Emergency Laparoscopic Surgery in the Elderly and Frail Patient

The global population is aging rapidly, and as a result emergency abdominal surgery for acute abdomen in the elderly has become a global issue. Demographic changes in the population have also altered the profile of emergency abdominal surgery, with typical causes of acute abdomen in the elderly including acute cholecystitis, incarcerated hernia, bowel obstruction and appendicitis. In these patients, recovery from surgery is often complicated, leading to longer hospital stays compared with younger patients. Laparoscopy in the emergency setting is well established and offers a number of advantages over open surgical approaches, such as reduced postoperative pain, length of hospital stay and complication rates. While laparoscopic surgery for acute diverticulitis has become more widespread, in other conditions, like small bowel obstruction and perforated peptic ulcer, laparotomy is still used in about 70% of cases. Further, despite the urgent need for knowledge regarding surgical treatmentof acute abdomen in the elderly, there is still a lack of evidence in this relevant clinical field.This book analyzes the management strategies and critically evaluates the outcomes of laparoscopic emergency surgery for acute abdomen in the elderly according to the principles of evidence-based medicine. Novel Methods of Diagnostics of Thyroid and Parathyroid Lesions

Thyroid nodular disease is one of the most frequent endocrine diseases. The prevalence of thyroid focal lesions detected by imaging techniques, according to studies on different populations, ranges from 10 to 70%. In a population of women over 50 years of age, approximately half of them will have a thyroid focal lesion. However, only 18% of thyroid nodules are diagnosed as malignant. Thyroid nodular disease is the most frequently diagnosed endocrine pathology, while thyroid cancer constitues the most common endocrine malignancy and is reponsible for about 67% of deaths due to neoplasms derived from endocrine organs. The incidence of thyroid cancer has risen by about 240% in the last three decades. Due to the increased availability of imaging techniques, recently, a rise in the detectability of thyroid cancer at the stage of microcarcinoma has been observed. Diagnostic and therapeutic decisions in patients with thyroid nodules require an interdisciplinary consensus between endocrinologists and physicians of other specialities (radiologists, pathologists, surgeons, oncologists). This book focuses on current trends in novel techniques of thyroid nodule diagnostics before they are implemented in the current guidelines on the management of thyroid nodular disease.

Medical Nutrition Therapy in Critically Ill and COVID-19 Patients

The recent COVID-19 pandemic, which resulted from SARS CoV-2 coronavirus infection, contributed to a rapid increase in hospital and intensive care unit (ICU) admissions. Although during the last 3 years there have been numerous research publications on patient care, data concerning the role of the dietary approach in the overall treatment of the disease are minimal. Moreover, with regard to the dietary approach during COVID-19 critical illness, practice guidelines are still based on data which were developed too quickly and were based on targeted recommendations on feeding the critically ill. Since then, new sources of data have emerged, which clearly display significant nutritional challenges. Why are we so interested in providing individualized nutritional therapy to critically ill patients with COVID-19? Bevause these patients tend to exhibit a significantly greater length of hospital stay, and a higher risk of developing muscle weakness, malnutrition and functional loss. Are there any data on best nutrition support practices? Unfortunately, there are no data from randomized clinical trials, with the exception of some micronutrient supplementation studies with immunomodulating actions. So, the key concept identified in this Special Issue was that optimizing dietary practices for patients both during their ICU stay and beyond is crucial. Clinicians should be capable of managing their patients both during their hospitalization and rehabilitation phase, in order to ensure continuous care and to minimize the susceptibility of adverse events due to malnutrition.

These controls, surprisingly enough, were not wielded by any state or federal agency, although almost every other aspect of American life is now under the absolute control of the bureaucracy. The physicians have their own autocracy, a private trade association, the American Medical Association. This group, which is headquartered in Chicago, Illinois, had gradually built up its power until it assumed total control over medical schools and the accreditation of physicians. The trail of these manipulators led me straight to the same lairs of the international conspirators whom I had exposed in previous books. I knew that they had already looted America, reduced its military power to a dangerously low level, and imposed bureaucratic controls on every American. I now discovered that their conspiracies also directly affected the health of every American. This conspiracy has resulted in a documented decline in the health of our citizens. We now rank far down the list of civilized nations in infant mortality and other significant medical statistics. I was able to document the shocking record of these cold-blooded tycoons who not only plan and carry out famines, economic depressions, revolutions and wars, but who also find their greatest profits in their manipulations of our medical care. The cynicism and malice of these conspirators is something beyond the imagination of most Americans. They deliberately mulct our people of millions of dollars each year through ''charitable'' organizations and then use these same organizations as key groups to bolster their Medical Monopoly. Fear and intimidation are the basic techniques by which the conspirators maintain their control over all aspects of our health care, as they ruthlessly crush any competitor who challenges their profits. As in other aspects of their ''behavioural control'' over the American people, their most constantly used weapon against us is their employment of federal agents and federal agencies to carry out their intrigues. The proof of this operation may be the most disturbing revelation of my work.

Nephropathic cystinosis (MIM # 219800) is a rare autosomal recessive disorder caused by mutations in the lysosomal cystine transporter cystinosin, encoded by the CTNS gene (17p13.2). This devastating condition initially affects kidneys and subsequently many other organs including eyes, thyroid, pancreas, muscles, and brain. While lysosomal cystine storage is a key feature of the disease and the main target of current therapy, recent groundbreaking research has revealed that cystinosin has diverse functions in cells, being involved in vesicle trafficking, energy homeostasis, and cell death mechanisms. These discoveries deepen our insights into the mechanisms of cystinosis and of lysosomal biology in general.
