Transgender Health - Advances and New Perspectives
Trans people suffer from significant health disparities in multiple areas, one of which is ​​public health. Real or perceived stigma and discrimination within biomedicine and healthcare delivery, in general, can affect trans people's desire and ability to access appropriate care, thereby impacting their own health. The biggest barrier to both safe hormone therapy and adequate general medical care for transgender patients is the lack of access to care. Do You Speak Patient?
Why join the medical profession in the first place? For many medical professionals, the drive is a powerful desire to conquer disease combined with a solemn covenant to care for people - fellow human beings - to the best of one's abilities and judgments. This sense of purpose combined with the fulfillment obtained through complex problem solving becomes a powerful motivator that allows one to push the limits of personal growth during education and training.After medical school, residency training and fellowship, or one of the many educational tracks in healthcare, professionals set out to deliver quality healthcare, often with inadequate early mentorship. Then somewhere along the way, the burning passion and motivation that initially provided all the energy necessary to make a difference in the lives of others gets replaced with a sense of frustration and futility. Once this pattern emerges, the template for personal trauma is in place. Until that pattern is interrupted, the risk to both professional and patient is on an escalating trend line. Danger is near. I wish I could claim to have THE answer, a single path forward to navigate away from the abyss. But the truth is much more modest. What I do have is a simple mental model that has helped me reconnect to purpose. I am going to share it with you in hopes that your version will help you. For me, when I think back to all the patient visits that began with me saying, "TELL ME," the acronym takes me directly to that sense of purpose. You see, "TELL ME" was always an invitation. "TELL ME how things are going," or "TELL ME what brings you in today" provided a powerful opening for my patient to talk, and for me to listen. The connection soon followed, yet "TELL ME" has always been the start. In this book, I will explain what "TELL ME" has come to represent for me.The acronym is simple, but not always easy to do. As we go through each chapter, I will highlight some of the "AHA" moments that have occurred to me during my career. It is my hope that these simple representations will stimulate thinking so that you are able to attach your career experiences to the framework. As that happens, it is my hope that your sense of purpose can be refueled in meaningful ways, that you can find ways to ignite others, and that we all move a little closer to understanding how much we count on each other for health and quality of life as we strive to help others overcome their most difficult life challenges. And of course, as has always been the case, "TELL ME" is just a beginning.
The Circle of Change
The clinician-patient relationship is an unapologetic reminder that clinical practice is an applied science, and that clinical knowledge only becomes useful through human interaction and application. Through dialog, the relationship serves as a powerful conduit for information flow. This text, utilizing stories of successes and failures I have journaled over 44-years practicing medicine, argues that there is hidden knowledge important to patient care beyond technology's reach that can only be gained through mutual trust, rapport and the right questions asked. But there are many external factors that constrain and strain the clinician-patient relationship. Institutional and cultural restrictions foisted upon medical practice are laden with bureaucratic, political, and economic demands which impinge upon time spent with the patient. Correcting the situation is challenging because many of the causative factors are of a societal nature and not within one individual's influence. But nurturing the clinician-patient relationship and harvesting information from patients' stories that may explain conflict, impasse, resistance to plan of care and lifestyle changes is well within our scope and is in fact necessary for good care. As such, it is the clinician's job to develop skills for impasse resolution and interview techniques to explore the personal, family and cultural relational dynamics and nuances of meaning, lying within the illness narrative. While physicians, physician assistants, nurse practitioners, and others are well-trained to take inventory of the body's biological systems, practitioners often lack the skill set to ask "the right questions" for exploring psychosocial systems, and therein lies the pedagogic vacuum to be filled. It is the purpose of this text to introduce a meaning-based problem-solving approach, The Circle of Change, as a pragmatic map and compass for gathering and processing patients' narratives to uncover meaning and care for

Summer's journey was bitter sweet, with highs and lows while she matriculated through the difficult times of assisting her mom. The love these two women shared with each other was priceless! Summer actually started writing this book as her form of therapy, but it also was her way to have written memories of some of the good and bad times with her mom. Mechanical Ventilation from Pathophysiology to Clinical Evidence
