The observations in the notes and the independent, dependent and interdependent actions, allowed validating the efficacy of the interventions and the satisfaction of patients and service providers. The procedure will achieve the implementation of a nursing care protocol and the design of instruments for its evaluation in order to achieve the termination of gestation, thus obtaining a live, healthy and uncomplicated newborn.

"Advanced Breast Cancer Detection Using Machine Learning and Segmentat
Breast cancer is the second leading cause of death among women, often undetected until it reaches advanced stages. Early identification is crucial, as accurate classification of benign and malignant tumors can prevent unnecessary treatments. This study explores the application of machine learning techniques for breast cancer diagnosis using the Wisconsin Breast Cancer Dataset from the UCI Repository.Initial experiments with the Na簿ve Bayes classifier yielded 88% accuracy for benign and 86% for malignant tumors. However, it faced limitations, such as low accuracy and issues with zero frequency probabilities. Switching to Artificial Neural Networks (ANN) improved results to 90% for benign and 92% for malignant classifications, but still did not yield optimal outcomes.The research ultimately employed Support Vector Machine (SVM) techniques, achieving the highest accuracy at 97% for benign and 95% for malignant tumors. This method effectively distinguishes between tumor types using a linear model based on hyperplanes. All algorithms were implemented using the R tool, which is user-friendly and free, facilitating data handling for breast cancer classification.

Smoking epidemic among young Tunisians

Tuberculin skin test
Introduction: Ganglionic forms of tuberculosis often escape diagnosis by conventional tests. Our aim was to evaluate the performance of the tuberculin skin test in the diagnosis of active tuberculosis in Tunisia, using ROC curve methodology. Methodology: This was a multicenter case-control study that was conducted in 11 tuberculosis clinics(DAT) in Tunisia in 2014. Results: In total, we included 1053 patients divided into 339 cases and 714 controls. The mean diameter of the tuberculin TST induration was significantly higher in cases than in controls(13.7mm vs.6.2mm;p=10-6).The area under the curve(AUC )was 0.789[95% CI:0.758-0.819;p=0.01], corresponding to a moderate discriminating power for this test. The most discriminative threshold value for the diameter of the induration of the intradermal tuberculin reaction, associated with the best sensitivity (73.7%) and specificity (76.6%), was 11 mm, with a Youden index of 0.503.Conclusion: We can conclude that the tuberculin skin test can be used to diagnose tuberculosis with good sensitivity and specificity. However, its interpretation and reading remain difficult and highly subjective.
Healthcare-associated infections
Introduction: Healthcare-associated infections (HAIs) are a major public health concern worldwide, and our aim was to provide an update on HAIs and to develop an example of a protocol for an incidence survey and an approach to extra costs in hospitals. Methods: A bibliographical review of the literature was carried out in order to provide a theoretical review and to draw up a protocol. Results: The protocol was a cohort study with prospective data collection. To estimate incidence, we used 3 epidemiological indicators: cumulative incidence, incidence density and the medical device exposure ratio. To assess the cost of HCAIs, we were interested in the direct medical cost, which was presented in terms of expenditure linked to the length of hospital stay (comparing infected cases with uninfected controls), as well as expenditure linked to the consumption of antibiotics necessitated by these infections.Conclusion: A method for regularly monitoring the incidence and cost of these infections needs to be introduced in every hospital establishment for effective control.
Towards the implementation of telemedicine in Tunisia
Introduction: The implementation of telemedicine in Tunisia has met with little success, and very few studies have focused on the subject.Our study aims to assess the knowledge, attitudes and practice of Tunisian doctors towards telemedicine.Methods: This was a descriptive cross-sectional online study of knowledge, attitudes and practices carried out during October2022.Results: A total of 243 doctors were included.The majority worked in the public sector and were medical specialists.More than half the doctors (59.3%)had a low level of knowledge of telemedicine.Concerning the evaluation of attitudes, the majority of respondents(89.3%)had a moderate or high score of perceived benefits.More than three quarters of respondents had a moderate or high score of perceived compatibility of telemedicine with their practice.The majority(93%) had a moderate or high attitude regarding willingness to try telemedicine practice.Conclusion: The positive attitude of Tunisian doctors towards telemedicine and their willingness to try it in their future practice are encouraging for its implementation.
Efficacy ́ of trichiasis surgery and degree ́ of satisfaction.
According to the World Health Organization, trachomatous trichiasis (TT) is the leading preventable cause of infectious blindness worldwide. It is the final stage in the evolution of trachoma before blindness. Badly operated, trichiasis can recur and continue its evolution towards irreversible blindness, increasing the precariousness of families. Our study aimed to assess the quality of TT surgery and patient satisfaction in the Kayes region of Mali. This was a cross-sectional study conducted from June 22 to July 1ᵉʳ, 2018, among 125 people randomly selected from the 492 people operated on for trichiasis in 2017 in the Kita and Diema health districts, one year after surgery. An experienced ophthalmologist examined the eyes using a 2.5-diopter magnification magnifier and a lamp, to detect possible recurrences and complications recorded in a standard questionnaire. Eye care was provided where necessary, with the participant's informed consent.

