Value of Dexamethasone in the Prevention of Nausea and Vomiting
Background: From November 2013 to May 2014, we conducted a randomised, single-blind study on the value of dexamethasone prophylaxis in the occurrence of postoperative nausea and vomiting in ENT surgery at the Gabriel Toure University Hospital in Bamako. This study was based on the analysis of pre-anaesthetic consultations, anaesthesia records, postoperative treatment records and patient questioning.Objectives: The aim was to study the interest of Dexamethasone in the prevention of PONV, through the description of the characteristics of PONV, the determination of the risk of their occurrence, and the analysis of their risk factors in relation to Dexamethasone prophylaxis.Results: A total of 158 patients were enrolled; 43 patients had PONV, i.e. an overall incidence of 27.22%; of which 35.4% (28/79) in the absence of prophylaxis (Group B) versus 19.0% (15/79) with Dexamethasone prophylaxis (Group A).Key words: PONV (postoperative nausea and vomiting), prophylaxis, Dexamethasone, risk of occurrence, incidence.

How Much Is the Cost of Coding Errors?
Casemix system or Diagnosis-Related Groups (DRGs) has been implemented in UKM-Medical Centre, currently known as Hospital Canselor Tuanku Muhriz UKM, since 2002 with the deployment of a locally developed MY-DRG casemix grouper. Coding of diagnosis and procedures using ICD-10 and ICD9-CM are among the major variables required for optimum implementation of casemix system. The impact of coding errors on hospital revenue and budget has rarely been assessed in countries that implement casemix system for provider's reimbursement. This book reports an outcome of the first study done in Malaysia to quantify the economic losses due to coding errors. A blinded re-coding process was conducted to evaluate the quality of clinical coding of randomly selected patient medical records from four major specialities in the hospital: Medical, Surgical, Paediatrics and Obstetrics & Gynaecology. The rates of overall coding errors were identified, and the different types of coding errors were analysed and reported in detail. The amount of losses in hospital revenue due to coding errors were estimated in the study. Factors that led to the coding errors of diagnoses and procedures were analysed and presented in this book. It is hope that results of this unique research reported in this book would encourage leaders in hospital services to pay serious attention on the problems and embark on intensive and continues training of coders and other clinical staff to effectively reduce the coding errors in the implementation of casemix system.

Methotrexate and psoriasis
Psoriasis is a chronic inflammatory disease with a major impact on the quality of life of patients. Its management remains a real challenge. The aim of our study is to evaluate the place of methotrexate in the treatment of moderate to severe psoriasis in our Moroccan context. We obtained good results with a complete remission in 50% of the cases, with few side effects thanks to a regular follow-up of our patients, as well as their supplementation in folic acid. Biotherapies are as effective as methotrexate with fewer side effects, however their cost remains high and they are not reimbursed by social security. In our context, methotrexate remains the first-line treatment for moderate to severe psoriasis, it is an effective, low-cost and well-tolerated treatment.

Since its first description in 1916, the spectrum of Guillain-Barr矇 syndrome (GBS) has broadened and it is currently used to refer to a group of acute autoimmune neuropathies with different clinical phenotypes. In the ensuing decades, US foreign assistance shifted away from massive modernization projects, such as water treatment facilities, toward point-of-use technologies like village water pumps and oral rehydration salts. This transition toward the small scale had massive implications for the practice of global health. Developing to Scale tells the history of appropriate technology in international health and development, relating the people, organizations, and events that shaped this consequential idea.
