Research Progress of Plant Compounds for Diabetes and Its Complications
Numerous chronic complications appear in the evolution of diabetes mellitus type 2 and type 1, and they significantly influence the duration of life of the patient. Diabetes mellitus is a metabolic disease where pro-oxidant and pro-inflammatory mechanisms are intensely expressed. Given the numerous beneficial effects present in compounds extracted from different plants, such as reduction in insulin resistance, improvement of endothelial function, improvement of beta-cell function, improvement of lipid metabolism, antioxidant effect, and anti-inflammatory effect, the idea behind this Special Issue is to present significant results from original research or from international data in review articles. It is not only the potential chemical substances extracted form plants that are important, but also the delivery of these substances to target tissues; therefore, the Issu also focuses on modalities to improve the biological effects of these substances, such as nanotechnology. The standardization of these substances and their potential evolution toward drug development is the key concept of this Special Issue, with the ultimate goal to develop new drugs starting from beneficial plant compounds to target the complications of diabetes mellitus at the molecular level.
Hereditary Hemorrhagic Telangiectasia
Hereditary Hemorrhagic Telangiectasia (HHT), or Rendu-Osler-Weber syndrome, is a dominantly inheritable rare disease with a prevalence of 1:5,000 - 10,000 people. The diagnosis of HHT, as a rare disease, follows the Cura癟ao Diagnostic Criteria: Nosebleeds (epistaxis), whether spontaneous or recurrent; (multiple) telangiectases at characteristic sites, including the lips, oral cavity, fingers and nose; Internal lesions: arteriovenous malformations (AVMs) or telangiectases in the stomach, lungs, liver, brain and spinal cord; Family history: a first-degree relative with HHT, according to these criteria. When a patient meets at least three of these criteria, it is considered definite that they have HHT. Nowadays, three subtypes of HHT have been identified. HHT type 1 refers to mutations on the endoglin gene ENG; HHT type 2 refers to mutations on the activin A receptor-like type 1 (ACVRL1) gene; and the third one, named juvenile polyposis-hereditary hemorrhagic telangiectasia (JPHT or JPHHT) overlap syndrome, refers to mutations on the gene MADH 4. There are two other subtypes (HHT-3 and 4) in which the mutations have not yet been completely identified, but they are known to be located on the 5q31.3-q32 and 7p14 chromosomal regions, respectively.

Secondary Metabolites for the Reduction of Oxidative Stress
Oxidative stress is at the root of several pathological conditions. Reactive oxygen species (ROS) are normally produced inside cells, and their amount is finely counterbalanced by antioxidant enzymes such as SOD, GPx, and catalase. However, when this homeostasis is interrupted, cells suffer from oxidative stress that can also lead to cardivascular and neurodegenerative diseases, as well as cancer, to name a few.In order to help cells fight excess ROS, esogenous antioxidant molecules can be administered through diet. For example, fruit and vegetables are rich in secondary metabolites with antioxidant properties, as are many beverages obtained from natural products. Wine is at the base of the Frech paradox, supposedly because of the high amount of polyphenols contained. Olive oil is one of the base components of the Mediterranean diet and strongly contributes to its beneficial health effects. Several molecules contained in these products, although at low concentrations, have already been shown to exert antioxidant activity, among others, both in vitro and in vivo.This reprint collected contributions from eight original research articles as well as four review articles regarding the exploitation of secondary metabolites in natural products (or derived from them) for reducing oxidative stress.

Inhaled Treatment of Respiratory Infections
Respiratory infections are conventionally treated with oral or intravenous antimicrobials (antivirals, antibiotics, and antifungals). However, these routes of administration are not ideal because the required drugs are systemically delivered rather than being targeted to the respiratory tract. Higher doses may also be needed to achieve sufficient drug concentrations in the lungs, which may consequently increase the risk of adverse effects. On the other hand, the drugs can be efficiently delivered into the airways as inhaled aerosols. Lower doses can then be used to attain relatively high local concentrations. There are specific challenges to the development of inhaled formulations, such as the optimisation of their physicochemical stability and aerosol performance. In addition, antimicrobial resistance is an urgent global public health issue. Novel strategies are required to overcome these problems. The papers in this reprint focus on recent advancements in inhaled antimicrobials and vaccines, including those for viral (SARS-CoV-2), bacterial (Mycobacterium tuberculosis, Mycoplasma pneumoniae, Acinetobacter baumanii), and fungal infections (moulds).

Endoscopic Ultrasound-Guided Diagnosis and Treatment of Gastrointestinal Diseases
Diagnostic endoscopic ultrasound (EUS) is an indispensable tool for staging and histologic sampling of upper and lower GI lesions, aided by elastography, contrast-enhanced EUS and artificial intelligence in diagnostic EUS.In the early 1990s EUS became interventional and since then the scope and range of EUS-guided interventions has dramatically increased, extending from EUS-assisted rendez-vous ERCP, EUS-guided celiac plexus block/neurolysis and fiducial placement, and EUS-guided biliary and pancreatic drainage-both in regular and altered anatomy-to the treatment of cysts and solid lesions in the pancreas/liver with EUS-guided local therapies (such as the EUS-guided injection of antitumoral agents/immunotherapy or EUS-guided ablative techniques), EUS-guided coiling/glue injection for gastric varices, and EUS-guided drainage of abdominopelvic collections. The introduction of lumen-apposing metal stents (LAMSs) revolutionized the therapeutic capabilities of EUS by allowing the creation of new gastrointestinal anastomoses. This, in turn, made possible interventions that were previously part of the surgical or interventional radiology realm, such as EUS-guided cystogastrostomy (EUS-CG)/endoscopic necrosectomy, EUS-guided gastroenterostomy (EUS-GE), EUS-guided gallbladder drainage (EUS-GD), EUS-directed transgastric ERCP (EGDE), and EUS-directed transenteric ERCP (EDEE).These and many other tremendous advances, too many to enumerate in a short paragraph, make diagnostic and therapeutic EUS a very exciting field. In this Special Issue, we summarize the current state of the art regarding EUS and EUS-guided therapies.

