Passion for Excellence
This book chronicles the life and career of Haralampos M. Moutsopoulos, an internationally renowned professor of medicine and prolific researcher on Sj繹gren's syndrome and autoimmune rheumatic diseases. In language that is simple and direct, he takes us on a fascinating journey from the days of his first scientific awakenings at a hospital in his native town of Ioannina, Greece, through his university years in Athens and training in the US, to his eventual return to his homeland. In Greece, he developed two centers of excellence in his field, first at the newly founded University of Ioannina Medical School Department of Internal Medicine and, second, at the Athens University Medical School, Department of Pathophysiology, where he taught until his retirement in 2011. A major theme throughout the book, and especially in this second edition, is his impassioned struggle for excellence, meritocracy, and transparency in universities and in the National Health System in Greece. This battle is relentless especially during critical periods, like the pandemic era, where human life was seriously threatened by the coronavirus and scientific community had to develop and pionner novel therapeutic approaches and bold administrative solutions. Peppered with both amusing and unsettling incidents from this lifelong crusade to raise professional standards and against misconduct, the book is a must-read for anyone interested in learning about or entering the medical profession as well as for those whose goal is to lead an adventurous, passionate life free from fear, prejudice and mediocrity.
Botulinum Toxin in Cosmetic Dermatology & Skincare Practice
Botulinum therapy occupies a special place in modern aesthetic medicine, not only as one of the most effective methods with more than 30 years of successful medical history but also as a tool that raises many questions. However, all known medical books on botulinum therapy primarily consider practical aspects - determination of injection points, dosage, etc. While this is certainly valuable information for botulinum therapy practitioners, everyone else is more concerned with other issues - whether botulinum toxin is safe, how it works, what effects can be expected from its use, and how to maximize its duration. These and other essential questions are addressed in our book - no one has ever dealt with botulinum toxin and its possibilities in such detail! We will identify the critical points related to botulinum toxin and its use in clinical practice. After discussing the nature of the active ingredient - botulinum toxin - we will move on to a brief review of commercial preparations, followed by the main factors affecting the treatment efficacy and safety, backed up by references to authoritative scientific studies. We will outline and justify the main directions of applying botulinum toxin for aesthetic and dermatological indications. We will also discuss the specifics of combining botulinum therapy with other injectable and device-based methods within the framework of complex aesthetic programs to prevent and correct age-related skin changes. This book is part of the Cosmetic Dermatology & Skincare Practice book series, covering the most relevant topics in modern dermatology and aesthetic medicine. All books are structured, written in accessible language, and answer critical questions about the problem or technology at hand. The information in this book will be useful to aestheticians, dermatologists, physiotherapists, neurologists, ophthalmologists, and plastic surgeons - both practicing and those just beginning to master botulinum therapy - as well as medical students and all interested parties.

The Practice of Electroconvulsive Therapy
Applied Physiology to Reduce Ventilator Induced Lung Injury
This book details the mechanisms of ventilator induced lung injury (VILI) at the alveolar level with the aim to identify optimal ventilation methods necessary to preserve lung function. Mortality associated with the acute respiratory distress syndrome (ARDS), including that caused by COVID-19, remains unacceptably high. The primary treatment is supportive in the form of protective mechanical ventilation, but set improperly this can cause an unintended secondary VILI significantly increasing mortality. To improve ventilation strategies needed to reduce VILI the alteration in alveolar mechanics caused by ARDS must be understood. The protective ventilation strategy must attempt to normalize alveolar mechanics, which would significantly reduce the mechanical damage subjected to lung tissue during mechanical ventilation. Written by leading experts with numerous diagrams, figures, and videos, this book takes the latest research in the field and translates it to clinical practice. Authors discuss the ARDS-induced alteration in alveolar mechanics that make it so susceptible to VILI and novel ventilation strategies necessary to normalize alveolar mechanics and reduce ARDS related morbidity and mortality. Chapters cover normal lung (alveolar mechanics and micro anatomy), how these are altered during acute lung injury, and the optimal Mechanical Breath Profile (MBp) necessary to stabilize and open the lung to reduce both VILI and acute lung injury-induced morbidity and mortality. This is an ideal guide for pulmonologists, critical care specialists, surgeons, and all medical professionals working with patients on ventilation.

