Uncertainty in Medicine
Medical uncertainty has been with us for centuries and remains a recurrent problem for patients, doctors, and researchers alike. Yet uncertainty in health care is still poorly understood and ineffectively managed; it is generally feared and avoided rather than directly confronted. This systemic disregard of uncertainty leads us to treat medical uncertainty as a pathological condition to be cured through the pursuit of knowledge, but often further medical knowledge begets further uncertainty in kind. Uncertainty in Medicine offers an alternative, multi-disciplinary perspective on this challenging problem. Integrating insights across clinical medicine and social science, Dr. Paul Han argues that uncertainty is an essential form of knowledge to be cultivated, rather than eradicated, in medical practice. He makes the case that the paradigm of medicine should be expanded to include not only the pursuit of medical knowledge but the treatment and palliation of medical uncertainty and its effects on physicians, other health professionals, and patients. Using clear language and a textbook approach, he analyzes the nature, etiology, and natural history of medical uncertainty, and develops a conceptual framework to guide its management. Metabolic Syndrome
Metabolic syndrome has been the topic of countless publications. It still remains a subject of debate and some experts have even questioned its clinical relevance. Its diagnosis is nevertheless predictive of an increased risk of type 2 diabetes and cardiovascular disease even in the absence of traditional risk factors. Many years ago, our team made the point that the most prevalent form of metabolic syndrome was linked to abdominal obesity, which can be found even among individuals who are not considered obese by body weight standards. Imaging techniques such as computed tomography and magnetic resonance imaging have revealed the link between regional body fat partitioning and cardiometabolic risk. Visceral obesity is the most dangerous form of obesity, with subcutaneous obesity being associated with lower health risk. We have proposed that excess visceral fat may be a marker of subcutaneous adipose tissue dysfunction not being able to serve as a metabolic sink, causing lipid accumulation at undesired sites, a condition described as ectopic fat deposition. Among the effective approaches to prevent, delay, or manage metabolic syndrome, lifestyle changes are the key elements, with an emphasis on the importance of healthy global dietary patterns, regular physical activity, and adequate sleep quality.
Retail Strategies to Support Healthy Eating
In January 2020, the Center for Science in the Public Interest (CSPI), The Food Trust, Johns Hopkins Bloomberg School of Public Health, and Healthy Eating Research (HER) met for a Healthy Retail Research Convention in Washington, D.C. Attendees included food industry representatives, researchers, and nonprofit organizations. The objective of the convention was to develop a national healthy retail research agenda by (1) determining the effectiveness of government policies, corporate practices, and in-store pilots in promoting healthy eating; (2) identifying gaps in the healthy food retail literature and generating questions for future research, with an intentional focus on reducing health disparities and improving equity; (3) highlighting best practices for partnering with retailers and food manufacturers on healthy retail research; (4) facilitating relationships between retailers and researchers to implement and evaluate retail interventions; and (5) identifying existing datasets, ongoing work, and new opportunities for retail-research partnerships.
Population-Based Nutrition Epidemiology
Nutritional epidemiology examines dietary or nutritional factors in relation to the occurrence of disease in various populations. It is a fact that substantial progress has been made in recent years in nutritional epidemiology. Compared to the practice during the 1990s, and with the improvements in data analytics, several new approaches are gaining ground. Results from a variety of large-scale studies in the field of nutrition epidemiology have substantially contributed toward the evidence used in guiding dietary recommendations for the prevention of cardiovascular diseases, metabolic disorders, some types of cancer, and other morbidities. In this Special Issue, we would like to bring readers closer to the state-of-the-art in the field by gathering papers covering different aspects of nutrition epidemiology from population-based observational studies. Advance in Gluten-Free Diet
The only effective and safe treatment of celiac disease (CD) is a lifelong, strict exclusion of gluten, the so-called gluten-free diet (GFD). As a consequence, strict adherence to the GFD is highly successful and useful to achieve optimal control of symptoms in celiac patients, although, sometimes, nutritional problems can persist despite a strict exclusion of gluten. However, following a strict GFD is not easy and an updated quality assessment of available products is needed for further improvement in gluten-free product development. Similar to CD, GFD is the common dietary approach in non-celiac gluten/wheat sensitivity (NCGWS). NCGWS is another common gluten-related disorder without the diagnostic features of CD. Increasing interest in the association and interaction between irritable bowel syndrome (IBS), functional dyspepsia, and gluten-related disorders can expand our knowledge and understanding of the management of these disorders. In this respect, GFD is considered a therapeutic option in IBS and functional digestive disorders. New insights into the GFD are an exciting scientific challenge for researchers.

Polyphenols are a heterogeneous group of bioactive compounds mainly found in plant-based foods. Numerous clinical and epidemiological studies have led to the result that polyphenol intake may protect against chronic diseases such as cardiovascular and neurodegenerative diseases, cancer, or type 2 diabetes, to name some. Polyphenol intake estimation can be obtained through food frequency questionnaires and nutritional biomarkers, both having their own advantages and disadvantages. Although the association between these bioactive compounds and health seems irrefutable, many questions remain still unanswered. Community health workers (CHWs) are an increasingly important member of the healthcare and public health professions who help build primary care capacity. Dietary Plant Origin Bio-Active Compounds, Intestinal Functionality and Microbiome
Plant-based diets contain a plethora of metabolites that may impact on health and disease prevention. Most are focused on the potential bioactivity and nutritional relevance of several classes of phytochemicals, such as polyphenols, flavonoids, carotenoids, phyto-oestrogens, and frucrooligo-saccharides. These compounds are found in fruit, vegetables, and herbs. Daily intakes of some of these compounds may exceed 100 mg. Moreover, intestinal bacterial activity may transform complex compounds such as anthocyanins, procyanidins, and isoflavones into simple phenolic metabolites. The colon is thus a rich source of potentially active phenolic acids that may impact both locally and systemically on gut health. Further, nondigestible fiber (prebiotics) are dietary substrates that selectively promote proliferation and/or activity of health-promoting bacterial populations in the colon. Prebiotics, such as inulin, raffinose, and stachyose, have a proven ability to promote the abundance of intestinal bacterial populations, which may provide additional health benefits to the host. Further, various pulse seed soluble (fiber) extracts are responsible for improving gastrointestinal motility, intestinal functionality and morphology, and mineral absorption. Studies indicated that the consumption of seed origin soluble extracts can upregulate the expression of BBM proteins that contribute for digestion and absorption of nutrients.

Governments have known since the 1960s that smoking results in irreversible health damage. This open access book examines why governments have done so little to combat this when they have been aware of the problem and its solutions for decades.