Ischemic stroke in newborns is the result of an interruption in blood flow to one of the main cerebral arteries due to embolism or thrombosis. Infantile strokes are 10 to 12 times rarer than in adults, with an estimated incidence of 3.3 per 100,000 births. Perinatal stroke is the most frequent form, and is the leading cause of cerebral palsy and the second most common cause of neonatal convulsions after anaxo-ischemic encephalopathy. Its clinical presentation is not very specific. The diagnosis is most often made on the basis of a brain imaging scan ordered in response to neurological warning signs, particularly neonatal convulsions. To this day, the pathophysiological mechanism of neonatal stroke remains debated. Multiple maternal and fetal risk factors have been identified. However, it is important to note that these foods are not a substitute for medication. Medical responsibility and the influence of medical delegates
The aim of this work is to study medical responsibility in the face of the influence of medical representatives in medical prescribing in hospitals.Method: This was a cross-sectional, descriptive and prospective study carried out in the public hospitals of Ouagadougou from January 08 to January 31, 2024. Physicians were asked to complete a self-administered questionnaire.Results: Our study involved 213 physicians in the city of Ouagadougou, with an average prescriber age of 34.7 years and a male predominance of 58.7%. Specialized physicians were the most represented: 51.2%. Doctors with less than 5 years' professional experience accounted for 50%. 95.8% of prescribers said they had received gifts, mostly drug samples 91.8% medical equipment 87.1% catering 39.7% conference funding 39.2% .... The Internet was the most widely used source of information (44.1%) for prescribers to verify the information provided by the medical representative. Antibiotics were the most represented drug class at 62.3%, with high specialty prescribing at 66%.
59 cases of occupational asthma
Occupational asthma (OA) has become the most common work-related respiratory disease.We carried out a retrospective descriptive study to investigate the prevalence of OA in southern Tunisia from 2002 to 2009, to support its diagnostic strategy, to describe the epidemiological features of affected subjects, and to identify the preventive and medico-legal etiological factors of the disease. Fifty-nine cases of PA were collected in our study, with a mean age of 42+/- 9 years. Most of the patients were manual workers. In the context of our study, occupational disease declarations were essentially made on the basis of 3 tables: table N簞42 relating to isocyanates; table N簞56 relating to cereals and flour; table N簞58 corresponding to other agents responsible for allergic respiratory affections. Of the 59 cases reported, 38 were recognized as occupational diseases. A permanent partial disability rate was assigned to each recognized case (average permanent partial disability rate was 24.07%).

Impact of smoking on lung disease COVID-19
: The 2019 coronavirus pandemic (COVID-19) caused by Severe Acute Respiratory Syndrome-Coronavirus 2 was responsible for high morbidity and mortality. Several prognostic factors have been identified, but the impact of smoking on this condition remains a matter of controversy.Our study concerned patients who were hospitalized with COVID-19 pneumonitis between 2020 and March 2021. The aim of our study was to evaluate the impact of smoking on the radio-clinical presentation of this condition and on its evolutionary course. Results 237 patients with a mean age of 62.7 years were included. The study population was divided into two groups: G1, a group of non-smoking patients (184 patients), and G2, a group of smoking patients (53 patients). G1 was predominantly female, while G2 was predominantly male. The two groups were comparable in terms of age, presence of diabetes, arterial hypertension, heart failure, arrhythmia and coronary insufficiency, but obesity was more frequent in the former group.

Objective: To estimate the prevalence of hyponatremia degrees in patients with liver cirrhosis admitted to the Gastroenterology area of the Public Hospital in the period from January 2021 to June 2022. Methodology: Observational, retrospective, analytical and descriptive study; a sample of 111 patients whose information was obtained from medical records was analyzed. Results: The predominance of female sex was identified (50.5%), as well as age between 51 and 70 years (64.9%) and 72.1% had a hospital stay of 1 to 15 days. Mild hyponatremia had a prevalence of 23.7%, moderate hyponatremia a rate of 10.1% and severe hyponatremia a rate of 1.3%. Regarding comorbidities, cancer was the only one that was related to the degrees of hyponatremia. On the other hand, ADH was the only complication. We conclude that the prevalence of hyponatremia in these patients was 35.1%.

