Infection, Super Infection and Antimicrobial Management in ICU
Assessing the risk of difficult-to-treat bacterial superinfections, such as multidrug-resistant (MDR) Gram-negative bacilli, according to comorbidities, lung abnormalities or systemic diseases, the severity of pneumonia and the risks of drug-related effects are crucial points for the clinical management. Despite the wide-and not always appropriate-use of broad-spectrum antimicrobials, specific data about bacterial and fungal co-infection in COVID-19 patients are lacking, and confirmed evidence is urgently required to build prospective evidence and to support the development of antimicrobial policy and appropriate stewardship interventions. The present Special Issue aims to focus on the following key points: Importance of superinfections in COVID-19 patients who are typically characterized by immunosuppression related to viral infection, need for prolonged hospitalization, treatment with immunomodulatory therapies (steroids, monoclonal antibodies, etc.); Their impact on the outcome of a cohort of patients who are relatively young, with few comorbidities, but requiring prolonged invasive support; Incidence, characteristics and impact of bacterial infections caused by multidrug-resistant, Gram-negative pathogens in the context of COVID-19 disease; Incidence, characteristics and impact of fungal infections, such as the most known invasive aspergillosis but also candidemia and/or other fungal infections (e.g., mucormycosis); Antimicrobial surveillance, control and management measures as implemented on a local, national and international basis.

Nutritional Modulation of Dietary Sugars as a Strategy to Improve Insulin Resistance and Energy Balance in Diabetes
This Special Issue covers an important topic that has relevance not only from a scientific point of view but also from nutritional, policy, and industrial perspectives. We are currently being subjected to increasing obesogenic pressures, with sugars being hidden in many foods, creating the perfect environment for the slow but consistent progression of metabolic dysfunction. The consumption and impact of dietary sugars on the gastrointestinal system is discussed, together with the dietary sources of naturally occurring and added sugars, as well as the role of different diets and dietary regimens in preventing post-prandial sugar increases and hyperinsulinemia. The intestinal metabolism of dietary sugars, including the formation and absorption of advanced glycation end products and their impact on gut microbiota, is also considered. In terms of the impact of dietary sugars on other metabolic processes, the increased and sustained consumption of sugars causes long-term alterations in beta-cell function and the hypothalamic pathways controlling energy balance and the development of addictive behaviors, inducing mitochondrial impairment, oxidative stress, and the modulation of inflammatory pathways. Seeking to advance the development of prevention strategies to avoid the negative impact of dietary sugars, we outline the role of exercise in reducing blood markers of glucose dysmetabolism and the importance of preventing excessive sugar consumption in critical phases of development owing to its long-term consequences. Additionally, we discuss pathophysiological processes and mechanisms of disease.

Lewis Charles Arlington, (author of In Search of Old Peking) is an American who worked in Beijing at the end of the Qing and the beginning of the Republic of China. In this essay he attempted to introduce the subject of Chinese pulses to a western audience. Strictly judging from the medical content of his work, his recitation of the various Chinese pulses is commendable as a quick summary of the basic information on the subject. Although it is not a comprehensive study of pulses that could be used to guide clinical treatment, it provides a glimpse into the vast and complex subject through western eyes.This version of his article had been skillfully edited to remove Arlington's obvious biases against Chinese medicine and the Chinese culture. By doing so we are left with a text that conveys a basic understanding of pulses that allows the lay person to gain an understanding of this complex field of Traditional Chinese Medicine.Arlington wrote his original text at a time when doctors in the west still believed that syphilis was due to the degeneration of the thyroid gland and not -as we know today- a Treponema pallidum infection. The author's overconfidence in the superiority of his erroneous beliefs over the Chinese theories is a timely reminder that in science there are no absolutes. With Chinese medicine becoming massively popular worldwide and with the advancements of conventional medicine, some of the author's rudest comments are now quite embarrassing and fortunately are removed from this edition. In regards to the 21st century, it's very disconcerting that Arlington's terms such as "quackery" used in the text to describe Chinese medicine could also very well portray many of the early 20th century western practices. It's also very disturbing that the same term still persists in current literature although clinical evidence to the contrary has become abundant. The editing and publication of this text by Mark Linden O'Meara comes at a time when it's important to look back and re-examine the relentless criticism that Chinese medicine had to face even before Nixon's historic trip to China, and also ascertain that modern arguments are often no better than Arlington's imperialist attempts to vilification.

Around 20%-30% of nonmetastatic prostate cancer patients have high-risk localized disease. The best treatment for high-risk localized disease is still unclear, and the reliability of the definition of "high-risk" may need to be validated first. Although recent advances in radiotherapy for prostate cancer have yielded excellent long-term results, even in high-risk localized disease, there are no studies directly comparing it with local treatment options, such as prostatectomy. To address current challenges in high-risk localized and locally advanced prostate cancer, 12 original and 3 review articles covering aspects from basic research to clinical trials are published in this Special Issue.

Research of Pathogenesis and Novel Therapeutics in Arthritis 3.0
Arthritis has a high prevalence globally and includes over 100 types; the most common types are rheumatoid arthritis, osteoarthritis, psoriatic arthritis, and inflammatory arthritis. All types of arthritis share common features, including monocyte infiltration, inflammation, synovial swelling, pannus formation, stiffness in the joints, and articular cartilage destruction. The exact etiology of arthritis remains unclear, and no cure exists as of yet. Anti-inflammatory drugs (NSAIDs and corticosteroids) are commonly used for the treatment of arthritis. However, these drugs are associated with significant side effects, such as gastric bleeding and an increased risk of a heart attack and other cardiovascular problems. Therefore, it is crucial that we continue to research the pathogenesis of arthritis and novel modes of therapy. This reprint summarizes and discusses the themes of 19 articles published in our Special Issue "Research of Pathogenesis and Novel Therapeutics in Arthritis 3.0". This book aims to show the effects of two psychoactive substances: cannabis, with THC as its active ingredient, which acts on the reward circuit, leading to dependence, and lysergic acid diethylamide (LSD), a hallucinogenic chemical that also acts on the reward circuit.

Coronary Syndrome

HPV-Vaccines
Human papillomavirus (HPV) is closely linked with the pathogenesis of cervical cancer and many other cancers. HPV vaccines are vaccines that prevent infection from certain types of human papillomavirus. However, despite the various prevention strategies and treatments, such as HPV screening, prophylactic HPV vaccines, surgery, radiotherapy, and chemotherapy, the disease burden remains heavy worldwide. Currently, three types of prophylactic vaccines-quadrivalent HPV vaccines, bivalent HPV vaccines, and a new nonavalent HPV vaccine-are commercially available. Although these vaccines protect against 90% of HPV infections, their capacity to eliminate pre-existing infections is limited.In this Topical Collection, we aim to systematically cover the progress, current status, and future prospects of various vaccines being developed for the prevention and treatment of HPV-associated lesions and cancers.
Application of Botulinum Neurotoxin in Lower Urinary Tract Dysfunctions
