Implementation and Scale Up of Point of Care (POC) Diagnostics in Resource-Limited Settings

Scale up and implementation of new point-of-care (POC) diagnostics is a global health priority to enable the adoption of new evidence-based POC diagnostics and to replicate and extend the reach of POC diagnostics. Global private and public sector agencies have significantly increased their investment in the development of POC diagnostics to meet the unmet needs of patients in resource-limited settings, particularly disease burdened settings with limited access to laboratory infrastructure. However, previous research has demonstrated that the availability of health technologies in these settings does not always guarantee patient-centered outcomes. The applicability, effectiveness and sustainability of diagnostic technologies is affected by the involvement of all stakeholders during planning and implementation, which must be relevant to each specific context and sensitive to local culture. Factors such as infrastructure, resources, values and characteristics of participants can influence the implementation, scalability and sustainability of health interventions such as POC diagnostics. Chapters 1-4 discuss the history and background of the original paper-based patient records, their purpose, and how they are written and structured. These initial chapters do not require any technical or medical background knowledge. The remaining eight chapters are more technical in nature and describe various medical classifications and terminologies such as ICD diagnosis codes, SNOMED CT, MeSH, UMLS, and ATC. Chapters 5-10 cover basic tools for natural language processing and information retrieval, and how to apply them to clinical text. The difference between rule-based and machine learning-based methods, as well as between supervised and unsupervised machine learning methods, are also explained. Next, ethical concerns regarding the use of sensitive patient records for research purposes are discussed, including methods for de-identifying electronic patient records and safely storing patient records. The book's closing chapters present a number of applications in clinical text mining and summarise the lessons learned from the previous chapters.The book provides a comprehensive overview of technical issues arising in clinical text mining, and offers a valuable guide for advanced students in health informatics, computational linguistics, and information retrieval, and for researchers entering these fields. Gestational diabetes mellitus (GDM) is one of the most common adverse medical conditions that occurs during pregnancy, and its prevalence is rising as part of a diabetes pandemic. Nutrition plays a key role in GDM, whether (1) as part of an 'unhealthy' diet, which contributes to its cause, or (2) as part of changes in dietary intake, which act as the frontline treatment for GDM (sometimes supplemented with exercise and pharmacological intervention). Dietary changes, therefore, can alter the risk of developing GDM in the first place, and once GDM has emerged during pregnancy, dietary changes can mitigate the risk of developing GDM-related complications, such as macrosomia, respiratory distress, hypoglycemia and jaundice in the neonate, pre eclampsia, increased need for caesarean section and placental abruption in the mother. In this Special Issue, we aim to highlight the role of nutrition in the aetiology of GDM, whether directly or indirectly through weight gain and obesity, and in its role as a GDM treatment to lower hyperglycemia and the risk of the aforementioned complications.

Newborn Screening for Cystic Fibrosis

The introduction and widespread implementation of newborn bloodspot screening (NBS) for cystic fibrosis (CF) has offered earlier diagnosis and better outcomes for children with CF in many countries of the world. It represents a paradigm shift in the diagnostic pathway for these families. In contrast to a clinical diagnosis, infants are now referred for diagnostic testing after a positive NBS result. The introduction of NBS has enabled the provision of early appropriate treatment to prevent the manifestations of the disease. In the near future, early diagnosis will facilitate the prompt use of new CFTR modulator therapies that correct the basic underlying molecular defect. NBS for CF has been a global success but continues to raise questions with many varied approaches and the development of new technologies, in particular the ability to undertake extensive gene examination. Which is the best protocol to achieve high sensitivity and specificity, and how to evaluate and manage infants with inconclusive diagnosis are all subjects of ongoing discussion. It is also open to question: what is the best approach to informing and counselling the parents about a positive or inconclusive NBS result? These questions are not easy to answer and require a balanced solution that reflects the local health care system and may appropriately result in different answers around the globe.
