Chronic Rhinosinusitis and Concomitant Medical Disorders
It is with great pleasure that we present to you this Special Issue of Medical Sciences. In this issue, we present a comprehensive and contemporary review of the medical comorbidities that contribute to chronic rhinosinusitis, and, conversely, how our interventions as otolaryngologists can impact those systemic conditions. Our understanding of chronic rhinosinusitis has evolved tremendously over the last two decades. As we have learned, chronic rhinosinusitis--a chronic inflammatory condition of the nasal cavity and paranasal sinuses--is often a local inflammatory response to a systemic or mucosal disorder. The underlying systemic medical conditions not only influence the presentation and diagnosis of chronic rhinosinusitis, but also modify the patients' response to medical and surgical interventions. Chronic rhinosinusitis associated with cystic fibrosis, for example, is a disorder quite distinct from that associated with aspirin-exacerbated respiratory disease. A clear understanding of the nuances that distinguish these unique and challenging disorders is critical for the practicing otolaryngologist. Equally important, however, is a clear understanding of the powerful benefits that our interventions as otolaryngologists can have for our patients' rhinologic and systemic health. Knowing that our rhinologic interventions might spare an asthma patient a trip to an emergency room or reduce lung infections in a cystic fibrosis patient makes this a very exciting time to be a rhinologist. Im ersten Abschnitt der theoretischen Grundlagen wird zun瓣chst ein historischer ?berblick 羹ber osteopathische Prinzipien, biologische Rhythmen und vorliegende Studien mit vergleichbarer Fragestellung gegeben. New Insights into Parvovirus Research
Viruses in the Parvoviridae family constitute one of the most diverse and intriguing fields of research. While they all share an ssDNA genome and a small capsid, they can differ widely in structure, genome organization and expression, virus-cell interaction, and impact on the host. Exploring such diversity and unraveling the inherent complexity in these apparently simple viruses is an ongoing endeavor and commitment for the scientific community. The translational implications of research on parvoviruses are relevant. Within the family, some viruses are important human and veterinary pathogens, in need of diagnostic methods and antiviral strategies; other viruses have long been studied and engineered as tools for oncolytic therapy, or as sophisticated gene delivery vectors, and can now display their wide and expanding applicative potential. This Special Issue of Viruses collects recent contributions in the field of parvovirus research, with a focus on new insights and research on unresolved issues, as well as new approaches exploiting systemic methodologies. Evolution, structural biology, viral replication, virus-host interaction, pathogenesis and immunity, and viral oncotherapy are a selection of the topics addressed in the issue that can be of relevance to the community involved in parvovirus research and of interest to a wider audience.
Treatment Strategies and Survival Outcomes in Breast Cancer
Treatment strategies for breast cancer are wide-ranging and often based on a multi-modality approach, depending on the stage and biology of the tumour and the acceptance and tolerance of the patient. They may include surgery, radiotherapy, and systemic therapy (endocrine therapy, chemotherapy, and targeted therapy). Advances in technologies such as oncoplastic surgery, radiation planning and delivery, and genomics, and the development of novel systemic therapy agents alongside their evaluation in ongoing clinical trials continue to strive for improvements in outcomes. Roles and Functions of ROS and RNS in Cellular Physiology and Pathology
Our common knowledge on oxidative stress has evolved substantially over the years and has been mostly focused on the fundamental chemical reactions and the most relevant chemical species involved in the human pathophysiology of oxidative stress-associated diseases. Thus, reactive oxygen species and reactive nitrogen species (ROS and RNS) were identified as the key players initiating, mediating, and regulating the cellular and biochemical complexity of oxidative stress either as physiological (acting pro-hormetic) or as pathogenic (causing destructive vicious circle) process. The papers published in this particular Special Issue of the Cells demonstrate the impressive pathophysiological relevance of ROS and RNS in a range of contexts, including the relevance of second messengers of free radicals like 4-hydroxynonenal, allowing us to assume that even more detailed mechanisms of their positive and negative effects lie in wait, and should assist in better monitoring of the major modern diseases and the development of advanced integrative biomedicine treatments.
Adipokines 2.0
Once viewed solely as fat storage cells, adipocytes and their adipokines have now been proven to be central for human health. Understanding that overweight and obesity may increase the risk for various diseases requires detailed characterization of adipokine function. Weight gain, weight regain, and fasting affect adipocyte health and accordingly their secretome. Different adipose tissue deposits exist and they vary in cellular composition and function. The evidence is strong of a role of adipokines in cancer, reproductive function, neurological diseases, cardiovascular diseases, and rheumatoid arthritis. Adipokines are considered useful biomarkers for adipose tissue and metabolic health, and may be used as diagnostic tools in rheumatoid arthritis, cancer, or sepsis. This book contains 10 original articles and 9 review articles focusing on these bioactive peptides. Several articles deal with chemerin, an adipokine discovered more than 20 years ago. Data so far have resulted in promising insights related to its biological function. We are only beginning to understand the multiple roles of chemerin, the mechanisms regulating its activity, and the signaling pathways used by this chemokine. Adipokine receptor agonists and antagonists may result in the formulation of novel drugs and ultimately may lead to new therapeutic targets to be used in clinical practice.

