It affects at least 12% of school children and 5 % of adults in the UK. This amounts to around 140 patients in a General Practitioner's average list of 2000, not all of whom can be managed by specialist chest clinics and over half of whom will require prophylactic therapy. There are therefore at least 3 million sufferers in Britain today. Yet, despite the improvement in treatment available in the past decade there has been a rise in numbers of attacks of acute asthma, notably in children, and a consequent rise in hospital admissions. The same pattern is also seen in adults, albeit less marked. Despite a number of possible incriminating causes, the real reasons for the rise are not clear. Whatever the cause may be, the inevitable effect is clear - the workload of doctors and nurses is expanding with only now, in the mid-1990s, the suggestion that this increase has begun to level off. Each year around 2000 people die from asthma in England and Wales and two-thirds of these deaths are preventable. The economic costs of asthma are huge.

Lithotripsy - Novel Technologies, Innovations and Contemporary Applications provides a comprehensive overview of urolithiasis management. Lithotripsy, the act of stone fragmentation, achieved either extracorporeally (ESWL) or intracorporeally (ISWL), has experienced tremendous progress in the last 40 years. ESWL, using an electrohydraulic energy source, which was introduced in the early 1980s, revolutionized the management of stones for which there were very limited options besides open surgery. Two chapters in this book discuss the history of ESWL and its contemporary applications and indications. The remaining four chapters discuss intracorporeal energy sources for stone fragmentation, including pneumatic, ultrasonic, and laser sources. Whereas pneumatic and ultrasound lithotripsy are the current workhorses of endourologists, particularly in the management of large and medium-sized kidney stones, lasers, on the other hand, are predominantly used for small renal and most ureteral stones. Due to innovations and rapid developments, laser technology is quickly replacing other energy sources for most kidney stones as well.
Molecular Biology and Treatment Strategies for Gliomas
A dramatic increase in knowledge regarding the molecular biology of gliomas has been established over the past few years, and this has led to the development of novel therapeutic strategies for these patients. This book describes some improvements in the surgical management of gliomas, including segmentation of brain MRI images using 4D MRI volumes to help with the diagnosis and monitoring of patients. Another novel topic reviewed involves the applications of photosensitizers and their efficacy in the generation of anti-tumor responses in photodynamic therapy. A review of the application of nanoparticles and their ability to deliver drugs to the tumor site with a reduction in systemic toxicity is another developing therapy discussed. The book also describes novel approaches involving the development of the use of microRNAs, which are non-coding RNAs that can be used as tumor suppressors that potentially can be developed to control the growth of gliomas. Molecular networks need to be better understood for the development of therapeutic strategies. Finally, the book reviews immunotherapeutic strategies potentially useful in treating brain tumors that involve either poxviruses engineered to secrete IL-15- or IL-2-secreting fibroblasts transfected with tumor DNA. The stimulation of the immune system to selectively attack malignant cells should lead to the prolongation of survival of brain tumor patients without a decline in cognitive functions or other side effects. It is hoped that this new information will lead to improved and efficacious treatment strategies for these challenging tumors.

Diverticulosis refers to an outpouching of colonic mucosa and submucosa outside of the intestinal lumen. It commonly occurs around the penetrating blood vessels. It can occur in any segment of the bowels; however, the sigmoid colon is the most affected segment. Diverticulosis (the presence of diverticula in the colon) remains asymptomatic in most individuals, but approximately 25% of people with diverticulosis will experience symptomatic diverticulosis, which is referred to as diverticular disease. In the past two decades, diverticular disease has become one of the most commonly diagnosed bowel disorders. It is seen across the globe and its incidence is high, especially in Western countries. It is estimated that in many developed countries around 50% of the population older than 50 years is affected by diverticular disease. It was once believed to be associated with increasing age, however, in recent studies, its prevalence in younger populations has increased. In about 10%-25% of patients with diverticulosis, the disease progresses and leads to diverticulitis. Many theories have been suggested for the different etiologies of diverticular disease, and the most common cause is believed to be a low-fiber diet. It is believed that diets low in fiber are associated with increased intracolonic pressure, which leads to diverticula formation. Diverticular disease can have variable clinical presentations. Most often it presents as abdominal pain or change in bowel habits and rectal bleeding. Its clinical picture can at times mimic inflammatory bowel disease or bowel malignancy. Its common sequelae include diverticulitis, perforation, and peri-colic abscess/phlegmon. The complications of diverticular disease include abscess, fistula, and bowel strictures. Small or localized collections can be managed with intravenous antibiotics and depending on the available resources, intervention radiology can also be used to manage such cases.