The spectrum of acute complications ranges from respiratory failure to cardiac and cardiovascular injury, liver and renal disorders, neurologic manifestations, secondary infections, and coinfections, as well as disseminated intravascular coagulation.Unfortunately, COVID-19 is not only a short-term infection; patients may also present with long-term complications. Post-COVID-19 conditions are found more often after severe illness, but any patient may experience long COVID-19, even those who have mild illness or no symptoms. Also, due to the intrinsic characteristics of SARS-CoV-2 infection, the elderly and patients with certain medical conditions have been demonstrated to be particularly at risk for various complications.At present, our research critically analyzes and discusses current knowledge on the clinical characteristics of SARS-CoV-2 infection, focusing on acute and long-term complications.

Dear colleagues, Given the success of the past year, I am happy to announce the release of a second edition of the Special Issue "The Impact of ENT Diseases in Social Life".In this edition, we want to underline how the otorhinolaryngology sector has a significant impact on social life. About 10% of cancers that affect the population annually involve the head and neck region, and each year, the guidelines evolve and change. Emergencies within the otolaryngology sector are among the most common, with numerous increases in hospitalizations in the ENT departments (bleeding, abscesses, dyspnea, etc.). The chosen interventions can significantly improve patient quality of life, and can help to avoid future complications. Given the importance of this medical and surgical branch, I invite you to read our reprint, with otorhinolaryngology as the main topic. New Advances in Dyslipidemia
Dyslipidemia is an important risk factor for atherosclerotic cardiovascular disease (ASCVD). However, despite current traditional therapies (e.g., statins), significant residual risks for the onset of ASCVD remain, warranting the need to better understand newer therapeutic options for diminishing this prospect. This Special Issue will focus on state-of-the-art concepts in the evaluation and management of dyslipidemia, including the rationale for newly set goals in the lipid treatment of this disease given the recent international guidelines, global trends in the epidemiology of dyslipidemia, the latest concepts and therapies in the evaluation and management of familial hypercholesterolemia, novel therapies for lowering LDL-C beyond statin therapy, the status of lipoprotein(a) as a causal risk factor and newer therapeutic approaches to address it, the evaluation and management of dyslipidemia in children, current and emerging therapies for hypertriglyceridemia, and nutraceuticals to treat dyslipidemia in patients with COVID-19. This Special Issue will discuss state-of-the-art clinical trials and other research developments regarding newer and emerging therapies for the management of dyslipidemia to reduce the residual risks of ASCVD. The hope is to provide clinicians with the latest information on the key aspects of the evaluation and treatment of dyslipidemia, including newer non-statin approaches for decreasing the residual risks of ASCVD, such as the settings of elevated LDL-C, lipoprotein(a), and familial hypercholesterolemia.

This Special Issue "Current Management of Hip Fracture", in the "Surgery" section of MDPI's journal Medicina, reports international studies regarding the different aspects of the treatment of patients suffering proximal femur fracture. Biomarkers of Tumor Metastasis and Invasiveness
The molecular characterization of tumors, investigated particularly through proteogenomic analyses, has led to a revolution in cancer research. Over the last decade, the development of quantitative proteomics, together with other major technological breakthroughs, have identified candidate biomarkers for diagnosis, prognosis, and drug efficacy/resistance follow-up. These improvements have allowed researchers to explore the capability of cancer cells to invade, metastasize, and finally, destroy normal tissues and organs. In parallel, new hypotheses have been formulated, and the means cancer cells use to exploit their surrounding environment have begun to be deciphered, leading to new therapeutic approaches. This Special Issue covers all these aspects, revealing new, recent insights on the molecular networks controlling the tumor invasiveness process; dynamic interactions between cancer cells; and the host stroma, stemness and epithelial-to-mesenchymal transition, as well as the links between inflammation and tumor metastasis. The scope is extended to studies on all cancer histological types (original research articles and reviews) conducted on experimental tumor models, tumor samples, and/or biofluids from patients with cancer.

Update on X-linked Hypophosphatemia
Rickets and osteomalacia are associated with impaired mineralization in growth plate cartilage and the bone osteoid. Vitamin D deficiency has been one of the major causes for rickets and osteomalacia. In addition, it has been also known that there are diseases called vitamin D-resistant rickets/osteomalacia which cannot be cured by native vitamin D. Since the cloning of FGF23 in 2000, it has become clear that most cases of vitamin D-resistant rickets/osteomalacia are caused by excessive actions of FGF23. X-linked hypophosphatemia (XLH) is the most common cause of inherited FGF23-reltated hypophosphatemic rickets. Patients with XLH suffer from life-long morbidity of bone, cartilage, ligament, joint, tooth and muscle which significantly affects quality of life. Since FGF23 was shown to be the humoral factor causing hypophosphatemia in patients with XLH, anti-FGF23 antibody that blocks the actions of FGF23 has been developed and become clinically available in several countries. The introduction of this new drug together with advanced knowledge concerning physiological and pathophysiological significance of FGF23 has further stimulated research and promoted dissemination of fruits of the studies in this field. From these backgrounds, a variety of both basic and clinical topics concerning XLH are covered in this special issue to provide up-to-date summaries of the current understanding.