This paper was inspired by the concerns of physicians and lawyers themselves, the other party investigated by us, since we are convinced that knowledge is the best weapon for the prevention of lawsuits against the medical corps and health teams. Many answers were directly based on the guidelines of Law 17132, the new Civil Code and other legislation together with the Code of Ethics of the Argentine Medical Association in its third edition, the Code of the Argentine Medical Federation and our own contributions. The important thing is that by knowing the legal norms in advance the professional will know when his behavior is licit or illicit, or can be questioned. The idea is that knowledge reduces the professional's daily stress and will lead him to the path of greater tranquility in his daily activity. In short, we do not want to prove anything but simply to show and promote reflection.
The compliance to and practice of social distancing amongst adults

Bronchial Dilatation
Bronchial dilatations (BDDs) result from a recurrent inflammatory process in the airways, causing irreversible changes and recurrent respiratory infections. Our retrospective study analyzed 100 patients diagnosed with DDBs between 2010 and 2020. The population was predominantly female (mean age 57.79), with a prevalence of a history of tuberculosis (40%) and recurrent respiratory infections (21%). The main symptoms were productive cough (80%) and dyspnoea (61%). Chest CT confirmed the diagnosis, revealing predominantly cylindrical and diffuse lesions. Antibiotic therapy, accompanied by respiratory physiotherapy, was the treatment of choice. Predictive factors for exacerbations included a history of tuberculosis, dyspnea, hypoxemia and the presence of Pseudomonas aeruginosa. Despite therapeutic advances, DDB remains a major challenge, with a significant impact on patients' quality of life.

Portfolio for medical interns
Recent medical education reforms in Tunisia have highlighted the importance of clinical skills and a learner-centered approach. In this context, the portfolio is a useful tool to support interns in their training.The aim of this project was to create a portfolio guide for interns at the Sousse Faculty of Medicine in hepato-gastroenterology (HGE). To achieve this, a literature review identified best practices, and an analysis of the specific needs of interns was carried out.The result is a portfolio guide that clearly defines its objectives and content. It includes an identification sheet, learning activities such as clinical situation narratives and an internship diary, as well as space for personal reflection. Assessment criteria have also been developed to measure the clinical and interpersonal skills of interns.In conclusion, this guide meets a specific need in the medical training of HGE interns. A pilot phase will be set up to adjust the guide according to feedback from interns and tutors.
Educational program on cerebrovascular diseases
Cerebrovascular disease, cerebrovascular accident or stroke, is a complex age-related disease with high mortality and long-term disability. In Cuba, it is the third leading cause of death. From January to December 2023, a quasi-experimental intervention study was conducted to assess the impact of an educational program in patients with risk factors for cerebrovascular disease. The main risk factor found was hypertension with 30 patients, followed by diabetes mellitus, for 85.7% and 51.4% respectively. In the study 27 patients presented inadequate knowledge about the types of stroke and after applying the Educational Program the figures decreased where only 6 patients reflected inadequate knowledge. After applying the program, 88.6% presented adequate knowledge about risk factors. A change in the lifestyle of the patients was observed, at the end of the intervention 45.7% and 71.4% presented a better control of Diabetes Mellitus and Arterial Hypertension respectively.
Oral mucosal lesions
Aging is a universal phenomenon that involves changes in the organism including the oral cavity of the elderly. In order to characterize older adults with oral mucosal lesions associated with the use of removable prostheses, an observational, descriptive, cross-sectional study was carried out. Predominantly older adults between 60 and 69 years of age were female. The most frequent lesions were subprosthetic stomatitis and fissured epulis. The main risk factors were smoking, acrylic upper total prosthesis, time of use from 6 to 11 years, frequency of continuous use, inadequate hygiene and poor condition of the appliance. A greater relationship was found with the type of prosthesis, the material used and the condition of the prosthesis.
Treatment of cancer pain in the elderly
In around 70% of cases, cancer pain in the elderly may be related to the disease itself, to its treatment or care, or to any of the other comorbidities common in this ageing population. Management of these patients is always comprehensive, combining general measures with specific drug and non-drug therapies. In all cases, cancer-specific treatment remains the reference treatment for pain linked to the disease itself. In cases where etiological treatment is not appropriate in the patient's management, or in any case to complement the analgesic effect of the latter, analgesics of the three WHO levels will be indicated. Their prescription, for purely symptomatic purposes, is based on the same rules in onco-geriatrics as for any other patient. Finally, the multiplicity of causes of such pain, and the many means available to manage it, must not interfere with a rigorous diagnostic and therapeutic approach. Thus, the need to know the precise and safe handling of analgesics or other associated molecules, which may be used to treat pain, must be taken into account.
Celiac disease and quality of life
Celiac disease (CD) is an autoimmune disorder whose management relies mainly on a strict gluten-free diet (GFD). This cross-sectional study, conducted on 100 Tunisian patients between October and December 2023, assessed the impact of CD on quality of life (QOL) using a generic questionnaire (the SF-12) and a specific questionnaire (the CDQ). The results show that CD significantly alters QoL, with low scores in both physical and mental domains. Factors associated with impaired HRQOL included non-university education and low socio-economic status. The study recommends the systematic use of HRQOL assessment tools and the implementation of supportive strategies to improve RSG adherence and patient management.