Self-Organizing Nanovectors for Drug Delivery
Nanomedicine represents one of the most investigated areas in the last two decades in the field of pharmaceutics. Several nanovectors have been developed and a growing number of products have been approved. It is well known that many biomaterials are able to self-organize under controlled conditions giving rise nanostructures. Polymers, lipids, inorganic materials, peptides and proteins, and surfactants are examples of such biomaterials and the self-assembling property can be exploited to design nanovectors that are useful for drug delivery. The self-organization of nanostructures is an attractive approach to preparing nanovectors, avoiding complex and high-energy-consuming preparation methods, and, in some cases, facilitating drug loading procedures. Moreover, preparations based on these biocompatible and pharmaceutical grade biomaterials allow an easy transfer from the lab to the industrial scale. This book reports ten different works, and a review, aiming to cover multiple strategies and pharmaceutical applications in the field of self-organizing nanovectors for drug delivery.
Natural Products and Drug Discovery
Natural products hold a prominent position in the current discovery and development of drugs and have diverse indications for both human and animal health. Plants, in particular, play a leading role as a source of specialized metabolites with medical effects. Other organisms, such as marine and terrestrial animals and microorganisms, produce very important drug candidate molecules. Specialized metabolites from these varied natural sources can be used directly as bioactive compounds or drug precursors. In addition, due to their broad chemical diversity, they can act as drug prototypes and/or be used as pharmacological tools for different targets. Some examples of natural metabolites that have been developed into useful medical drug are cardiotonic digoxin from Digitalis sp., antimalarial artemisinin from Artemisia annua, anti-cancer taxol from Taxus sp., or podophyllotoxin from Podophyllum peltatum, which served as a synthetic model for the anti-cancer etoposide. The study of natural products is still attracting great scientific attention and their current importance, as a valuable lead for drug discovery, is undebatable. I cordially invite authors to contribute original articles, as well as survey articles, that give the readers of Molecules **MOLECULES NEEDS TO BE ITALICIZED** updated and new perspectives on natural products in drug discovery, including but not limited to natural sources, identification and separation of bioactive phytochemicals, standardization, new biological targets, pre-clinical and clinical trials, pharmacological effects/side effects, and bioassays.
Kidney Inflammation, Injury and Regeneration
Acute kidney injury (AKI) is still associated with high morbidity and mortality incidence rates, and also bears an elevated risk of subsequent chronic kidney disease. Although the kidney has a remarkable capacity for regeneration after injury and may recover completely depending on the type of renal lesions, the options for clinical intervention are restricted to fluid management and extracorporeal kidney support. The development of novel therapies to prevent AKI, to improve renal regeneration capacity after AKI, and to preserve renal function is urgently needed. The Special Issue covers research articles that investigated the molecular mechanisms of inflammation and injury during different renal pathologies, renal regeneration, diagnostics using new biomarkers, and the effects of different stimuli like medication or bacterial components on isolated renal cells or in vivo models. The Special Issue contains important reviews that consider the current knowledge of cell death and regeneration, inflammation, and the molecular mechanisms of kidney diseases. Arthropod Venom Components and Their Potential Usage
Thousands of arthropod species, ranging from arachnids (spiders and scorpions) to hymenopterans (ants, bees, and wasps) and myriapods (centipedes), are venomous and use their venoms for both defense and predation. These venoms are invariably harmful to humans, and some may cause serious injuries, e.g., those from scorpions, spiders, and wasps. Arthropods' venoms are also known as rich sources of biologically active compounds and have attracted the attention of toxin researchers for years. In this century, venom component analysis has progressed considerable due to the advances in analytical techniques, in particular, mass spectrometry and next-generation deep (DNA and RNA) sequencing. As such, proteomic and peptidomic analyses using LC-MS have enabled the full analysis of venom components, revealing a variety of novel peptide and protein toxins sequences and scaffolds, potentially useful as pharmacological research tools and for the development of highly selective peptide ligands and therapeutic leads, like chlorotoxin. Due to their specificity for numerous ion-channel subtypes, including voltage- and ligand-gated ion channels, arthropod neurotoxins have been investigated to dissect and treat neurodegenerative diseases and control epileptic syndromes. This Special Issue collects information on such progress, encouraging contributions on the chemical and biological characterization of venom components, not only peptides and proteins, but also small molecules, their mechanisms of action, and the development of venom-derived peptide leads.

