Metabolic Features and Nutritional Interventions in Chronic Diseases
A growing body of research indicates an extensive metabolic dysfunction in a spectrum of chronic diseases, comprising aging, cancer, and diseases affecting the cardiovascular system, liver, and kidneys. The phenotypes associated with metabolic dysfunction vary depending on disease type and stage. In turn, the altered metabolism drives disease pathology. Consequently, the nutritional modulation of metabolic status in chronic disease is widely acknowledged to reduce disease incidence. For instance, tailored nutritional intervention in cancers with altered metabolism determines the improved therapeutic responses. Additionally, counteracting muscle loss and osteoporosis are clinical goals of nutritional support in an aged population. Mechanistically, bioactive nutrients or tailored nutritional regimen modulate metabolism through mitochondrial homeostasis, immuno-regulation, as well as epigenome regulation to impact disease progression. This reprint aims to be an interdisciplinary platform that covers (1) characterization of metabolic features and/or nutritional requirement in a certain type of chronic disease; (2) defining the effects and mechanism of a specific nutrient or nutritional intervention on metabolism; (3) development of new nutritional strategy that improves disease outcome by altering metabolism.
Morphofunctional Nutritional Assessment in Clinical Practice
The diagnosis of malnutrition is difficult because it does not depend only on weight at a given time, but also on its evolution and the underlying pathological situations. Therefore, the clinical use of body composition measurements is essential for adequate assessment of this malnutrition, especially in the evaluation of muscle mass and function. In this context, nutritional assessment can no longer be based on the determination of classical anthropometric measurements. The concept of morphofunctional nutritional assessment postulates that the diagnosis and monitoring of nutritional status must be carried out using techniques and biomarkers that determine the evaluation of intake, anthropometry, body composition, muscle strength, and function that include techniques such as bioelectrical impedance analysis or nutritional ultrasound, and new biological parameters too. This Special Issue aims to show the implementation of this new concept of nutritional evaluation in the management of patients and in clinical research in nutrition.

Comprehensive Review on Upper Tract Urothelial Carcinoma
Upper tract urothelial carcinoma (UTUC) is a relatively uncommon type of cancer; however, its incidence has risen in the past few decades, mainly due to improved detection through advanced imaging and endoscopic technologies. It is an aggressive tumor with up to 60% and 25% of patients presenting with non-organ confined and metastatic diseases at diagnosis, respectively. Radical nephroureterectomy with bladder cuff excision is the gold standard for the management of UTUC, regardless of tumor location. Nevertheless, kidney-sparing techniques, including endoscopic ablation and segmental ureterectomy, have become the preferred options in select patients, especially those with low-risk disease. Despite surgery with curative intent, long-term cancer-specific survival is still low, predominantly due to undetected microscopic metastasis. The management and prognosis of UTUC have improved in recent years, given the widespread use of minimally invasive approaches and novel neoadjuvant and adjuvant systemic therapies.This Special Issue, titled "Comprehensive Review on Upper Tract Urothelial Carcinoma: An Update in 2023", highlights different aspects of the multidisciplinary management of UTUC, including imaging, novel diagnostic markers, neoadjuvant and adjuvant therapies, and surgical interventions. Pharmaceutical Surge Requirements for the Prime Vendor Program-Europe in Support of Military Operations Other Than War
The U.S. military in Europe often provides medical support to military operations other than war due to unstable situations in that region of the world. The increasing support to peace operations and humanitarian assistance have led the military medical units in Europe to reexamine the pharmaceuticals traditionally maintained to support surges. This research was a qualitative, exploratory study using multiple-case study strategy to identify the specific pharmaceuticals utilized in the initial stages of military operations other than war primarily peacekeeping, peace enforcement and humanitarian assistance. The operations chosen as the units of analysis were Operation Provide Promise (Croatia), Operation Support Hope (Rwanda), and Operation Joint Endeavor (Bosnia-Herzegovina). The results of the research show that some unique surge requirements exist for military operations other than war. Taking advantage of existing clauses in its Prime Vendor contracts, the military medical community in Europe can position itself to adequately support surges for MOOTW. Immediate and Delayed Drug Therapy Effects on Low Dose Sarin Exposed Mice Myocardial Performance
Recent studies have shown that a single asymptomatic dose exposure to the nerve agent sarin can lead to long term cardiac dysfunction. This study looked at immediate and delayed treatment therapies post exposure on cardiac function. The mice were studied using electrocardiography (QTc, STc, and Tc intervals) and histological techniques (hematoxylin and eosin staining and brain natriuretic peptide (BNP) staining). Male C57BL/6J mice were injected with sarin (0.4LD₅₀) and studied up to 11 weeks. Mice which were given a single dose of the nerve agent treatment atropine/2PAMCL, showed significantly higher cell size and BNP levels than the control (p = 0.001) and increased QTc and Tc intervals, all of which are indicators of cardiac insult . Mice which received drug treatments after the onset of cardiac remodeling showed increased (thus worsening) normalized heart size for isoproterenol (β agonist) treated mice (p = 0.0018) versus saline treated mice along with inverted T-waves pre cardiac stressing. Propranolol (β blocker) treated mice showed only T-wave depression and no difference in normalized heart size versus the saline group. These results suggest that standard treatment methods for nerve agent exposure are not effective in preventing long term cardiac dysfunction and in fact cause them.This work has been selected by scholars as being culturally important, and is part of the knowledge base of civilization as we know it. Applications of Antioxidant Nanoparticles
Reactive oxygen species (ROS) are essential in regulating various physiological functions. However, the overproduction of ROS can be related to many diseases. Antioxidant administration represents an effective strategy to treat oxidative stress. In recent decades, nanotechnology and nanoparticles have received great interest and seen applications in many fields, from pharmaceutics to cosmetics and food. For instance, lipid nanoparticles, due to their "green" characteristics related to their biodegradation and non-toxicity, have received significant attention as a carrier for antioxidant molecules. Over the past few decades, significant achievements have been made in the development of antioxidant nanotherapies, exhibiting multiple advantages.This Special Issue collects recent advances in the design, production, characterization, use, and effect of antioxidant nanoparticles, including polymeric or lipid-based nanoparticles, ROS-scavenging inorganic nanoparticles, organic nanoparticles with intrinsic antioxidant activity, and drug-loaded antioxidant nanoparticles, to be administered by different routes. In other words, articles focus on the use of antioxidant nanoparticles in the biomedical, pharmaceutical, and cosmetic field, as well as in the food sector.
An Update on Mechanisms and Treatments for Thyroid Cancer in Current Oncology
The incidence rate of thyroid cancer (TC) has risen sharply in recent decades on a global scale. It is the most common malignant tumor of the endocrine system, accounting for 4.1% of all new cancer cases worldwide, with age-standardized incidence rates (ASIRs) of 9.1 per 100,000 in 2022, establishing itself as a significant threat to population health. Although TC generally exhibits a better prognosis compared to other cancers, poorly differentiated thyroid cancer (including poorly differentiated TC, PDTC and anaplastic TC, ATC, two subtypes of TC) displays the shortest average survival time. The precise cause of TC's high incidence remains unclear, particularly the molecular mechanisms underlying its development. Consequently, the primary approaches for treating TC involve surgery, often supplemented by 131I therapy, radiotherapy, endocrine therapy, and biomarker-targeted therapy. Unfortunately, these treatments, except for targeted biological therapies, often result in irreversible harm to patients, such as thyroidectomy or radiation injuries. While targeted therapy can be expensive, and current TC biomarkers remain limited, their effects are still being explored. Additionally, aside from surgical resection, alternative treatment methods remain subject to ongoing controversy.
