Vasoplegic Endothelial Dysfunction

This unique work presents one of the most significant organ failure condition - cardiocirculatory shock - and the prominent role of Methylene Blue (MB). Through their extensive experience, the authors' consider that the lifesaving feature of MB is still underestimated. For this reason they outline key concepts of MB, mentioning essential experimental data, yet focusing mainly on its application in the care of critically ill patients. In an easy-to-read format, the 18 chapters were organized in a didactically structured order, distributed in four parts. The first part introduces the general conceptual aspects and physiology of the endothelium. The second part presents the particularities of endothelial dysfunction in different types of shock. Certain rare connective tissue disorders can also affect the bones, such as Marfan syndrome, osteogenesis imperfecta, and osteochondrodysplasias. Many specialists are involved in the management of bone development disorders in children and adolescents, such as neurosurgeons, plastic surgeons, general surgeons, ORL surgeons, maxillofacial surgeons, orthopaedics, radiologists, and pediatric intensive care physicians. Role of MicroRNA in Cancer Development and Treatment

Many researchers around the world have demonstrated that the expression of miRNAs is dysregulated in different tumors. Such dysregulation is caused by multiple mechanisms, and exposure to different carcinogens causes dysregulated epigenetic changes and defects in the miRNA biogenesis machinery. Cancer cells with abnormal miRNA expression evolve the capability to sustain proliferative signaling, evade growth suppressors, resist cell death, activate invasion and metastasis, and induce angiogenesis. Genome-wide profiling demonstrates that miRNA expression signatures are associated with tumor type, tumor grade and clinical outcomes, so miRNAs could be potential candidates for diagnostic biomarkers, prognostic biomarkers, therapeutic targets and preventive screening programs. Although miRNAs have multiple targets, their function in tumorigenesis is due to their regulation of a few specific targets. After the first detection of altered miRNA in leukemia, microRNAs have been demonstrated to be constantly altered in all cancer. More recently, microRNA has been shown to be altered by exposure to environmental carcinogens, thus driving the whole process of carcinogenesis. Our aim is to provide a rigorous peer review and publish cutting-edge research on the role of microRNA in cancer prevention therapy to educate and inspire the scientific community worldwide.

Albumin-Based Drug Delivery Systems

Albumin is playing an increasing role as a versatile, biodegradable drug carrier in clinical theranostics. By applying different techniques, smart drug-delivery systems can be developed from albumin in order to improve drug delivery of different active pharmaceutical ingredients, even small-molecule drugs, peptides or enzymes. Principally, three drug delivery technologies can be distinguished for binding small-molecule or peptide drugs through the charged amino acids, carboxyl, and amino groups of albumin: physical or covalent binding of the drug to albumin through a ligand- or protein-binding group, the fusion of drug with albumin or the encapsulation of drugs into albumin nanoparticles. The accumulation of albumin in inflamed tissues and solid tumours forms the rationale for developing albumin-based drug delivery systems for targeted drug delivery. Besides tumour therapy, albumin-based drug delivery systems can be successfully applied as anti-inflammatory and anti-thrombotic coating for medical devices. The development and optimization of albumin nanoparticles may also be a rational and promising tool for conventional or alternative administration routes in order to improve therapy. This collection provides an overview of the significant scientific research works in this field, which may inspire researchers towards further development and utilization of these smart drug delivery systems.

Regional Intestinal Drug Absorption

The gastrointestinal tract (GIT) can be broadly divided into several regions: the stomach, the small intestine (which is subdivided to duodenum, jejunum, and ileum), and the colon. The conditions and environment in each of these segments, and even within the segment, are dependent on many factors, e.g., the surrounding pH, fluid composition, transporters expression, metabolic enzymes activity, tight junction resistance, different morphology along the GIT, variable intestinal mucosal cell differentiation, changes in drug concentration (in cases of carrier-mediated transport), thickness and types of mucus, and resident microflora. Each of these variables, alone or in combination with others, can fundamentally alter the solubility/dissolution, the intestinal permeability, and the overall absorption of various drugs. This is the underlying mechanistic basis of regional-dependent intestinal drug absorption, which has led to many attempts to deliver drugs to specific regions throughout the GIT, aiming to optimize drug absorption, bioavailability, pharmacokinetics, and/or pharmacodynamics. Disruptive behavior disorders (DBD) refer to a group of conditions that typically share difficulties in modulating aggressive conducts, self-control, and impulses, with resulting behaviors that constitute a threat to others' safety and to social norms. Problematic issues with self-control associated with these disorders are commonly first observed in childhood, but may often persist into adolescence and adulthood, or pose a developmental risk for subsequent negative outcomes. Extension of Healthy Life Span of Dialysis Patients in the Era of a 100-Year Life

Malnutrition is becoming a more prominent health problem, with an increasing number of elderly CKD patients being put on dialysis. In addition, the presence of inflammation, sarcopenia/frailty, diabetes, and CVD is a definite and independent risk factor associated with higher mortality in this population. Although the restriction of protein intake has been recommended to protect eGFR decline, hyperphosphatemia, and hyperkalemia in CKD patients, it might accelerate the loss of skeletal muscle and adipose mass, leading to a poor prognosis. Therefore, flexible responses are considered regarding whether protein restriction should be continued or loosened in pre-dialysis CKD patients. In undernourished elderly patients undergoing hemodialysis, sufficient calorie/protein intake is necessary to counteract the development of sarcopenia/frailty. It is expected that the application of new drugs including phosphate binders and potassium chelators may achieve both a high enough intake and balanced levels of phosphate and potassium. Furthermore, the improvement of deficient micronutrients and poor appetite is also necessary. Comprehensive care is essential for the wellbeing of elderly patients undergoing hemodialysis. The topicof this Special Issue is "Extension of Healthy Life Span of Dialysis Patients in the Era of a 100-Year Life".