This book aims to give a comprehensive overview of the current challenges and solution posed to the health care professionals who need to use mechanical ventilation to treat their patients.Mechanical ventilation is a cornerstone of the treatment of critically ill patients, as also dramatically underlined by the recent COVID-19 pandemic. The topic is not simple to approach, since it requires integration of multiple data which, in turn, result from complex interplays between patient's condition and ventilatory settings. While technological development empowered advanced monitoring and decision support, these also increase the burden of data on the practitioners. Furthermore, considering that sometimes mechanical ventilation is seen under two, apparently opposite, approaches, "physiology vs. protocols", the book aims to reconcile these two aspects. And this has been done by each author following the above trajectory in their chapters. The exposure of the topic begins from the "pathophysiology" (i.e. the "physiology of the disease") so that the reader can better understand the concept and rationale of any given approach. At the same time, any rationale or hypothesis (for as much as supported by physiology) must hold at the proof of clinical research and evidence, which is summarized in each chapter.In summary, the purposes is that the readers understand not only which is the best clinical practice to adopt but also why and which mechanisms this is based upon and how to approach a novel issue they might encounter. Radical Prescription
Extinguishing a public health threat is difficult under any condition, let alone during a sweeping national revolution. In this first comprehensive study of tuberculosis in modern Cuba, Kelly Urban analyzes the medical, social, and governmental responses to the highly contagious disease as the island was heading into and emerging from the Revolution of 1959, providing a window onto broad questions of citizens' rights, biomedicine and public health, and political change.Drawing on a diverse range of sources revealing the perspectives of those at the center of power and those on the margins, Urban finds that the Cuban republican state intervened to confront the tuberculosis problem only after coming under intense grassroots pressure. Cuban citizens forged an activist political subculture around tuberculosis, rejecting discourses that blamed the sick for their own illness. This loose coalition of sanatorium patients, tenement dwellers, black public intellectuals, labor organizers, and reform-minded physicians won entitlements to state health care and pressed for other social rights that influenced health. Their critiques of the state's politicized and inefficient tuberculosis program contributed to the declining legitimacy of the Batista government, helping to spur the Revolution and an innovative restructuring of the public health system.

New Orleans is a city that is rich in culture, music, and history. It has also long been a site of some of the most intense racially based medical inequities in the United States. Kevin McQueeney traces that inequity from the city's founding in the early eighteenth century through three centuries to the present. He argues that racist health disparities emerged as a key component of the city's slave-based economy and quickly became institutionalized with the end of Reconstruction and the rise of Jim Crow. McQueeney also shows that, despite legislation and court victories in the civil rights era, a segregated health care system still exists today.In addition to charting this history of neglect, McQueeney also suggests pathways to fix the deeply entrenched inequities, taking inspiration from the "long civil rights" framework and reconstructing the fight for improved health and access to care that started long before the boycotts, sit-ins, and marches of the 1950s and 1960s. Practical Strategies to Assess Value in Health Care
These days, the idea of "value" is at the center of many activities and decisions in health care in the United States. While there exist books that detail the technical steps for how to carry out a specific type of value assessment, such as cost-effectiveness or return on investment, there are few that attempt to teach healthcare professionals how to think about value. This is the third edition of my book, "Self-Assessment for MRCP(UK) part 1", which was published in 2009. The second edition, entitled "Self-Assessment: 650 BOFs for MRCP(UK) and MRCP(I) Part I" was published in late 2016. It has been six years! The third edition has undergone several changes and updates. COVID-19 strikes every chapter. In authoring this book, I tried my best to include the most common examination themes. You may encounter negative or positive stems. The questions' objective is to teach, i.e., a rapid review of every subject and theme. This book differs from my book "Get Through MRCP part I; BOFs," which was published in the year 2008 by the Royal Society of Medicine Press in London, and my book "MRCP(UK) and MRCP(I) Part I Best of Fives, Volume I", which was published in 2022. The former concentrates mainly on the diagnosis and management, i.e., what is the diagnosis, what is the next best step, what feature is consistent with your preliminary diagnosis, and so on. The latter concentrates on several updates in the management of many diseases, addresses some novel diseases and topics, and focuses on COVID-19. Undoubtedly, if you are well-prepared, you will pass the examination very easily. No need to panic when you hear about your colleagues' experiences. Lack of preparation is the single most common reason for failure. Remember, practice makes perfect. Read and self-assess; that's it! Good luck with your career and exams!
