Lumbo-aortic curage in pelvic gynaecological cancers
The role of lumbo-aortic curage is a hotly debated topic, due to the complexity of the procedure and the complications that can arise, both during and after the operation. It is crucial to identify the factors limiting this procedure and to assess its impact on the prognosis of patients with pelvic gynecological cancers. A retrospective study of 85 patients operated on at Ben Arous Hospital over a 19-year period revealed that factors such as obesity, complications during pelvic curage, type of cancer, and surgeon experience influence the performance of lumbo-aortic curage. Although this procedure is associated with complications, it significantly improves recurrence-free survival at 3 years. In conclusion, despite the technical challenges and associated risks, lumbo-aortic curage, when performed, appears to offer a survival benefit.

Prognostic factors for gastric cancer in Tunisia
Stomach adenocarcinoma is a serious disease, accounting for 90% of all malignant tumors of the stomach. It is the 5th most common cancer in the world, with over a million new cases each year. According to the national cancer registry (Nord-Tunisie; 2004-2006), stomach cancer ranks 6th in men and 7th in women.Our study is conducted through a retrospective series of 56 patients diagnosed with gastric adenocarcinoma from 2015 to 2020 in the Nabeul gastroenterology department (HTMN).The aim of our work was to study the epidemiological profile and prognostic factors of gastric cancer in our center.
Bronchiolitis obliterans in children
The aim of our work was to describe the clinical and paraclinical profile of children followed for obliterative bronchiolitis and to assess the impact of our management on their prognosis. Descriptive cross-sectional study including 18 children followed for obliterative bronchiolitis between January 2013 and December 2021. Mean age was 9.6y [3-17y] with a male predominance (gender-ratio=1.5). Bronchiolitis obliterans were post-infectious in 33% of cases and post-bone marrow transplant in 67%. Eight children were in chronic respiratory failure. All patients were on inhaled corticosteroids combined with bronchodilators in 77% of cases, anti-leukotrienes in 33% and macrolides in 72%. The mean duration of follow-up was 51 months. The average number of hospitalizations was 2.38 per year. The evolution was marked by weaning from long-term oxygen therapy in two children, stability in seven, worsening in six, and two deaths. Bronchiolitis obliterans is responsible for significant morbidity and mortality.

We conducted a descriptive, retrospective, monocentric study in the emergency and intensive care unit of the Habib Thameur Hospital in Tunis, over a period of six months. Chest trauma in Africa
Chest trauma is the second leading cause of trauma-related mortality worldwide. Their incidence is rising rapidly in Sub-Saharan Africa. Their management is complex, especially in a difficult context. The aim of this study was to describe current treatment modalities for thoracic injuries in two referral hospitals in Yaound矇, Cameroon. To this end, we conducted a cross-sectional study of 358 patient records admitted for thoracic trauma from January 1, 2016 to December 31, 2020 at the Centre Hospitalier et Universitaire de Yaound矇 (CHUY) and the Centre des Urgences de Yaound矇 (CURY). Statistical analysis was performed using SPSS 23.0 and MS Excel 2019. Of the 358 cases collected, 83% were male and 17% female, with a sex ratio of 5H/1F. The median age was 30 [23-40] years, with extremes of 1 and 96 years. The annual hospital frequency of thoracic trauma was 16.5%, with a peak in 2019. The mode of transport (93.6%) was very often non-medical. Pulmonary contusion (65.3%) was the main injury. Treatment was mostly conservative (70%). The overall mortality rate was 13.6%.