Examination of the in Vitro Regenerative and Neuroprotective Potential of Short Chain Fatty Acid Butyrate
Immune-mediated polyneuropathies stem from dysregulation of the immune system and affect a wide range of the population. Inflammatory diseases such as chronic inflammatory demyelinating polyneuropathy (CIDP) and Guillain-Barr矇 syndrome (GBS) are the most common acute and chronic treatable immune-mediated polyneuropathies. The immune system mediates primarily demyelination but also neuronal degeneration through oxidative stress. As a result, patients suffer from hyporeflexia or areflexia accompanied with sensory and motor deficits as the disease progresses, which can lead to hospitalization and death. Over the years, nutrition and intestinal metabolism have become more relevant for the regulation of the immune system. In particular, short chain fatty acids (SCFA) have become the focus of attention. Currently, the best-studied SCFA is butyrate, produced by bacteria in the intestine along with propionate and acetate. Butyrate has already achieved promising results in areas such as oncology and immunology. Now, in the context of this master thesis, the question arises of the neuroprotective and neuroregenerative abilities of butyrate under oxidative stress.

End of Life Choices for Cancer Patients
There have been impressive improvements in the diagnosis and treatment of cancer in recent decades. In economically advantaged countries with well-developed healthcare systems over 50% of all cancer patients achieve long term survival and are probably cured. Not only has survival improved radically for cancer patients but also there has been an increasing focus on the quality of patient's lives, on improving the patient experience of care and on developing effective support for the very many cancer survivors. Despite the progress in cancer treatment, unfortunately a substantial number of cancer patients will still ultimately die of their disease. For many this will follow periods of successful treatment which results in good remissions and good quality of life. Helping patients to make the right choices about their care towards the end of their lives is one of the greatest and most challenging responsibilities of all healthcare professionals. Legal change on the provision of assisted dying by healthcare professionals has occurred in a substantial number of jurisdictions. This work brings together contributions on end of life choices from experienced professionals from oncology disciplines, palliative care, law, nurses and professions allied to medicine. The goals are: To better inform cancer care professionals and the wider community about developments in choices in end of life care for cancer patients internationally. To better answer questions from patients and respond to requests from patients, including questions about and requests for assisted dying in countries where it is legal to do so. To have a balanced and well informed dialogue about choices available to patients without developing a formal policy position on change in law. To provide a basis of information for future educational activities.

He's Not Mad, He's My Brother
Jason was a happy child, sensitive and kind, much loved by his older brother. So what caused him to become an emaciated drug addict and alcoholic before his late teens? Yes, he was hurt by the family break-up, becoming a little more withdrawn, yet such things are not uncommon these days. Then there were the dark paranormal phenomena that haunted the family home and which, Jason himself says, have followed him ever since wherever he went. A child's over-imagination? But he wasn't the only one to be aware of them...This beautifully written memoir by his brother, with thoughtful contributions by Jason himself, is loving and compassionate. It is also searingly honest. It warns us all that mental health can be so very fragile. And it points an accusing finger at a society that simply doesn't seem able - or, often, willing - to help.
Good Psychiatric Management for Borderline Personality Disorder and Alcohol Use Disorder
Six percent of people in North America will develop borderline personality disorder (BPD) in their lifetime, and about 46% of them will have alcohol use disorder (AUD) at the same time. Alcohol use exacerbates the symptoms of both diseases, rendering treatment more challenging and increasing the risk of suicide. Integrated treatments have been lacking. Good Psychiatric Management for Borderline Personality Disorder and Alcohol Use Disorder meets the urgent need for such an integrated approach. It provides clearly articulated descriptions of both BPD and AUD, outlining clinical patterns and how to diagnose them with confidence. Relying on general psychiatric principles with which most clinicians are already familiar, as well as up-to-date standards of care for both BPD and AUD, the handbook pays particular attention to areas of potential synergy, providing clinical logic for addressing complex, real-world cases. Topics include the following: - Progress assessment, psychoeducation, and goal setting;- Managing suicidality and nonsuicidal self-harm;- Pharmacotherapy;- Multimodal treatments, including mutual-help groups and family intervention; and- Level-of-care considerations. The authors stress that, in the absence of evidence-based manualized therapy for treating BPD and AUD simultaneously, clinicians already have the tools to increase treatment retention, reduce the risk of suicide and death, and provide a sensible road map in the face of interpersonal, behavioral, and emotional challenges inherent to recovery for both conditions.

