Doctoral Thesis / Dissertation from the year 2018 in the subject Medicine - Gynecology, Andrology, grade: PhD, language: English, abstract: The aim of this study is to evaluate the frequency of HPV infection and genotypes among women with normal and abnormal cytological results and its correlation according to the severity of the lesions as well as to investigate the correlation between the TERT gene expression levels and the cytological sub-group and to evaluate the correlation between the HPV infection and TERT gene expression levels as diagnostic and prognostic marker in cervical precursor lesions and cervical cancer. Early events associated with disease progression and cervical cancer include hTERT up-regulation at the transcriptional level mediated by high-risk HPV E6 oncoproteins via indefinite mechanisms with concomitant immortal phenotype of the cell in vitro and increased replicative potential of cells in precancerous cervical lesions and cervical cancer. Materials and methods: This study is a prospective observational case-control study conducted in Obstetrics and Gynaecology Clinic in Pristina and Molecular and Biology-Genetics Department in Skopje. Cervical samples from 214 women (median age 45.28 years; range 20-65) from the Outpatient Clinic in University Clinical Center in Kosovo were tested for HPV-DNA and quantitative TERT gene expression after performing conventional Pap smear.

Multimodality Treatments in Metastatic Gastric Cancer
Gastric cancer represents one of the most frequent and lethal tumors worldwide today, finding itself in the fifth place in incidence and the third in mortality. Surgery remains the only curative treatment for localized tumors, but only 20% of patients are suitable for surgery due to the lack of specific symptoms and the late diagnosis, especially in Western countries. Additionally, even in patients who receive curative treatment, rates of locoregional relapse and distant metastasis remain high. Palliative chemotherapy is the principal treatment in cases of metastatic disease even if the prognosis of patients receiving chemotherapy is still poor. Therefore, a multidisciplinary evaluation is important in order to improve the efficacy of active treatments. In this context, there is an unmet need for a better understanding of genetic alterations and prognostic and predictive factors in order to choose the best tailored therapy for each patient.The aim of this Special Issue is to focus on the results and problems of multimodality treatment in metastatic gastric cancer, the search for prognostic and predictive factors, and the evaluation of novel strategies for individualized treatment. We are inviting relevant original research, systematic reviews, meta-analyses, and short communications covering the above-mentioned topics.
Science and Logic in Medical Diagnosis
Clinicians and patients know the importance of correct diagnosis for proper treatment. So important - a correct diagnosis - it is the first theme of this book. Several other themes are also important. A second theme is established as "cause and effect," common or peculiar linked entities in at least some humans. A third theme deals with probabilities, both objective and statistical and/or subjective probabilities. The fourth theme covers the inhomogeneity among all humans. That is, all people are unique. Is science a theme? Not of this kind. Instead, science encompasses all of the themes above and others. Logic holds their framework. The diagnostic model above has been modified by allowing wider scope of inference similar to the earlier "differential diagnoses" model. A quite radical model called "Evidence-Based Medicine (EBM)," was created about thirty years ago. Its advocates depend largely on statistics, with little interest in science. Discussion and comparisons with traditional and EBM models argue here undesirable shortcomings of the latter. Far from dismissing science, but robust medical science, knowledge, experience and professional clinicians continues in caring her/his individual patients.