Teaching from the patient management problem
Abstract Introduction: The PMP is a pedagogical tool developed́ from the mid-1960s in the United States. The aim of this work was to evaluate the impact of learning with the PMP in practical nephrology teaching, as well as learners' perceptions of the value of the PMP in learning and assessment. Methods: We conducted a prospective evaluative study in the nephrology department of Mongi Slim la Marsa Hospital. We included DCEM2 externs. We prepared a PMP in electronic format on the management of threatening hyperkalemia in the chronic hemodialysis patient. The teaching session included a briefing phase, followed by the pre-test, the individual PMP session, the group debriefing phase, the post-test and the satisfaction questionnaire.Results: Thirty students participated in the study. All students combined, 83% scored average on the PMP assessment. The PMP led to a significant improvement in test scores, and consequently to the acquisition of new knowledge.
Physiotherapeutic treatment by dry needling
Dry needling is a technique used in physical therapy to treat myofascial pain by inserting fine needles especially into myofascial trigger points. These trigger points are hyperirritable areas within tight bands of muscle that cause referred pain and dysfunction. Dry needling treatment seeks to release muscle tension, improve local circulation and reduce pain.In the upper limb, this technique is frequently used in pathologies such as rotator cuff tendinopathies, epicondylitis, carpal tunnel syndrome, among others, achieving a significant improvement in mobility and functionality. The procedure is precise and requires a deep anatomical knowledge to avoid sensitive structures such as nerves or blood vessels. Dry needling is an effective option within a comprehensive physiotherapy approach, complementing other manual techniques and therapeutic exercises.

Factors predicting the success of intrauterine inseminations
Intrauterine insemination (IUI) is a simple method of assisted reproduction often used for unexplained infertility, but its results are sometimes disappointing. A 4-year retrospective study in Tunis examined 196 infertile couples to identify factors predictive of IUI success. With 328 cycles analyzed, the clinical pregnancy rate was 15.2%. Women with polycystic ovary syndrome had the best pregnancy rates (35.1%). Factors influencing success included duration of infertility, basal FSH level, sperm quality and quantity, and endometrial thickness. The results suggest that taking these parameters into account could improve the chances of IUI success.
Urinary tract infection in newborns
Urinary tract infection of the newborn is a particular entity which is characterized by its non-specific, often misleading clinical symptoms and its frequent association with malformative uropathy. UTI of the newborn is a condition that requires prompt medical attention due to its potential to cause severe complications. Early recognition of symptoms, accurate diagnosis and appropriate treatment are essential to preserve the renal and general health of the newborn. Prevention through good hygiene and monitoring of newborns at risk also helps reduce the incidence of this infection. In short, a proactive and systematic approach is crucial to guarantee the well-being of the youngest patients.Urinary infection remains a worrying pathology in newborns. Knowing how to prevent it and treat it in time helps reduce its morbidity and mortality.
Nephropathy associated with contrast media
Iodinated contrast nephropathy (ICN) is increasingly encountered in interventional cardiology. It is the third most common cause of acute renal failure in hospitals, after functional renal failure and drug-induced causes. It is a formidable complication that can hamper clinical prognosis and make subsequent management of patients more difficult. Several factors can predict the occurrence of this entity. The aim of this study is to identify factors predictive of the occurrence of nephropathy associated with iodinated contrast media in cardiology.Methodology: This is a retrospective study of 133 patients investigated by coronary angiography or treated by coronary angioplasty in the cardiology department of Mongi Slim Hospital over a 3-month period between April and June 2023.
Berger’s disease
IgA nephropathy (IgAN) is the most common primary glomerulonephritis worldwide. It is characterized by variability in its clinico-biological, histological and evolutionary manifestations. We conducted a retrospective descriptive, analytical and comparative study in the Department of Internal Medicine A at Charles Nicolle Hospital, Tunis, from 1992 to 2021. The study included all patients with primary IgA nephropathy confirmed by renal biopsy.Two hundred and thirteen patients were included. The mean age at diagnosis was 34簣12 years.
Chronobiology and glycemic balance in type 1 diabetics
Chronobiology is the science of biological rhythms. It is constantly expanding its areas of application in the medical field, despite the fact that it has not yet been fully integrated into everyday practice.The aim of our study was to compare the results of empirical and chronobiological glycemic control in type 1 diabetics at the Marc Sankal矇 Center (Dakar).A prospective, cross-sectional study was conducted over a 6-month period. It included type 1 diabetic patients being monitored and educated in insulin therapy. A survey form was used to assess capillary blood glucose values, control times and insulin dose injected.The first step was for the patient to record his or her daily fasting capillary blood glucose at his or her usual blood glucose control times over a one-month period.Then, this same control was carried out according to a chronobiological rhythm, i.e. between 8 a.m. and 10 a.m. (the time interval when the acrophase of the glycemic rhythm occurs).

