Traditional Chinese Medicine in the Treatment of Post-COVID-19 Syndrome
The COVID-19 pandemic dominated the globe for at least three years, and infected a large proportion of the worldwide population. After the acute infection, many stay in poor health for months. The nature of this aftermath is not yet fully understood, therefore the management of this syndrome through biomedical therapy is not ideal. Health services are struggling to help those who are still suffering. The condition has now been recognised as post-COVID-19 syndrome (PCS) -- providing a common platform for academic exchange.Traditional Chinese Medicine (TCM) has been applied to similar conditions for over 2,000 years, including in the aftermath of previous pandemics, and this understanding of such conditions has been validated in clinical practice. In TCM, patterns are established to group the weakness, the residue of pathogens or interaction between pathogens and the defence system. Those patterns form the framework for understanding the illness after acute infections. The authors use this ancient understanding in their own contemporary practice, which is particularly rewarding when the illness within PCS is treated with acupuncture, Chinese herbs, and other therapies in tandem. This is the holistic TCM approach strongly recommended by the authors as they demonstrate great outcomes. Respiratory Disease in the COVID-19 Era
Coronavirus disease 2019 (COVID-19) is a new respiratory disease caused by infection with severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) that caused a global pandemic. COVID-19 patients often develop acute respiratory distress syndrome (ARDS), which involves acute and lethal respiratory failure. In a meta-analysis, the median incidence of ARDS in COVID-19 patients was found to be 35 (2-79)%, while the mortality rate in COVID-19-associated ARDS patients was found to be 39 (12.5-73.1)% (Hasan, et al. Expert Rev Repoir Med. 2020; 14: 1149). As of 12 December 2021, 270,373,764 cases had been confirmed as COVID-19 and 5,321,321 cases had resulted in death, according to the World Health Organization webpage. Various symptoms such as persistent fatigue, myalgia, and insomnia can persist after recovery from COVID-19; these symptoms are called "long COVID-19". Patients with COVID-19 who survive but develop pulmonary fibrosis may experience persistent dyspnea upon exertion, a decrease in pulmonary function, and/or a radiological abnormal shadow. Antifibrotic drugs including nintedanib and pirfenidone are candidate drugs for the treatment of pulmonary fibrosis due to COVID-19; randomized controlled trials are ongoing.However, there are limited studies about respiratory disease caused by COVID-19. Given the clinical significance of this topic and its impact on clinical practice and public health, Medicina have published a Special Issue entitled "Respiratory Disease in the COVID-19 Era" with the aim of gathering accurate and up-to-date scientific information of this topic.

Nanoparticles in Ocular Drug Delivery Systems
Conventional ophthalmic formulations lack prolonged drug release effects and mucoadhesive properties, decreasing their residence time in the precorneal area and, therefore, in drug penetration across ocular tissues, presenting a low bioavailability with a consequent reduction in the therapeutic efficacy. These limitations are related to the physiological mechanisms of the eye, for example, the diluting effect of formulations exerted by the tear fluid, constant turnover of the tear fluid, blink reflex, limited capacity of the conjunctival sac, and drainage of formulations applied by gravity.To increase the residence time of formulations on the surface of ocular tissues and increase their ability to penetrate these tissues, different strategies can be used, namely, the use of viscosifying agents, mucoadhesive polymers, stimuli-responsive polymers, microparticles, and colloidal carriers (e.g., micelles, liposomes, nanosuspensions, nanoemulsions, polymeric, and lipid nanoparticles). This Special Issue combines the latest research and review works reporting the use of nanoparticles in ophthalmic formulations to increase their bioavailability and improve the therapeutic efficacy of ophthalmic formulations.

Marine Natural Products as Anticancer Agents 2.0
Global cancer incidence and death are expected to increase in the next decades. Inappropriate therapeutic regimens and tumor recurrence due to the development of drug resistance are two major clinical challenges impacting poor patient outcomes, which is critical to the discovery and development of new anticancer drugs. Over the last few decades, the marine environment has proved to be a key source of molecules with potent anticancer properties. The Special Issue, "Marine Natural Products as Anticancer Agents 2.0", gathered eleven publications, including two reviews and nine research articles, about the anticancer activities of marine natural and/or derived products on in vitro 2D and 3D cellular models, and/or in vivo models of colorectal, breast, lung, myeloma, liver, leukemia, and melanoma cancers. Altogether, the eleven scientific papers published in this Special Issue provide an exciting overview of marine natural products as potential anticancer agents.
