its third edition, the Code of the Argentine Medical Federation and our own contributions. The important thing is that by knowing the legal norms in advance the professional will know when his behavior is licit or illicit, or can be questioned. The idea is that knowledge reduces the professional's daily stress and will lead him to the path of greater tranquility in his daily activity. In short, we do not want to prove anything but simply to show and promote reflection.
The Effect of ”Armenicum” Paste on the Course of Induced Wound Process
The problem of wound infection has been highly urgent in modern medicine. In many countries of the world is stated the absence of tendency to reducing the number of people with regional purulent-inflammatory diseases, which is attributed first of all to the tolerance of wound microflora to antibacterial drugs particularly to antibiotics, then to the low efficiency of the conventional therapy and the long-term periods of treatment. We selected "Armenicum" paste as for testing new effective means of wound process therapy. The effect of "Armenicum" paste on the course of a regional inflammatory process has been observed on an induced model of aerobic purulent wound in laboratory rats. The topical application of "Armenicum" paste had significant bactericidal effect. In the result of wound cleaning from the persistent microorganisms processes of complete phagocytosis were activated. The application of the paste on the wound surface was accompanied with activation of in situ processes of fibronectin synthesis. It stimulates the I and III types of collagen synthesis which was expressed in the acceleration and further differentiation of collagen fibrils.

Portfolio for medical interns
Recent medical education reforms in Tunisia have highlighted the importance of clinical skills and a learner-centered approach. In this context, the portfolio is a useful tool to support interns in their training. The aim of this project was to create a portfolio guide for interns at the Sousse Faculty of Medicine in hepato-gastroenterology (HGE). To achieve this, a literature review identified best practices, and an analysis of the specific needs of interns was carried out.The result is a portfolio guide that clearly defines its objectives and content. It includes an identification sheet, learning activities such as clinical situation narratives and an internship diary, as well as space for personal reflection. Assessment criteria have also been developed to measure the clinical and interpersonal skills of interns. In conclusion, this guide meets a specific need in the medical training of HGE interns. A pilot phase will be set up to adjust the guide according to feedback from interns and tutors.
Treatment of cancer pain in the elderly
In around 70% of cases, cancer pain in the elderly may be related to the disease itself, to its treatment or care, or to any of the other comorbidities common in this ageing population. Management of these patients is always comprehensive, combining general measures with specific drug and non-drug therapies. In all cases, cancer-specific treatment remains the reference treatment for pain linked to the disease itself. In cases where etiological treatment is not appropriate in the patient's management, or in any case to complement the analgesic effect of the latter, analgesics of the three WHO levels will be indicated. Their prescription, for purely symptomatic purposes, is based on the same rules in onco-geriatrics as for any other patient. Finally, the multiplicity of causes of such pain, and the many means available to manage it, must not interfere with a rigorous diagnostic and therapeutic approach. Thus, the need to know the precise and safe handling of analgesics or other associated molecules, which may be used to treat pain, must be taken into account.
Cirrhotic cardiomyopathy
Cirrhotic cardiomyopathy (CMC) is an emerging clinical condition characterized by cardiac dysfunction in cirrhotic patients with no known cardiac pathology. This cross-sectional study, carried out at CHU Sahloul in Sousse on 76 cirrhotic patients, aimed to assess the prevalence of CMC and identify its predictive factors. Results showed that CMC affected 53.9% of patients according to the consensus definition, with a lower prevalence (40.8%) based on advanced echocardiographic techniques. Predictive factors identified include age, female gender and a CHILD PUGH score >=9. The study highlights the importance of new echocardiographic techniques for early diagnosis, and suggests revising the definition of CMC to incorporate these parameters. The relationship between CMC and the severity of cirrhosis remains controversial, and further research is needed to define appropriate treatments.
Oral mucosal lesions
Aging is a universal phenomenon that involves changes in the organism including the oral cavity of the elderly. In order to characterize older adults with oral mucosal lesions associated with the use of removable prostheses, an observational, descriptive, cross-sectional study was carried out. Predominantly older adults between 60 and 69 years of age were female. The most frequent lesions were subprosthetic stomatitis and fissured epulis. The main risk factors were smoking, acrylic upper total prosthesis, time of use from 6 to 11 years, frequency of continuous use, inadequate hygiene and poor condition of the appliance. A greater relationship was found with the type of prosthesis, the material used and the condition of the prosthesis.
Celiac disease and quality of life
Celiac disease (CD) is an autoimmune disorder whose management relies mainly on a strict gluten-free diet (GFD). This cross-sectional study, conducted on 100 Tunisian patients between October and December 2023, assessed the impact of CD on quality of life (QOL) using a generic questionnaire (the SF-12) and a specific questionnaire (the CDQ). The results show that CD significantly alters QoL, with low scores in both physical and mental domains. Factors associated with impaired HRQOL included non-university education and low socio-economic status. The study recommends the systematic use of HRQOL assessment tools and the implementation of supportive strategies to improve RSG adherence and patient management.
Hematological and biochemical tests
Mastering the interpretation of routine haematological and biochemical tests is an asset for every medical student in our environment.Indeed, with the advent of automated systems in our healthcare facilities, it is now possible to carry out most of these tests automatically.Despite this, the role of the healthcare professional remains paramount, especially as he or she is called upon to interpret the results of these examinations clinically. In view of the above, we are convinced that this book will enable any student consulting it to correlate the results of the haematological and biochemical examinations discussed here with the clinical situation. As this book is not exhaustive, we suggest that you read future editions in this field.
Human Robotics
A synthesis of biomechanics and neural control that draws on recent advances in robotics to address control problems solved by the human sensorimotor system. This book proposes a transdisciplinary approach to investigating human motor control that synthesizes musculoskeletal biomechanics and neural control. The authors argue that this integrated approach--which uses the framework of robotics to understand sensorimotor control problems--offers a more complete and accurate description than either a purely neural computational approach or a purely biomechanical one.The authors offer an account of motor control in which explanatory models are based on experimental evidence using mathematical approaches reminiscent of physics. These computational models yield algorithms for motor control that may be used as tools to investigate or treat diseases of the sensorimotor system and to guide the development of algorithms and hardware that can be incorporated into products designed to assist with the tasks of daily living. The authors focus on the insights their approach offers in understanding how movement of the arm is controlled and how the control adapts to changing environments. The book begins with muscle mechanics and control, progresses in a logical manner to planning and behavior, and describes applications in neurorehabilitation and robotics. The material is self-contained, and accessible to researchers and professionals in a range of fields, including psychology, kinesiology, neurology, computer science, and robotics.
