Balloon kyphoplasty in spinal fracture surgery

Spinal cord injuries remain a serious problem for all mankind. They not only cause pain, but also significantly limit the social activity of the injured. New types of minimally invasive surgical interventions are being developed to correct and stabilise the spine. One of the promising trends in this area is balloon kyphoplasty. The essence of the method consists in percutaneous insertion of a special balloon into the body of the affected vertebra. The balloon is inserted in a compressed state, and then fluid is injected into it under pressure. The balloon spreads in the vertebral body, providing reduction of post-traumatic compression. The balloon is then removed and the cavity formed is filled with paste-like bone cement. The methacrylate-based bone cement polymerises during the operation, which provides primary stability. The advantages of this method include minimally invasiveness, rapid clinical effect, the use of local anaesthesia, and the possibility to perform the intervention on an outpatient basis. The book will be of interest to orthopaedic traumatologists, rheumatologists, neurologists, neurosurgeons and medical students.

Our bodies need protection against pathogen invasion and for that we employ our immune system which is armed with an arsenal of cells and proteins. This immune network is destined to eliminate microbes and substances determined to pose significant threat to the normal functioning of our bodies by confronting viruses, bacteria, toxins, fungi and parasites, or even by battling tumoral cells. So, the immune system develops mechanisms at least equal in effectiveness and ingenuity to the infectious or transforming threat. At the same time, the maintenance of homeostasis and prevention of self-destruction is a very important function managed also by the immune system via tolerance mechanisms. This orchestration and customisation of the immune system aim in securing our best survival1,2. The network of immune cells is usually divided into the innate and adaptive leg (figure I.1). The first harbours ready-for-action cells (e.g. monocytes, neutrophils, natural killer cells) and the second highly specialised agents (T and B lymphocytes). Even though separated in categories the cross talks and cooperation between the two legs are constant and crucial. T and B lymphocytes recognise and bind a broad range of antigens through the highly diverse antigen receptors on their surface (T-cell receptor for T lymphocytes and B-cell receptor for B lymphocytes) and confer the essential aspect of immunological memory1. In the T spectrum, there are unconventional cases of more rapid responders such as mucosal-associated invariant T (MAIT) cells (αβ), natural killer T (NKT) cells (αβ) or the γδ T cells3. The focus of this work is on the human γδ T cells and especially the Vγ9Vδ2 T cell population which provide unique functional specialisation.

Epidemiological profile of dyslexic patients in ABC Paulista - Brazil

Dyslexia is a disorder that causes difficulty in reading and also compromises written language. Studies show that dyslexia has genetic characteristics, with heredity in between 23 and 65% of cases. Dyslexia affects between 5-17% of schoolchildren. The first description of this condition dates back to 1896 and was described by the English neurologist Pringle Morgan, who named it congenital verbal blindness. Since then, various theories have been put forward about the origin of this disorder, the most widely accepted today being the neurobiological cause. The Brazilian Dyslexia Association (ABD), as well as the International Dyslexia Association (IDA) and the National Institute of Child Health and Human Development (NICHD), define dyslexia as follows: "a specific learning disorder, of neurobiological origin, characterized by difficulties in accurate and/or fluent word recognition, decoding skills and spelling. These difficulties usually result from a deficit in the phonological component of language and are unexpected in relation to age and other cognitive abilities."

Immunological and Molecular Study of Human Metapneumovirus

The aim of the book is to identify the role of human Metapneumovirus in children with respiratory infections. This study was conducted on 100 patients with respiratory tract infections who were admitted to Al Zahra Teaching Hospital and the AL-Forat AL-Awsat Teaching Hospital in Najaf Governorate during the period November 2020 to February 2021, Nasopharyngeal swabs were collected from patients to molecular diagnosis of human metapneumovirus by Real-Time-PCR, serum specimens were also collected from patients and control to measure the concentration of both TNF-α and IL-10 using ELISA. The results obtained from nasopharyngeal samples were amplified by RT-PCR for the diagnosis of hMPV the percentage of samples were positive for the virus (53%) from (100) specimens.

Urinary Lithiasis in Children

Urinary lithiasis in children poses a serious problem, affecting the functional future of the kidney. Etiological investigation must be exhaustive in order to avoid recurrence and prevent impairment of renal function. A retrospective study of 168 observations of urinary lithiasis collected over 8 years. All stones were analyzed by infrared spectrophotometry.The annual average was 21 cases, the most frequent symptomatology being urinary tract infection. The stone was located in the upper tract in 65% of cases. Lithiasis was associated with a metabolic etiology in 17% of cases, and with malformative uropathy in 20%. Treatment consisted of open surgery in 96% of cases. Calculi were pure in 36% of cases. Whewellite was present in 72% and predominant in 38% of stones.The therapeutic indication depends essentially on the size and location of the stones. Extracorporeal lithotripsy can be considered as 1st-line treatment, or in association with percutaneous nephrolithotomy and ureteroscopy. Surgery is still indicated when minimally invasive techniques fail.

Chronic shoulder instability in young people

It should be noted that anterior shoulder instability is not only clinically manifested by anteromedial dislocation, but that there are also frustrated clinical forms.The bone block described in 1954 by Latarjet was modified by Patte in 1980 under the name of triple locking (bone, muscle and capsular).This technique gives good functional results: 92% of patients are satisfied.

Bone fluorosis in Tunisia

Bone fluorosis results from the accumulation of fluorine in bone tissue due to prolonged exposure to high doses, mainly through ingestion of contaminated water. Initially asymptomatic, it can manifest itself in adulthood with serious neurological complications.In our study of 18 patients, fluorosis was diagnosed by epidemiological and radiological data, due to the unavailability of biological data. Symptoms were often severe, but stabilization of signs was observed in the majority of cases. Sources of exposure included hydro-telluric intoxication and two cases of occupational intoxication. Improving the water distribution network could help limit fluorosis. Worldwide, studies are mapping endemic areas and exploring genetic predispositions.Although cases of bone fluorosis are declining, our understanding of the disease remains crucial, as new aspects are emerging and arousing the interest of researchers.

Hepatitis E virus, history and epidemiological trends

The hepatitis virus (HEV), the fifth viral hepatitis virus identified in 1983, is a small, naked ribonucleic acid virus with 8 genotypes distributed worldwide. In recent years, a better understanding of the natural history of HEV infection has clarified its epidemiology. Hepatitis E is the world's leading cause of acute non-A viral hepatitis of fecal-oral or enteric transmission. It is now recognized as a serious threat to global public health, and has become a major concern in both industrialized and underdeveloped countries. According to the World Health Organization, an estimated 2.3 billion people (a third of the world's population) have already been infected with HEV. In endemic regions such as Asia, Africa and Central America, HEV is the cause of major epidemics of acute hepatitis, mainly of waterborne origin linked to faecal peril. In various industrialized countries of Europe and America, hepatitis E has long been considered an infection contracted during a stay in an endemic zone. Improved virological tools have shown that many cases of hepatitis are in fact a sporadic, locally-acquired form, defining the emerging nature of this infection.