Out of Her Mind
For centuries so called 'difficult women' have been labelled as 'hysterical' and 'out of their minds'. Today they wait longer for health diagnoses, often being told it's 'all in their heads'. Although healthcare systems are overburdened, why are women the first to feel the effects of this? Why is it so hard for women to find the kind of help they need? Why is no one listening to them? And why have so many lost faith in mental healthcare? Drawing on the lived experiences of women, alongside expert commentators, recent history, current events, and her own personal and professional experience, Dr Linda Gask explores women's mental healthcare today. In doing so she confronts her role as a psychiatrist, recalling experiences treating women and as a woman who has received mental healthcare, illustrating the dire need for more change, faster. Women can't all be out of their minds.
Federated Learning for Internet of Medical Things
The book intends to present emerging Federated Learning (FL) based architectures, frameworks, and models in Internet-of-Medical Things (IoMT) applications. Unseen
Stigma still surrounds suffering from mental health problems, and in most low-resource settings in "developed" and especially the "developing" world, this prejudice is compounded by an utter lack of resources to address these problems. The class of mental health problems is the greatest source of morbidity worldwide compared to all other categories of health problems and is also the most neglected. The difference between the vast mental health needs worldwide and the scarce resources that address them is known as the "mental health gap." This book portrays this gap through the story of two men suffering from schizophrenia and clinical depression in an unnamed low-income country. At the same time, it portrays how "there" can be "anywhere" by providing glimpses of the mental health issues of a relief worker's mother back home. This book relies on the stories of Sam and Berko to bring alive the mental health gap and raise awareness of the all too often "unseen" presence of mental illness throughout the world, a problem that contributes to enormous suffering and disability without ever showing up on an X-ray or in blood tests, let alone in public discourse. The book aspires to educate on the mental health gap by showing rather than telling and using narrative rather than epidemiology and statistics. It will inspire mental health professionals to apply their craft outside of customary areas of mental health practice; policy makers and government planners to better allocate protections and resources to mental health problems; the public to advocate for change; and everyone to reflect on the sources of our distress and our contentment.

The Mystery of Mental Illness
Most people are interested in their roots, often being highly gratified if their personal ancestry can be traced back to some notable historical character. Roots, however, go a lot further back than the written records about people and are implicated in the puzzle of our origin: why are we such symbolically proficient biological creatures that have distanced ourselves so far away from the rest of the animal kingdom? One solution is the magical one, that we arrived at the behest of an omnipotent being 6,000 years ago, fully formed and frolicking with the dinosaurs, as did the rest of our fellow creatures. For those who think the magical solution is a trifle simplistic there is the real problem that the human brain reached modern size around 250,000 years ago - but our lifestyle was still exceedingly primitive. Our most notable talents are therefore less than 150,000 years old and arrived, frequently tinged with serious flaws now termed, collectively, as the mental illnesses. Sensitive Skin in Cosmetic Dermatology & Skincare Practice
Patients with sensitive skin usually pose a problem for skincare specialists. First, as their complaints are often subjective - burning, itching, tightness, and tingling, which cannot be seen with the naked eye - it is not always clear how to diagnose and evaluate the therapy outcomes. Second, for many people, skin hypersensitivity is initiated by cosmetics, which is the primary skincare tool, so skincare practitioners may avoid such clients who likely expect cosmetics only to make things worse. This book was created to help skincare practitioners better understand this syndrome, allowing them to tailor their treatment protocols to each case. We have gathered as much up-to-date information as possible, given that research in recent years has helped significantly advance our understanding of this condition. Why do not all patients with sensitive skin have problems with the skin barrier, even though it was previously considered a "must"? Why is hypersensitivity present even though a reduction in nerve endings is found in many patients? Does the microbiome influence the development of sensitivity symptoms, and what are the dangers of psychological stress? You will find the answers to these and other questions in our new book. The main part of the book is devoted to building an effective care strategy for sensitive skin. All complex treatment approaches are considered, from limiting triggers, protection, and cleansing, which are very specific to this condition, to modern therapeutic modalities that affect each of the mechanisms of sensitive skin pathogenesis. The peculiarities of diagnosing this condition and how to distinguish it from other skin pathologies - atopic, allergic, contact dermatitis, and rosacea - are provided. Separate sections are devoted to skin sensitivity of aged skin, the treatment of the sensitive scalp, as well as psychological help to patients, which can be provided by a skincare specialist.
Mechanisms of Adaptation and Reconstruction in the Hypoperfused Brain
This book delves into the molecular mechanisms after stroke and gives an overview about current findings in ameliorating the recovery phase. It provides insights into post-ischemic recovery processes and illustrates novel findings of targets for therapeutic interventions for brain ischemia. Focus topics are microenvironmental changes mediated by the molecules interleukin 6 (IL-6) and SorCS2 in murine models of ischemia. One particular focus of the book is the evaluation of the effect of the inflammatory cytokine IL-6 on the brain parenchyma in carotid stenosis, a chronic form of brain ischemia. IL-6 plays an ambivalent role in the hypoperfused brain. The study in this book shows that on a network and molecular level, IL-6 can mediate potentially beneficial changes and provides potential therapeutic targets for continuing research. In a second study, the role of SorCS2 in stroke is analyzed, providing new insights into the role of this sorting protein in stroke recovery. By shining light on the prediction of humane endpoints in ischemic mouse models, this book also shines light on the 3R approach of reducing animal suffering in experimental research.
Learning from Clinical Cases in Primary Care
This new book is designed around 20 clinical cases, each representative of scenarios encountered by doctors who work in primary care and which will prompt learning in related issues surrounding each clinical scenario. Each case description concludes with an initial exam style question which MRCGP candidates will recognize as being directed towards learning for the Applied Knowledge Test (AKT). An expanded answer to that initial question is followed by a detailed explanation, concluding with further questions for self-assessment and reflection. The cases also provide a template for the preparation of the Simulated Consultation Assessment (SCA) component of the MRCGP exam and can be used by trainers and trainees alike as a prototype for role play. Some cases offer learning opportunities through discussion of discrete clinical entities, whilst others use the clinical cases to introduce the reader to broader concepts which often need grappling with in primary care.Key Features: True-to-life problems illustrate the complex interplay between patient and doctor in the consultation process Content reflects the natural variation in disease presentation, not just from patient to patient but in the same patient at different times Text is interspersed with bullet points and important tips to aid the learning process Multiple choice questions expand knowledge around inter-related topics and provide further invaluable preparation for the AKT and SCA Covering the wide range of specialities that are encountered in primary care, the book reflects the remarkable breadth of clinical knowledge that is required of a primary care practitioner and demonstrates why general practice remains a challenging but rewarding specialty to master.