Marine natural products containing a heterocyclic moiety in their structure are present in a wide variety of sponges, corals, algae, and fungi. Many of them show important biological activities such as cytotoxic properties against several cancer cell lines. How Can Traditional Ethopian Medicine Help Against Diarrhea? Antidiarrheal and Antispasmodic Avtivities of Stephania Abyssinica (Minispermaseae)

The aim of this work was therefore to evaluate the antidiarrheal and antispasmodic activities ofthe aqueous and methanol extract of the root and leaf of S. abyssinica. Antidiarrheal activities were studied in mice using castor oil-induced diarrhea at doses of 25, 50,100, and 200 mg/kg body weight. The extracts significantly prolonged the time of diarrheal induction, increased diarrhea free time, reduced the frequency of diarrhea episodes, decreased the weight of stool, and decreased general diarrheal score in a dose dependent way. This special issue entitled "Soft and hard tissue regeneration" will cover both periodontal and implant therapies. Regenerative periodontal treatment goal is to restore functional periodontal support offering a valuable treatment alternative even for teeth with large periodontal destruction, which may be successfully treated and maintained in health for long periods. In most cases where teeth are extracted for periodontal reasons, implant therapy will demand large bone augmentation procedures. Lack of sufficient bone volume may prevent placement of dental implants. In extreme cases, large bone reconstruction is indispensable before implant placement can be performed. Although, most bone grafts are only able to fill and maintain a space, where bone regeneration can occur ("osseoconductive"), the ideal bone graft will also promote osseous regeneration ("osseoinductive"). Several bone augmentation procedures have been described, each, presenting advantages and shortcomings. Success of bone augmentation procedures depends on the presence of bone forming cells, primary wound closure over the augmented area, space creation and maintenance where bone can grow and proper angiogenesis of the grafted area. Factors that influence the choice of the surgical technique are the estimated duration of surgical procedure, its complexity, cost, total estimated length of procedure until the final rehabilitations may be installed and the surgeons' experience. This special issue will have a definite clinical orientation, and be entirely dedicated to soft and hard tissue regenerative treatment alternatives, both in periodontal and implant therapy, discussing their rationale, indications and clinical procedures. Internationally renowned leading researchers and clinicians will contribute with articles in their field of expertize.

In the recent years, the interest in micron and submicron systems in pharmacy as surged. Micron system comes in range of μm and submicron in nm. Typically colloid is an intermediates size between molecular range and coarse range. There has been a considerable research interest in the area of drug delivery using particulate systems. Particulate systems used as a physical approach to alter and improve the pharmacokinetic and pharmacodynamic properties of various types of drug molecules. Griseofulvin is an important agent in the treatment of dermatophytosis. Results of present work lead to a conclusion that reduced size of the drug particles with presence of the surfactants and polymers on the surface of griseofulvin microparticles are responsible for meteoric rise in solubility and dissolution velocity. Surfactants play an important role in the solubility of the drugs by increasing the wettability due to adsorption on the surfaces. Microparticles have been used as a physical approach to alter and improve the pharmacokinetic and pharmacodynamic properties of various types of drug molecules due to increased surface area by several times.

Biomedical Insights that Inform the Diagnosis of ME/CFS
Myalgic encephalomyelitis/chronic fatigue syndrome (ME/CFS) is a severe chronic health condition that is often misunderstood or ignored by health establishments. The lack of definitive diagnostic markers to separate ME/CFS patients from the healthy population as well as from other chronic disorders is problematic for both health professionals and researchers. A consortium of Australian researchers gathered to systematically understand ME/CFS, ranging from a deep analysis of clinical and pathology data to metabolomic profiles and the investigation of mitochondrial function. From this broad collaboration, a number of compelling insights have arisen that may form the basis of specific serum, blood, and/or urinary biomarkers of ME/CFS. This Special Edition reports on a conference centred on these biomedical discoveries, with other contributions, with a translation focus for predictive markers for ME/CFS diagnosis. By supporting health professionals with developments in diagnostics for this condition, the patients and their families will hopefully benefit from an improved recognition of the biomedical underpinnings of the condition and will be better able to access the care that is urgently required. This Special Edition contains a mix of speaker submissions and other accepted manuscripts that contributed to our objective of advancing biomedical insights to enable the accurate diagnosis of ME/CFS.
