Then somewhere along the way, the burning passion and motivation that initially provided all the energy necessary to make a difference in the lives of others gets replaced with a sense of frustration and futility. Once this pattern emerges, the template for personal trauma is in place. Until that pattern is interrupted, the risk to both professional and patient is on an escalating trend line. Danger is near. I wish I could claim to have THE answer, a single path forward to navigate away from the abyss. But the truth is much more modest. What I do have is a simple mental model that has helped me reconnect to purpose. I am going to share it with you in hopes that your version will help you. For me, when I think back to all the patient visits that began with me saying, "TELL ME," the acronym takes me directly to that sense of purpose. You see, "TELL ME" was always an invitation. "TELL ME how things are going," or "TELL ME what brings you in today" provided a powerful opening for my patient to talk, and for me to listen. The connection soon followed, yet "TELL ME" has always been the start. In this book, I will explain what "TELL ME" has come to represent for me.The acronym is simple, but not always easy to do. As we go through each chapter, I will highlight some of the "AHA" moments that have occurred to me during my career. It is my hope that these simple representations will stimulate thinking so that you are able to attach your career experiences to the framework. As that happens, it is my hope that your sense of purpose can be refueled in meaningful ways, that you can find ways to ignite others, and that we all move a little closer to understanding how much we count on each other for health and quality of life as we strive to help others overcome their most difficult life challenges. The clinician-patient relationship is an unapologetic reminder that clinical practice is an applied science, and that clinical knowledge only becomes useful through human interaction and application. Through dialog, the relationship serves as a powerful conduit for information flow. As such, it is the clinician's job to develop skills for impasse resolution and interview techniques to explore the personal, family and cultural relational dynamics and nuances of meaning, lying within the illness narrative. While physicians, physician assistants, nurse practitioners, and others are well-trained to take inventory of the body's biological systems, practitioners often lack the skill set to ask "the right questions" for exploring psychosocial systems, and therein lies the pedagogic vacuum to be filled. This book aims to give a comprehensive overview of the current challenges and solution posed to the health care professionals who need to use mechanical ventilation to treat their patients.Mechanical ventilation is a cornerstone of the treatment of critically ill patients, as also dramatically underlined by the recent COVID-19 pandemic. The topic is not simple to approach, since it requires integration of multiple data which, in turn, result from complex interplays between patient's condition and ventilatory settings. While technological development empowered advanced monitoring and decision support, these also increase the burden of data on the practitioners. Furthermore, considering that sometimes mechanical ventilation is seen under two, apparently opposite, approaches, "physiology vs. protocols", the book aims to reconcile these two aspects. And this has been done by each author following the above trajectory in their chapters. The exposure of the topic begins from the "pathophysiology" (i.e. the "physiology of the disease") so that the reader can better understand the concept and rationale of any given approach. At the same time, any rationale or hypothesis (for as much as supported by physiology) must hold at the proof of clinical research and evidence, which is summarized in each chapter.In summary, the purposes is that the readers understand not only which is the best clinical practice to adopt but also why and which mechanisms this is based upon and how to approach a novel issue they might encounter. The book - addressed to physicians, nurses and respiratory therapist - features chapters on "novel" or "hot" topics like, obviously, COVID-19, ECMO, but also MV in low resource setting.

Cuban citizens forged an activist political subculture around tuberculosis, rejecting discourses that blamed the sick for their own illness. This loose coalition of sanatorium patients, tenement dwellers, black public intellectuals, labor organizers, and reform-minded physicians won entitlements to state health care and pressed for other social rights that influenced health. Their critiques of the state's politicized and inefficient tuberculosis program contributed to the declining legitimacy of the Batista government, helping to spur the Revolution and an innovative restructuring of the public health system.

New Orleans is a city that is rich in culture, music, and history. It has also long been a site of some of the most intense racially based medical inequities in the United States. Kevin McQueeney traces that inequity from the city's founding in the early eighteenth century through three centuries to the present. The former concentrates mainly on the diagnosis and management, i.e., what is the diagnosis, what is the next best step, what feature is consistent with your preliminary diagnosis, and so on. The latter concentrates on several updates in the management of many diseases, addresses some novel diseases and topics, and focuses on COVID-19. Undoubtedly, if you are well-prepared, you will pass the examination very easily. No need to panic when you hear about your colleagues' experiences. Lack of preparation is the single most common reason for failure. Remember, practice makes perfect. Read and self-assess; that's it! Good luck with your career and exams!
