Unsere heutige Gesellschaft ist gepr瓣gt von Bewegungsmangel und zu hoher Energiezufuhr 羹ber die Nahrung. Dies f羹hrt langfristig von ?bergewicht bis hin zu Adipositas, was zahlreiche gesundheitliche Komplikationen mit sich bringt. So nimmt nach der Deutschen Diabetes Gesellschaft ab einem BMI von 27 bis 30 die Mortalit瓣t an Herz-Kreislauferkrankungen und Karzinomen zu. Dem geht einher, dass immer mehr Menschen 羹bergewichtig und adip繹s sind. Dabei ist Adipositas nicht nur ein nationales, sondern ein globales Problem. 1980 waren 857 Millionen Menschen weltweit 羹bergewichtig oder adip繹s, 2013 waren es schon ca. 2,1 Millionen. Neue Studien zeigen nun, dass starkes ?bergewicht bzw. Adipositas nicht zwingend mit metabolischen Anomalien einhergehen muss. Dieses neue Konzept wird als metabolisch gesunde Adipositas ("Metabolically Healthy Obesity") bezeichnet und ist bislang in der Wissenschaft noch umstritten. So weisen nach Brinkmann (2013) 20 % der adip繹sen Menschen kein erh繹htes Risiko f羹r metabolische Krankheiten auf. Aus diesem Grund soll innerhalb der vorliegenden Arbeit 羹berpr羹ft werden, ob die Mindestmengenregelungen und die Qualit瓣tsindikatoren im Bereich des Einsetzens von k羹nstlichen Kniegelenken aus den Jahren 2015, 2016 und 2017 signifikante Hinweise darauf geben, ob die Einhaltung der Mindestmengen auch zu einer Verbesserung der Qualit瓣t f羹hrt. Dazu werden die Qualit瓣tsindikatoren "Indikation zur elektiven Knieendoprothesen-Erstimplantation" und die "Beweglichkeit bei Entlassung" des Krankenhauses betrachtet. Im Jahr 2004 hat der Gemeinsame Bundesausschuss (G-BA) eine Mindestmengenregelung f羹r ausgew瓣hlte planbare Behandlungen eingef羹hrt, u.a. f羹r das Einsetzen eines k羹nstlichen Kniegelenks. Bei diesen Behandlungen besteht ein Zusammenhang zwischen der H瓣ufigkeit einer durchgef羹hrten Operation und der daraus resultierenden Behandlungsqualit瓣t. Ziele der Mindestmengenvereinbarung sind zum einen die Erh繹hung der Versorgungsqualit瓣t aus Patientensicht und zum anderen die Einschr瓣nkung der Einrichtungen, die solche Operationen durchf羹hren k繹nnen. Denn nur ein Krankenhaus, welches die vorgegebenen Mindestmengen erreicht, darf auch im Folgejahr diese Operation erneut durchf羹hren und zulasten der Krankenversicherungen entsprechend abrechnen. Trotz dieser strikten Vorgaben gibt es M繹glichkeiten, bei denen auch Einrichtungen, die nicht die vorgegebene Mindestmenge erreichen, die Behandlung weiterhin durchf羹hren k繹nnen.