Recent Development of Electrospinning for Drug Delivery
Several promising techniques have been developed to overcome the poor solubility and/or membrane permeability properties of new drug candidates, including different fiber formation methods. Electrospinning is one of the most commonly used spinning techniques for fiber formation, induced by the high voltage applied to the drug-loaded solution. With modifying the characteristics of the solution and the spinning parameters, the functionality-related properties of the formulated fibers can be finely tuned. The fiber properties (i.e., high specific surface area, porosity, and the possibility of controlling the crystalline-amorphous phase transitions of the loaded drugs) enable the improved rate and extent of solubility, causing a rapid onset of absorption. However, the enhanced molecular mobility of the amorphous drugs embedded into the fibers is also responsible for their physical-chemical instability. This Special Issue will address new developments in the area of electrospun nanofibers for drug delivery and wound healing applications, covering recent advantages and future directions in electrospun fiber formulations and scalability. Moreover, it serves to highlight and capture the contemporary progress in electrospinning techniques, with particular attention to the industrial feasibility of developing pharmaceutical dosage forms. All aspects of small molecule or biologics-loaded fibrous dosage forms, focusing on the processability, structures and functions, and stability issues, are included.
Gastrointestinal Variables and Drug Absorption
This book presents some of the state-of-the-art methods for the study of the gastrointestinal variables affecting oral drug absorption. Practical applications of new in vitro release/dissolution methods are presented, as well as in vitro permeability studies to explore segmental differences. The application of MRI methods for the study of colon physiology is presented to illustrate its potential applications in controlled release dosage form design. Some examples of successful in vitro-in vivo correlations show how implementing the gastrointestinal physiological variables in the new in vitro methods can improve the predictions of in vivo drug product performance. Transmucosal Absorption Enhancers in the Drug Delivery Field
Development of strategies to assist the movement of poorly permeable molecules across biological barriers has long been the goal of drug delivery science. In the last three decades, there has been an exponential increase in advanced drug delivery systems that aim to address this issue. However, most proprietary delivery technologies that have progressed to clinical development are based on permeation enhancers (PEs) that have a history of safe use in man. This Special Issue entitled "Transmucosal Absorption Enhancers in the Drug Delivery Field" aims to present the current state-of-the-art in the application of PEs to improve drug absorption. Emphasis is placed on identification of novel permeation enhancers, mechanisms of barrier alteration, physicochemical properties of PEs that contribute to optimal enhancement action, new delivery models to assess PEs, studies assessing safety of PEs, approaches to assist translation of PEs into effective oral, nasal, ocular and vaginal dosage forms and combining PEs with other delivery strategies.
Cell-Free Nucleic Acids
The deficits of mammography and the potential of noninvasive diagnostic testing using circulating miRNA profiles are presented in our first review article. Exosomes are important in the transfer of genetic information. The current knowledge on exosome-associated DNAs and on vesicle-associated DNAs and their role in pregnancy-related complications is presented in the next article. The major obstacle is the lack of a standardized technique for the isolation and measurement of exosomes. One review has summarized the latest results on cell-free nucleic acids in inflammatory bowel disease (IBD). Despite the extensive research, the etiology and exact pathogenesis are still unclear, although similarity to the cell-free ribonucleic acids (cfRNAs) observed in other autoimmune diseases seems to be relevant in IBD. Liquid biopsy is a useful tool for the differentiation of leiomyomas and sarcomas in the corpus uteri. One manuscript has collected the most important knowledge of mesenchymal uterine tumors and shows the benefits of noninvasive sampling. Microchimerism has also recently become a hot topic. It is discussed in the context of various forms of transplantation and transplantation-related advanced therapies, the available cell-free nucleic acid (cfNA) markers, and the detection platforms that have been introduced. Ovarian cancer is one of the leading serious malignancies among women, with a high incidence of mortality; the introduction of new noninvasive diagnostic markers could help in its early detection and treatment monitoring. Epigenetic regulation is very important during the development of diseases and drug resistance. Methylation changes are important signs during ovarian cancer development, and it seems that the CDH1 gene is a potential candidate for being a noninvasive biomarker in the diagnosis of ovarian cancer. Preeclampsia is a mysterious disease--despite intensive research, the exact details of its development are unknown. It seems that cell-free nucleic acids could serve as biomarkers for the early detection of this disease. Three research papers deal with the prenatal application of cfDNA. Copy number variants (CNVs) are important subjects for the study of human genome variations, as CNVs can contribute to population diversity and human genetic diseases. These are useful in NIPT as a source of population specific data. The reliability of NIPT depends on the accurate estimation of fetal fraction. Improvement in the success rate of in vitro fertilization (IVF) and embryo transfer (ET) is an important goal. The measurement of embryo-specific small noncoding RNAs in culture media could improve the efficiency of ET.