Mammary tuberculosis
Mammary tuberculosis is a rare form of extra-pulmonary tuberculosis, even in endemic regions, representing only 0.06% to 0.1% of all tuberculosis cases. It is often confused with a malignant tumour because of its clinical and radiological similarities, and mainly affects women of childbearing age. Diagnosis is based on bacteriological and histopathological evidence, and treatment is mainly based on anti-tuberculosis drugs. Sometimes surgery is required.A literature review was carried out using the PubMed, Google Scholar and Web of Science databases. The search terms used were 'tuberculosis' and 'breast', and studies published up to 2023 in English and French were included.

Electrolyte Disorders in a Group of Hospitalized Patients
The aim of this book is to describe electrolyte disorders in a group of hospitalized HIV-infected patients in the city of Bamako. We conducted a retrospective analytical cross-sectional study in the infectious diseases department of the CHU du Point G, from 1 January 2011 to 31 December 2016. Our study parameters were ionogram results during hospitalisation. One hundred and twenty-six patients living with HIV who had a blood ionogram during hospitalisation were included. The mean natraemia value was 133.06簣12.16 mmol/l and the mean kalaemia value was 3.89簣1.15 mmol/l, with extremes of 83.00 and 166.00 mmol/l and 1.60 and 9.00 mmol/l respectively. The electrolyte disorders found were hyponatraemia (41.2%), hypokalaemia (12.7%), hypernatraemia (7.8%), hyperkalaemia (2.9%) and associated disorders (35.3%). Statistically significant associations were found between these disorders and certain events that occur frequently in HIV-infected patients. Electrolyte disorders are common in patients living with HIV.
Children suspected of being allergic to beta-lactam antibiotics
Many patients are falsely said to be allergic to betalactam antibiotics, with all the attendant restrictions. The aim of this study was to investigate the characteristics of children with suspected betalactam allergy, and to outline the diagnoses used. It was a cross-sectional, descriptive study of children suspected of being allergic to betalactam antibiotics. It involved 38 children. Typical urticaria was diagnosed in (52.6%). Typical maculopapular exanthema was noted in (18.4%). According to the B矇gaud score, chronological imputability was doubtful in N=18 patients (47%).Differential diagnoses were evoked in cases of doubtful semiological imputability (S1), such as infection N= 21 (55%), allergy to other concomitant treatments N=4 (10.5%), food allergy following seafood consumption N=2 (5.26%), and chocolate-induced histaminoliberation (2.6%). This study serves as a reminder of the possibility of alternative diagnoses other than allergy in the paediatric population, and encourages paediatricians to re-prescribe them after the pharmacovigilance survey.
Anesthesia in Shocked Patients
The aim of this work was to describe the epidemiological profile, means and methods of anesthetic management of shocked patients in the context of obstetric and digestive emergencies. We carried out a retrospective and descriptive study on the records of patients operated on for obstetric and digestive emergencies, during the period from January 1, 2011 to January 31, 2012 at the Yaound矇 Gyneco-obstetric and Pediatric Hospital. The number of patients included in the study was 144. The essential pathologies that caught our attention were listed and categorized. For obstetric emergencies, these were: ruptured ectopic pregnancies, haemorrhagic placenta previa and retroplacental haematomas, haemostasis hysterectomy, uterine rupture and haemoperitoneum. Digestive emergencies included intestinal occlusions and peritonitis. GEUR was the main pathology likely to induce hemodynamic instability. Half the patients were between 25 and 35 years of age.
Urinary incontinence
Urinary continence is the result of a balance between intra-vesical and urethral pressures.The International Continence Society (ICS) defines urinary incontinence as the involuntary and disturbing loss of urine.The layout of the pelvic floor explains why the consequence of childbirth is muscular trauma.We began by looking at the onset of postpartum urinary incontinence, perineal rehabilitation and the moments of exchange women enjoy during pregnancy and the postpartum period. Then, we developed and conducted 70 questionnaires with 70 deliveries in the postpartum services at AZIZA OTHMENA Hospital between February 10, 2014 and March 10, 2014 and a second questionnaire with 30 midwives to assess their knowledge about urinary incontinence and perineal rehabilitation.Midwives are primary care professionals, now responsible for women's reproductive health. Knowledge of the perineum and its physiology should be assessed at every consultation, and may or may not lead to pelvic-perineal education sessions.
