Intraocular Pressure and Ocular Hypertension

Primary open-angle glaucoma (POAG) is a multi-factorial progressive optic neuropathy characterized by retinal ganglion cell degeneration and progressive visual field loss which, if left untreated, may lead to blindness. Increased intraocular pressure (IOP) is considered to be the main risk factor for developing POAG, and its reduction has been shown to correlate with a decrease in glaucoma incidence and progression. Considering that fewer than 10% of the subjects with ocular hypertension (OHT) will develop morphological and/or functional glaucomatous damage within 5 years if not treated, glaucoma causes and molecular changes leading to ocular tissue damage in glaucoma are still largely unknown. The contemporary treatment of POAG is mainly oriented towards reducing IOP; the importance of the IOP reduction in other types of glaucoma, such as the "normal pressure glaucoma", is still discussed. The IOP value is maintained by balancing the amount of fluid contained within the anterior and posterior chambers of the eye; our comprehension of the mechanisms underlying the secretion and active and passive outflow of the aqueous humor is extremely important for improving the treatment of glaucoma. Innovative pharmacological approaches, and laser and surgical procedures aiming to reduce IOP, have been developed in recent years. This book provides a compendium of topics regarding IOP, aqueous humor dynamics, tonometry, and medical and surgical techniques developed to reduce the IOP in subjects with ocular hypertension or glaucoma.

This book introduces "network pharmacology" as an emerging frontier subject of systematic drug research in the era of artificial intelligence and big data. Network Pharmacology is an original subject of fusion system biology, bioinformatics, network science and other related disciplines. It emphasizes on starting from the overall perspective of the system level and biological networks, the analysis of the laws of molecular association between drugs and their treatment objects, reveals the systematic pharmacological mechanisms of drugs, and guides the research and development of new drugs and clinical diagnosis and treatment. Polypharmacy is a necessary and important aspect of drug treatment; however, it becomes a challenge when the medication risks outweigh the benefits for an individual patient. Drug-drug interactions and the introduction of prescribing cascades are common features of polypharmacy, which can lead to ineffectiveness and increased risk of adverse drug reactions (ADR). Genes encoding CYP450 isozymes and other drug-related biomarkers have attracted considerable attention as targets for pharmacogenetic (PGx) testing due to their impact on drug metabolism and response. This Special Issue is devoted to explore the status and initiatives taken to circumvent ineffectiveness and to improve medication safety for polypharmacy patients. Specific areas include drug-drug interactions and consequences thereof in therapeutic management, including PK- and PD-profiling; the application of PGx-based guidance and/or decision tools for drug-gene and drug-drug gene interactions; medication reviews; development and application of deprescribing tools; and drivers and barriers to overcome for successful implementation in the healthcare system.

Frontiers in Antimicrobial Biomaterials

Biomaterials can be used as implantable devices or drug delivery platforms, and have significant impacts on a patient's quality of life. Indeed, every year, a substantial number of new biomaterials and scaffolding systems are engineered and introduced in the biomedical field, with increased health benefits. However, their long-term use can be threatened by the adhesion and proliferation of microorganisms or by the formation of fibrosis. Pathogenic microorganisms may cause local infections and lead to implant failures. Additionally, they can hinder the delivery of therapeutic molecules by specialized carriers, rendering them ineffective. Many alternatives have been proposed over the years to prevent such events, including the use of antiseptics and antibiotics, or physical modifications of the biomaterial surface with the incorporation of biomolecules. From specialized polymers and functional groups to silver and, more recently, antimicrobial peptides and natural extracts, different functionalization and modification techniques have been employed in this fight against pathogenic agents. This reprint aims to further our understanding of the antimicrobial actions of specialized biomaterials and introduce new surface modification strategies, original polymeric chemical structures, and new antimicrobial agent-material combinations, from which infection control or microbial eradication can be achieved.

Brain diseases currently affect one in six people worldwide; they include a wide range of neurological diseases, from Alzheimer's and Parkinson's diseases to epilepsy, brain injuries, brain cancer, neuroinfections, and strokes. Updates in Management of SARS-CoV-2 Infection

Severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) has spread worldwide from the beginning of 2020. The infection is mostly asymptomatic but some patients may develop COVID-19 (coronavirus disease 2019) with a severe or critical course leading to pneumonia, acute respiratory distress syndrome, and multiorgan failure. Apart from the virus-related damage of the lungs, pathomechanism of the disease seems to be linked to thromboembolism and inflammation accompanied by overproduction of proinflammatory cytokines, termed a cytokine storm, responsible for multiorgan damage and death. Since the development of a new therapeutic molecule, dedicated strictly to a particular virus is time-consuming, physicians and scientists have started to test and repurpose old medications. Unfortunately, after one year of pandemics, there is still a lack of optimal therapy and no clear indicators of recovery. A major issue is also insufficient knowledge on predictors of the severe or deadly course of the disease, which could also help to switch from one therapeutic option to another. Due to many gaps still existing in the management of COVID-19, there is a need for the accumulation of new data particularly from real-world experience, which could be applicable to practice guidelines. The objective of this special issue of the Journal of Clinical Medicine is to provide an update on the mangement for the diagnostic workup and pharmacotherapy of SARS-CoV-2 infection.
