Palliative Use of Inotropics in End-Stage Heart Failure
On average 1% of the population over 40 years of age acquires an initial stage of heart failure and this percentage doubles every 10 years, representing 10% in those over 70 years of age. This implies the great impact of the disease in relation to its incidence and prevalence; it currently affects more than 6 million adults in the United States, and 8 million patients are projected by 2030.Throughout the life of the heart failure patient it is usually necessary to apply a series of complex and invasive treatments, which however in advanced stages of the disease are less effective.A review of the American College of Cardiology / American Heart Association (ACC / AHA) Guidelines emphasizes palliative inotropic therapy as part of an overall plan to improve quality of life in this group of patients. This proposes a major challenge that diverts us from the main focus of these drugs; that is why through this review we propose that inotropic drugs would have a place as palliative treatment in end-stage heart failure.
Acute respiratory failure in the elderly in the emergency room
Non-invasive ventilatory therapy is often proposed to assist elderly subjects in acute respiratory failure (ARF) in the emergency department. Our objective was to highlight the difficulties encountered during this practice and the predictive factors of failure. The study was retrospective, conducted in the emergency department of the University Hospital La Rabta (Tunisia). Seventy-five patients over 65 years of age with ARF and recourse to non-invasive ventilation (NIV) were included. The etiological diagnoses of ARF were often associated with 72% of acute COPD decompensation, 61% of acute heart failure, 37% of pneumopathy and 1% of pulmonary embolism. NIV failure resulted in 24% in-hospital mortality and 8% in intensive care unit admission. History of hypertension (p=0.026), hepato-jugular reflux (p=0.028), lower limb edema (p=0.016), uremia (p=0.005), CRP (p=0.004) and initial pH (p=0.044) were correlated with mortality. Thus, several predictive factors for failure of non-invasive ventilatory therapy were identified in our study. The frequency of acute comorbidities could explain the difficulties encountered.

Biomechanics of Superior Femoral Epiphysis in Adolescents
Introduction: The objectives of this study were to identify the biomechanical factors involved in the occurrence of upper femoral epiphysiolysis through a series of cases and a review of the literature and to study the biomechanical factors of stability of the upper femoral epiphysis after percutaneous screw fixation.Methods: This was a retrospective longitudinal study conducted in the orthopedic department at the Tunis Children's Hospital over a period of 7 years. The evaluation of the results was clinical by the PMA score and radiological by the calculation of the residual tilt angle. Results: The main biomechanical factor involved in the occurrence of EFS was obesity. It was identified in 58% of the cases with an adiposo-genital morphotype that represented 11%.Conclusion: Knowledge of the biomechanical factors at the time of placement of the cephalic screw is the guarantee of a stable osteosynthesis.

Ki67 in breast carcinoma: correlation with major prognostic factorsBreast cancer (BC) is a major public health problem in Tunisia and worldwide. Few studies have focused on the correlation of Ki67 index with conventional prognostic factors of BC. The objectives of this work were to study through a series of cases, the clinico-pathological characteristics of CS and to evaluate the correlation of the Ki67 index with the main prognostic factors.A retrospective descriptive study of CS cases over a five-year period was performed.A total of 69 cases were collected. X-radiography inspection techniques are generally used in non-destructive evaluationindustry. Manual assessment of the inspection may fail and turn into false assessment dueto a large number of examining while inspection process. Accordingly, incorrectassessment through human vision may cause immense industry disaster. Therefore, it isessential to examine through machine vision, which is capable to avoid false assessment.Digital X-radiography has widely been used in Non-Destructive Testing (NDT) andparticularly in weld defect detection. Weld defects occurrence is an unavoidable problemduring the welding process. Most common weld defects some of which are porosity, gaspore, tungsten inclusion, longitudinal crack, lack of penetration, and slag inclusion.Digital image processing techniques are a foremost way to experiment in the NDT. Thedetection and classification of weld defects depend on the quality of the digitized image, which is subjected to certain factors, such as noise, the mode of the image histogram, defects of different dimensions, indiscernible defects in the image background, and lowcontrast or unevenly illuminated image. More accuracy can be achieved during classificationof weld defects are always be subject to the deliverables of low and mid-level imageprocessing techniques. Therefore, it is desirable to provide more importance for these levelsin the weld X-radiography image.