Lasers in Cosmetic Dermatology & Skincare Practice
Much has been written about lasers, but only a few books cover laser cosmetic dermatology from beginning to end. We decided to address this gap and collected all the information that would be useful for specialists in this book. It focuses on modern laser and photodynamic technologies and deals in detail with various issues concerning high-energy and low-intensity devices. The book consists of three parts. In the first part, we explain the nature of laser radiation and provide a general understanding of how a laser device is constructed. A separate chapter is devoted to the interaction of laser radiation with the skin - the targets and mechanisms of laser action and the essence of photothermolysis. This part also considers the features of intense pulse light (IPL) treatment, low-level laser therapy (LLLT), and photodynamic therapy (PDT).The second part is devoted to practical issues - the specific capabilities of light-based and photodynamic technologies. The entire spectrum of dermatological and aesthetic problems in which lasers and IPL devices yield significant results are considered. These are laser rejuvenation, removal of benign growths, treatment of vascular and pigmentation disorders, scars, acne, psoriasis, and onychomycosis, as well as tattoo, unwanted hair (epilation), and fat deposit removal. In this part, we also discuss the current perspectives on the combination of laser treatment and injectable procedures, and the possibilities of using lasers for the transdermal delivery of active ingredients. A separate chapter on complications associated with laser procedures and ways to reduce their risks concludes the second part. The third part touches on some key organizational aspects, such as requirements for safe operation and the choice of laser devices.

Hormones, Health and Human Potential
Over 2,500 years ago, Hippocrates advocated the personalisation of "nourishment and exercise" as the "safest way to health". He knew that our behaviours influence our health, but he did not know why. This book explains the missing link: the crucial role played by our hormones and what we can do to optimise them.Hormones set in motion the physiological processes that keep us alive, bringing our DNA to life by directing gene expression. Hormones are the protagonists in the interaction between lifestyle and health. We can harness our hormones positively to optimise health through our choice of behaviours. Conversely, mistiming between our actions and internal hormone biological clocks can derail the intricate interactions of hormone networks. For example, in relative energy deficiency in sport (RED-S).This book reveals the clandestine world of hormones, navigating the path to achieving full personal potential, through the different stages of life. This is relevant for both men and women, whatever your objectives, whether you are a high-level athlete; looking to improve health and fitness; seeking to understand hormone disruption; or meeting the challenges of physiological changes in hormones during life such as perimenopause and menopause. Practical tips and "hormone stories" provide actionable advice and real-life examples of how to tune in to your hormones and maintain hormone health. As the human lifespan extends, it becomes ever more desirable to remain healthy into old age. Understanding how and why your choice of lifestyle affects your hormones empowers you to take informed decisions to improve your personal health. A life-long, healthy mix of exercise, nutrition and sleep is open to anyone, at minimal cost.
