Obturation Techniques

Obturation of the root canal system is an integral component in promoting periapical healing and preventing disease progression. The root canal filling material accomplishes this by reducing microleakage and entombing any inflammatory irritants. The effectiveness of a material to adequately seal the root canal space is established by its physical properties and handling characteristics. Over the past century, numerous obturation materials and delivery techniques have been introduced in an attempt to obtain a microbiologic barrier within the confines of the root canal system. Different materials have their limitations and advantages. Thus, numerous techniques have been proposed, experimented with and implemented to achieve the densest obturations. A number of different methods were developed for filling root canals with warm gutta-percha where movement of the material into the lateral canals and apical deltas was possible. These include warm lateral condensation, warm vertical condensation, thermatic compaction. Thus, the aim of this dissertation is to critically review of various methods used for the obturation of root canal systems in endodontics.

Inflammation involves two basic processes: early inflammatory response followed by healing/resolution. Acute inflammation is temporally and spatially controlled to maintain homeostasis. Unresolved acute inflammation is detrimental to tissue function, promoting dysbiosis which leads to chronic inflammatory diseases including periodontitis. Periodontitis is a multifactorial disease of inflammatory origin affecting the teeth' supporting tissues, resulting in progressive destruction of the periodontal apparatus leading to teeth loss. Tissue destruction is mainly due to the host-derived inflammatory mediators (Pro-inflammatory cytokines, Matrix Metalloproteinases). Inflammation must be resolved to avoid progression to chronic inflammation and promote repair or regeneration. Resolution of inflammation is an active biomechanical process regulated by specialized pro lipid mediators (SPMs) like Lipoxins, Resolvins, Protectins, and Maresins which will help to return homeostasis.

Orthodontists’ Knowledge of Temporomandibular Dysfunction

This study arose from consultations with several patients with Temporomandibular Dysfunction (TMD) and Orofacial Pain (OOP). Many colleagues have the same doubt due to concepts which state that TMD is related to the teeth and not the muscles (80% of TMD). We therefore decided to measure the knowledge of orthodontists in the state of Amazonas about TMD and OOF, in order to compare it with research carried out elsewhere in Brazil and the world. The sample consisted of 98 specialists out of a total of 137 registered with the CRO-AM, assessed using a questionnaire developed at the University of Washington (USA), Korea and Brazil. The translated questionnaire consisted of 35 statements from four areas related to TMD: pathophysiology; psychophysiology; chronic pain; and psychiatric disorders. After collection and tabulation, the data was compared with the opinions of TMD experts on these items. The results were quite alarming and are quoted and discussed in this book. After the research was completed, the author set to work trying to disseminate his truth about TMD, a very preventive and habit-changing truth without extensive, invasive and irreversible procedures.

Fluoride plays a significant role in dental and skeletal health. It has been widely studied and debated. While fluoride at appropriate concentrations is beneficial, excessive fluoride intake, especially in children, can lead to severe health issues such as dental fluorosis, skeletal fluorosis, and non-skeletal fluorosis. Long-term overexposure to fluoride, particularly in endemic areas, has been associated with a range of adverse health effects, including neurotoxicity, which affects the development of intelligence in children. IQ is a measure of intelligence which is derived from standardized tests. Multiple studies conducted across the world have consistently demonstrated an inverse relationship between fluoride exposure and IQ levels in children. While evidence suggests an association between fluoride exposure and lower IQ, these findings are inconclusive and not generalizable. IQ is influenced by multiple factors, which have not been adequately assessed in these studies. To establish a causal relationship, we need longitudinal studies that control for the these confounding factors to better understand and mitigate fluoride's potential impact on intelligence.

Clinically, biomimetics refers to mimicking the physiognomies of a natural tooth repair of affected dentition through biomimetic procedures and materials. For example, to improve the osseointegration of dental implants, biomimetic-dental-implant coatings of calcium phosphate (Ca P) and hydroxyapatite (HA) have been investigated and implemented. Similarly, the biomimetic process is applied in adhesive-restorative materials that demonstrated aesthetics mimicking natural teeth and tooth morphology. During the last decades, the restorative approach has steadily evolved, progressing from mechanical retention to advanced adhesion. Composite-resin materials and adhesive dentistry have become valuable tools on this context. The principles of biomimetic dentistry impose introduction of advanced composite restorative materials to clinical practice, which should align with the nature and integrity of the tooth tissues.

Surgical endodontics is often a last resort when nonsurgical retreatment is impractical or unlikely to improve on a previous result. In particular, only surgical intervention might resolve cases involving a persistent lesion that is related to a periapical cyst or complex canal anatomy. In recent years, modern techniques that include the use of magnification tools, micro-instruments, ultrasonic instruments, and more biocompatible filling materials have been introduced. Current EMS techniques entail the use of an operating microscope to better identify canals, fractures, and isthmuses angled ultrasonic root-end preparation instruments facilitating accurate root-end preparation along the long axis of the root canal without blocking visibility; and new root-end filling materials, such as mineral trioxide aggregate (MTA), that have improved leakage resistance, biocompatibility, antimicrobial properties, and moisture tolerance. The ultrasonic technique healing success of 95% was compared with 65% success with the traditional techniques.

The clinical success of bonded restorations depends primarily on a good bond to tooth/dentin and an optimal marginal fit after cementation. The resin luting cements are the latest type of luting agents for the cementation of indirect aesthetic restorations such as all-ceramic and porcelain veneers. One of the leading causes of dental restoration failures is volumetric shrinkage of resin-based dental products as a result of polymerization. The composite mass has a significant impact on polymerization shrinkage. Luting with resin cements can result in gaps at the margins of adhesive restorations. Microleakage caused by volumetric shrinkage can also be caused by these gap formations. Marginal staining, post-operative sensitivity, and chronic caries may all be caused by uncontrolled shrinkage. The amount of filler in resin-related materials is thought to have a significant impact on their shrinkage-strain kinetics. To ensure the durability and success of indirect restorations, the composition of the resin cements should also be assessed. Nanoparticles have demonstrated their potential to enhance the mechanical properties of polymeric materials in numerous studies.

Lesion Sterilization and Tissue Repair (Lstr) in Primary Molars

LSTR is an endodontic treatment approach that involves non-instrumentation or minimum instrumentation followed by the administration of an antibiotic mixture in a vehicle to disinfect root canal systems and peri-apical lesions. LSTR is based on the notion of repair by the host's natural defence mechanisms. Sterilising the canals and pulp chamber using medicaments helps lessen the bacterial burden. This technique is less invasive, less time consuming, and come with primary aim of elimination of all the microorganism i.e, sterilization of root canal spaces which results in good healing. If the procedure is successful tissue repair can be expected. It needs the application of a different antibiotic combinations known as "Triple Antibiotic Paste" (TAP). LSTR therapy could be a viable treatment modality for infected/nonvital primary molars with poor prognosis, in cases of internal and external resorption and intended to be maintained for shorter duration in the oral cavity as natural space maintainers.