Therapeutic Potential for Cannabis and Cannabinoids
Cannabis has a history of medical use dating back millennia; however, the use of cannabis for medicinal purposes fell out of favor in the middle of the last century. There is currently a renewed interest in the potential medicinal value of this plant, as well as individual compounds produced by Cannabis (as well as synthetic analogs). Currently, nearly 50 countries permit the medical use of cannabis, and over a dozen other countries have legalized cannabis-based products (CBD, Sativex, Dronabinol, etc.) for medical use. While there exists a plethora of anecdotal reports indicating that cannabis can treat a variety of diseases, there is a limited amount of strong, evidence-based scientific data to support these claims. This Special Edition of Biomedicines, "Therapeutic Potential for Cannabis and Cannabinoids", was dedicated to providing evidence for the medicinal value of cannabis to treat human illness and disease. Topics included, but were not limited to, preclinical animal models examining the efficacy of cannabis and cannabinoid derivatives on pain, anxiety, cancer, and other diseases; and cell and molecular models of cannabinoid pharmacokinetics, drug-drug interactions, and disease treatment. These studies are of great importance not only for progressing the cannabis field, but also for validating the use of cannabis and cannabinoids as therapeutics.
Pathogenesis and Novel Therapeutics in Asthma
Asthma is a prevalent chronic inflammatory disease of the airways with complex etiology and mechanistic pathways and variable clinical presentation. Such heterogeneity renders the search for targeted therapies difficult. Currently, the best-known disease phenotype with tailored biological treatment is eosinophilic asthma; however, even in some of these patients, anti-IL5 treatment is ineffective. Furthermore, about a quarter of patients, especially those with a severe phenotype, develop progressive and irreversible airway obstruction, leading to persistent symptoms despite optimized treatment. Therefore, new therapeutic approaches are still needed, particularly in severe asthma. The Special Issue presents asthma research focusing on modern personalized therapeutic approaches. Particular interest is directed to novel asthma biomarkers, including molecular and epigenetic ones, shining new light on pathogenesis, phenotyping, and customized therapy in various airway inflammatory patterns of the disease. This book provides an overview of Data Monitoring Committees(DMC) - what was done in the past, what is currently being done, and thoughts on improvements for the future. Previous works focused primarily on large cardiovascular studies (where DMCs originated more than 30 years ago) but updated references are needed that discuss smaller, more flexible studies in areas such as oncology. Abuse of Minors in Clinical Studies
With the emergence of effective drugs and observed drug toxicities in babies, two mantras emerged: that children are therapeutic orphans, and that children are not small adults. US and EU laws demand pediatric studies as a condition for the approval of new drugs in adults. This is called "Pediatric Drug Development" (PDD). Although apparently reasonable, there are catches. Children are vulnerable at birth, but they grow and become bodily mature with puberty, well before coming of age. Minors are not another species. The 18th birthday, an administrative/ legal limit, does not correspond to a physiological change. Drugs treat the body, not the legal status. PDD results in pointless studies in bodily mature adolescents, and in exaggerated studies in younger minors. Minimally Invasive Urological Procedures and Related Technological Developments
We are in the modern era of innovations, technological developments and minimisation in the field of urology. While laparoscopy and robotic surgery have revolutionised pelvic and upper tract oncology, there is continuous evolution and increasing use of these in the field of benign conditions and paediatric urology. As lasers have changed endourology, new minimally invasive surgical techniques (MISTs) have given a new meaning to benign prostate hyperplasia (BPH) surgery. Oncological developments for prostate cancer now include treatments such as robotic prostatectomy, brachytherapy, radiotherapy, cryotherapy and HIFU. Robotic partial nephrectomy and cryotherapy have changed the face of renal cancer. While robotic cystectomy is also gaining popularity for muscle-invasive bladder cancer, new concepts of En-bloc resection of bladder cancer are emerging and being increasingly adopted. MISTs for BPH include laser (Holmium, thulium and green light), water-based treatments (Rezum, Aquablation) and other minimally invasive procedures such as prostate artery embolisation (PAE), TIND and Urolift. In recent decades, numerous metabolites with uncommon structures and efficacious bioactivities have been discovered in mangrove-derived microorganisms, along with mangrove plants. For this reason, mangrove ecosystems have taken the limelight as an attractive biodiversity hotspot, attracting significant attention from organic chemists and pharmacologists. Additionally, many unique and novel chemical structures with a wide range of structural classes have suggested various biosynthetic origins containing novel functional genes and corresponding enzymes with unique catalytic functions.
